An unusual community outbreak of influenza A.

The Hong Kong H3N2 subtype of influenza A virus appeared in 1968 and since then has caused epidemics of varying degrees of severity. We describe a community outbreak of influenza A H3N2 which occurred in members of a bowls club in an English rural village in late April 1989. The explosive onset, high attack rate (34/41 = 83 per cent) in those exposed, and the clinical presentation initially suggested a toxic or allergic aetiology. Twenty-three persons consulted their general practitioners; before the diagnosis was made all cases were considered to merit antibiotic therapy and 17/23 were prescribed steroids or bronchodilators on account of persisting severe wheeze and chest tightness. One of the 23 was admitted to hospital. There were no deaths. Influenza vaccination is recommended for people at special risk but protective efficacy is relatively low and short-lived. Only two of the group had received influenza vaccination since the beginning of October 1988 and both became symptomatic. The use of amantadine for the prevention and early treatment of influenza A in selected situations merits serious consideration, provided the diagnosis is made sufficiently early.

Sparing effect of hemiplegia on scleroderma.

The paretic limb is spared in patients who develop rheumatic diseases after a hemiplegic stroke. This has been described previously in rheumatoid arthritis, gout, and osteoarthritis. A similar presentation in a case of scleroderma is described in this report. Scleroderma skin changes are absent in the completely paretic limb and were markedly reduced in the weak left leg. Inflammation may be modified either by neuropeptides or by an anatomical neurological lesion and this may explain the phenomenon.

An ultracompliant glass microelectrode for intracellular recording.

We describe the simple fabrication of a glass microelectrode that was designed for the stable impalement of contracting isolated cardiac preparations. The electrodes can be made with the aid of a standard pipette puller. The design uses a long-shanked glass electrode that is stepped below the shoulder at 70 degrees to the shaft and again near the tip. The electrodes are of marginally higher resistance than equivalent length straight electrodes (34.9 vs. 26.0 M omega). The stepped design imparts a low stiffness to the electrode in the vertical and horizontal planes, and the mechanical basis for this performance is described and discussed with regard to measured behavior. The duration of impalements with stepped electrodes is significantly longer than for straight electrodes and can usually be maintained for indefinite periods with minimal damage to small, highly contractile muscle preparations.

The calcium endocrine system.

The role of calcium as a modulator of a large number of physiological processes is well known. It is not surprising that the two principally in the understanding of its subcellular physiology and molecular biology. There is new knowledge on hepatocellular uptake of vitamin D. The subsequent hepatic conversion of vitamin D to 25 hydroxy vitamin D may indeed be under the feedback control of the active vitamin D, 1,25 dihydroxyvitamin D. Vitamin D now also appears to have many new and diverse functions some of which already have clinical application.

Human seminal plasma allergy in India.

Human seminal plasma allergy (HSPA) in women has been documented in the West. We describe here the first case of HSPA reported in India. An 18-year-old married woman presented with a 2-year history of episodic wheezing dyspnea. She had always had local postcoital symptoms since her first intercourse. Systemic symptoms developed subsequently. Despite more than 2 years of unprotected coitus, the patient had not conceived. The intradermal tests with seminal plasma antigen prepared from her husband's semen and from a healthy volunteer were markedly positive in the patient. Similar tests on her husband were negative. HSPA, especially the local forms, may be difficult to identify in our social conditions.

Neonatal listeriosis: a report of seven cases.

Seven neonates with listeriosis admitted to Farwaniya Hospital, Kuwait, are reported. Six had the meningitic type and one the septicaemic type. Serotyping showed 1/4b in all neonates. The response to 2 weeks ampicillin and amikacin was excellent with no mortality or morbidity.

Surgical management of the cardiovascular complications of Kawasaki's disease.

During a 31-year period, 54 patients were treated for Kawasaki's disease at the Texas Heart Institute and Texas Children's Hospital. Classically, the illness is characterized by prolonged fever, conjunctivitis, oral mucosal inflammation, exanthema, and later skin desquamation and cervical lymphoadenopathy. Seventy percent of patients have electrocardiographic abnormalities consisting of prolongation of the PR and QT intervals and ST-T wave changes. Most deaths occur within 1 to 2 months after onset of the disease. The risk of coronary abnormalities and cardiac death appear to be higher in those patients under 1 year of age with prolonged fever, elevated white blood count, and erythrocyte sedimentation rate.
