Bilateral Ota naevus.

We present the case of a 22-year-old woman, who had presented since the age of 15 a pale-blue spot spread on the right-hand side of her forehead and in her bulbar conjunctiva (first and second branches of the trigeminus nerve), consistent with Ota naevus. A few years later another with similar characteristics appeared on the other side of her forehead, cheek and sclera. No deafness, neurological defect nor visual loss were detected. We comment on the rarity of this case because the patient is Caucasian and also we explain the main complications derived of this disease and consider the therapeutic options.

[Contribution of radioguided detection and selective biopsy of the sentinel lymph node to staging in 100 patients with cutaneous melanoma].

OBJECTIVE: A selective sentinel node (SN) biopsy appears to be an alternative to conventional lymph node dissection for staging patients with cutaneous melanoma. This study has aimed to analyze our experience in the localization of the sentinel node with a probe detector and lymphoscintigraphy and its utility in the staging of this disease. MATERIAL AND METHODS: 100 patients, 56 female and 44 male, 51.5 31.5 mean age, diagnosed of cutaneous melanoma were studied. The lymphoscintigraphy was performed in all cases to detect the area of lymphatic drainage after peritumoral intradermal administration of 600 uCi of 99mTc-colloidal sulfur, and planar images were acquired every 10 minutes until activity was detected in these areas. Afterwards, the intraoperative localization of the sentinel node with a probe detector was performed and the selective biopsy of the node was sent to the Pathology Department for its histologic study. RESULTS: The lymphoscintigraphy study was positive in 99 of the 100 cases and it was identified intraoperatively in 98 cases. The histologic analysis was negative in 78.9% and positive in 21.1% of the nodes. CONCLUSIONS: Radioisotopic lymphography and intraoperative probe detection are two very useful techniques for locating the sentinel node and staging patients with cutaneous melanoma. Many patients are benefiting from the selective biopsy of the sentinel node because of the decreased post-surgical morbidity and better staging of the cutaneous melanoma.

[Skin aging and smoking].

Facial wrinkling is a marker of skin ageing. The association between smoking and facial wrinkling has been previously studied. Nevertheless, there are no reports assessing the role of sun exposure and smoking in an area with a great proportion of sunny days. Smoking habits were investigated in 282 healthy individuals. Facial wrinkles were measured according to the Daniell scale at the external canthus. A logistic regression model was developed controlling for age, sex, sun exposure, smoking habit and searching for the interaction between sun exposure and smoking. A statistically significant risk was found for smoking habit (OR = 3.1; 95% CL 1.28-7.76; p = 0.008), sun exposure (OR = 1.50; 95% Cl 1.25-1.80; p = 0.05), and age (OR = 1.18; 95% Cl 1.13-1.23; p = 0.024) for facial wrinkling. These results show the evidence of an accelerated skin ageing in smokers. A clear explanation given to smokers could be a useful strategy to persuade them to leave smoking.

[Lymphomatoid papulosis evolving into malignant lymphoma. Clinico-pathological and immunohistochemical study of 3 cases].

Lymphomatoid papulosis is a disorder characterized by recurrent skin lesions with histological features suggestive of malignant lymphoma. In most cases the cutaneous lesions heal spontaneously but the course of the disease is long-lasting and an evolution into a Hodgkin's disease and non-Hodgkin lymphomas may be seen. We report herein the clinical, pathological and immunohistochemical study of three patients having a long-standing lymphomatoid papulosis, which turned into Hodgkin's disease, mycosis fungoides and nodular paragranuloma, respectively. Immunohistochemical analysis showed that the immunophenotype of atypical cells in lymphomatoid papulosis was similar to that observed in Reed-Sternberg cells in Hodgkin's disease and the neoplastic cells of mycosis fungoides. However, the immunohistochemical profile of cells in lymphomatoid papulosis differed from those observed in cells of nodular paragranuloma, developed by one of the three patients. The relationship between lymphomatoid papulosis and malignant lymphomas associated to lymphomatoid papulosis is discussed. The results show that no definitive criteria can be infered from an immunohistochemical study in lymphomatoid papulosis, in predicting the clinical evolution of the disease.

[Epidemiologic aspects of leprosy in the province of Jaen].

Jaen is the province with the greatest number of patients with lepra. This study analyzes the prevalence of the disease in Jaen during 1984 and 1985. At that time, all the patients known to the health and consume provincial delegation of Jaén were studied. The 305 patients included received a protocol aimed to study the different risk factors (age, clinical form, the people living together, socioeconomic standing, ethnic, etc ...). The disease shows to have a highly significant and special predilection in gypsies and it is located in a significant geographical area in the province. Lepra did not show a rural distribution, however it prevails in middle size urban nuclei. Although all patients belonged to a low social class, the disease did not show to exhibit any association among the ecology and the income per inhabitant.

Zinc deficiency dermatitis in parenteral nutrition: an electron-microscopic study.

Two cases of zinc deficiency dermatitis (acute phase) on parenteral nutrition are reported. Subepidermal blisters caused by a decrease in the number of desmosomes and hemidesmosomes located between the discontinuous basal membrane and cell membranes are shown. The collagen fibers presented changes which probably are responsible for the breaking or fragility of the capillaries. These facts went unnoticed in previous studies.

Awareness, attitudes and behaviour of teenagers to sunlight.

Most campaigns for the prevention of skin cancer have detected more new cases and decreased the number of advanced cancers. Since the incidence of skin cancer continues to increase, however, we believe that primary prevention is the best way to control it. It must be kept in mind that sunlight exposure is the main changeable risk factor for skin cancer and that this exposure is most significant in childhood and adolescence. Our aim in this study was to evaluate the need for a campaign and design one if necessary. We therefore proposed to determine the level of awareness and the behaviour of students with respect to sunlight exposure. We surveyed 628 teenage students from 9 high schools in the city of Granada (Spain). The questions were grouped into four sections: 1. Relationship Sun and Skin, 2. Relationship Sun and Environment, 3. Relationship Sun and Health, 4. Evaluation of Attitudes and Behaviour. More than 60% of the students gave satisfactory answers with regard to awareness, in contrast to the responses for attitudes and behaviour. Prevention campaigns for students are definitely necessary, keeping in mind in their design that a high level of awareness does not translate into healthy habits with regard to sunlight. Intervention to change behaviour patterns should be the main goal of primary prevention campaigns.
