Intravial distribution of moisture during the secondary drying stage of freeze drying.

Particularly for proteins that may be damaged by overdrying, the distribution of residual moisture in the dried product may be as important as the mean water content. Protein at the top of the cake could be "overdried" even though the mean water content was optimal. Theoretical studies suggest that, particularly for large fill depths, the residual moisture near the top of the cake is much lower than the moisture near the bottom, at least during and immediately after freeze drying. This work is an experimental study of the distribution of moisture within a vial as a function of time in secondary drying (mean residual water content of about 3%-6%) for dextran, human serum albumin, and bovine somatotropin. A core sample of the dried product was taken and sectioned into "top," "middle," and "bottom" sections. These three "core sections" and the remaining "outer" section (i.e., near the vial wall) were assayed for moisture. In general, the moisture content in the top section is less than the moisture content in the bottom section, but the "outer" section is consistently found to be lowest in moisture content. This observation suggests that drying was faster along the walls of the vial than in the core region, resulting in a highly curved ice-vapor interface. It is proposed that faster drying along the vial walls is a result of the observed product shrinkage during drying which provides a low resistance pathway for vapor escape along the vial wall. Product temperature measurements support the above speculation.

Screening for pregnancy and contraceptive use among women admitted to a Denver detoxification center.

OBJECTIVE: To determine the prevalence of pregnancy and contraceptive use among female inpatients at a Denver detoxification center who were at risk of becoming pregnant. METHODS: The authors administered surveys and pregnancy tests to 373 female admissions to the Denver CARES detoxification center between March and September 1996. RESULTS: Most were admitted for alcohol-related reasons, and most reported previous pregnancies. Seven percent of admissions had positive pregnancy tests, and 56% of admissions reported not using contraceptives with every sexual encounter. CONCLUSIONS: Routine assessment of pregnancy status and contraceptive use in women admitted for detoxification, along with on-site family planning services, may be important components of efforts to prevent adverse outcomes associated with substance abuse during pregnancy, including fetal alcohol syndrome.

Arrhythmogenic right ventricular dysplasia: electrophysiologic and morphologic features in nine patients.

Arrhythmogenic right ventricular dysplasia is a potentially life-threatening disorder, not previously well described in India. We analysed the electrocardiographic, electrophysiologic, angiographic, signal-averaged electrocardiogram and magnetic resonance imaging features of nine patients having arrhythmogenic right ventricular dysplasia at our centre. There were seven males and two females, aged 25 to 55 years. Eight patients presented with sustained monomorphic ventricular tachycardia. The electrocardiogram showed prolonged QRS duration in four, T inversion in leads V2-V3 in four, and epsilon wave in two patients. Abnormal late potentials were present in eight patients. During electrophysiologic study in seven patients, 13 different VTs were induced. Seven patients had right ventricular dilatation and dysfunction and left ventricular involvement was seen in three patients. Eight patients were treated with amiodarone, including one who received an implantable cardioverter-defibrillator. At a follow-up of 8.6 +/- 5.3 months, there was no death, while one patient had recurrence of ventricular tachycardia. Arrhythmogenic right ventricular dysplasia seems to predominantly affect middle-aged men, the presentation most often being sustained monomorphic ventricular tachycardia. Depolarisation and/or repolarisation abnormalities in the electrocardiogram are commonly present. Multiple ventricular tachycardia morphologies during electrophysiologic study and abnormal right ventricular angiograms are usually observed.

Nurse-led diabetes clinics benefit black and Asian patients.

There is strong epidemiological data that shows that prevalence of diabetes varies with ethnic origin. This article describes the establishment of diabetic clinics within general practices in an attempt to identify and meet the needs of minority ethnic groups in relation to this condition. A future article will describe opportunistic screening of the target population.
