Reconstitution of intestinal Na(+)-phosphate cotransporter.

The rabbit intestinal brush-border membrane Na(+)-phosphate cotransporter was purified from sodium dodecyl sulfate (SDS)-brush-border membrane vesicles (BBMV) protein (SDS-treated Ca(2+)-precipitated BBMV) by a three-column chromatography protocol. The purification included a preparative scale chromatofocusing chromatography column over the pH range from 7.4 to 4 after solubilization in 3-[(3-cholamidopropyl)-diamethylammonia]-1-propanesulfonate (CHAPS), a chromatofocusing column over the pH range from 5.6 to 4 after solubilization in n-octyl glucoside, and gel filtration chromatography on a Sephacryl S-200 column. Verification of Na(+)-phosphate cotransporter purification involved substrate affinities, substrate stoichiometry, and inhibitor sensitivity after proteoliposome reconstitution and SDS-polyacrylamide gel electrophoresis (PAGE). After gel filtration Na(+)-dependent phosphate uptake was 3,300-fold enriched compared with the cell homogenate. A single 130-kDa polypeptide was visualized by SDS-PAGE under reducing conditions using silver stain. The coenrichment of this 130-kDa polypeptide and proteoliposome reconstituted Na(+)-dependent phosphate uptake suggest that the intestinal brush-border membrane Na(+)-phosphate cotransporter has been purified and proteoliposome reconstituted.

Medical Illness Simulating Trauma (MIST) syndrome: case reports and discussion of syndrome.

Medical Illness Simulating Trauma (MIST) Syndrome is a clinical syndrome characterized by pain, discomfort, or the observation of a mass that is presented to the physician as the result of trauma. The underlying cause of the patient's symptom(s), however, is a medical condition unrelated to the trauma. The reported trauma may not actually have occurred and if it did, is only coincidentally related to the onset of symptoms, which bring attention to a previously unrecognized or ignored pain, discomfort, or mass. The incidence of MIST Syndrome is unknown. MIST Syndrome is distinguished from other conditions and syndromes presenting with histories that are inconsistent with subsequent findings by the lack of intent to deceive the physician or misrepresent the physiologic nature of the underlying medical condition. This syndrome can be a source of error or delay in diagnosis and treatment.

[Psoriasis in a female chimpanzee].

Psoriasis, a widespread genodermatosis in Homo sapiens, also appears in primates. We report on a female chimpanzee in Leipzig Zoo. After years of captivity the animal developed erythematosquamous, highly hyperkeratotic, lesions, some confluent, on the knees, elbows, back of the hands and feet and on the forearms and the seat, which showed histologically characteristic features of psoriasis. It may be that both previous infections and psychic stress resulting from social isolation had caused eruption of the disease. In the literature single cases of psoriasiform dermatoses have also been described in other species of monkeys and even in a springer spaniel. Nonetheless, the search for an animal model of psoriasis vulgaris is still going on.

[Cefoxitin in the preventive treatment of infectious complications in high-risk gastro-jejuno-colic and biliary surgery].

Cefoxitin is a bactericidal beta lactam antibiotic of the cephamycins group, with wide antimicrobial efficacy and active against both aerobes and anaerobes. In this study, 58 patients were divided in four groups according to the site of the disease (stomach, gallbladder and biliary tract, jejunum ileum and colon-rectum). The efficacy of cefoxitin in the prophylaxis of surgical infections was assessed for each group and for all patients. When compared with the combination of antibiotics used previously, cefoxitin led to a reduction of the mean duration of fever (7.2 and 5 days respectively with ampicillin/cloxacillin and lincomycin/gentamycin, 3.2 days with cefoxitin). Cefoxitin also led to a reduction in the incidence of surgical infections (50% with ampicillin/cloxacillin, 9.19% with lincomycin/gentamycin, 8.6% with cefoxitin) and in the incidence of localized surgical infections (29.6%, 20.3%, 8.6% respectively). In this study, cefoxitin has been found to be very useful and well tolerated in the prophylaxis of surgical infections.

[Biomechanico-clinical interpretation of firearm wounds. General problems (III). A propedeutic ABC on terminal ballistics].

The surgeon should have a comprehensive knowledge of firearms as thermochemico-ballistic machines in order to come to grips in an adequate manner with the wounds they inflict on the human body. The traces left on the body to all intents and purposes embrace a cultural "cul de sac" which risks being defrauded of most of its content by a lack of those propedeutics elements which painstaking reflection is capable of affording us. Only the interpretation of each individual ballistic phase, both as a complete phenomenon in itself and as a component in a single articulated complex, can offer such elements: credit is due at least for a wholehearted commitment in this direction, while never losing sight at any time of the clinical perspective of such wounds.

Evaluation of potential heart affine compounds by use of the isolated perfused rat heart.

99mTc-organocation complexes were screened for heart affinity using the isolated perfused rat heart. The 99mTc-I compounds are better extracted by the heart and retained in it than the 99mTc-III compounds. 99mTcCl2(DMPE)2+ shows low uptake (15%) and fast washout, but no impairment of uptake after incubation with human serum albumin(HSA), with a modified preparation, initial uptake is not much better (18%) but washout is clearly diminished. 99mTc(DMPE)3+ is taken up to a greater extent (33%) if administered solely, but when given in conjunction with HSA, uptake decreases to about 4% because of strong affinity of the complex to the protein, but this compound persists well in the tissue. Detergents like tween 80 and dioctylsulphosuccinate are able to cause higher uptake of 99mTc(DMPE)3+ even in the presence of HSA, thus indicating a diminished binding of the compound to the protein. 99mTc(CN-t-butyl)6+ shows very high uptake (80%) by the heart, which is only moderately diminished by addition of HSA (to 38%), and persists excellently in the myocardium. Basic features of the compounds such as accumulation in and elimination from the myocardium as well as plasma protein binding and their mutual relationships are well reflected by the isolated heart model.

Preparation of [99mTc(DMPE)2Cl2]+ for myocardial imaging by using Fe-(DMPE) complexes.

The preparation of [99mTc(1,2-bis(dimethylphosphino)ethane)2Cl2]+ ([99mTc(DMPE)2Cl2]+) for imaging the myocardium is investigated. Starting from Fe(III)- or Fe(II)-DMPE and 99mTcO4- different preparation variants are compared. In these reactions either ascorbic acid or DTPA serves as an agent for complexing iron. A simple procedure using lyophilized initial components is represented. Its application yields [99mTc(DMPE)2Cl2]+ with a radiochemical purity of more than 95%. Organ distribution studies performed in rats emphasize the high myocardial accumulation of this preparation.

Emergency department management of the sexual assault victim.

The optimal management of the sexual assault victim involves a multidisciplinary effort on the part of all legal, police, medical, and support personnel who interface in the emergency department. History, general physical examination, and pelvic examination are performed methodically, keeping in mind that the primary goal is to tend to the patient's medical needs. The gathering of evidence proceeds simultaneously with the physical examination. Evidence to be obtained and techniques are reviewed. Treatment entails attention to physical injuries, potential venereal disease and pregnancy, and psychiatric intervention. Management of the male rape victim or child victim of sexual abuse requires special attention to the peculiarities of those problems.

The mechanism of injury, diagnosis, and treatment of maxillofacial, laryngeal, tracheal, and bronchial trauma are considered. Also discussed is pulmonary barotrauma.

Botulism.

Clostridium botulinum is ubiquitous in the environment, yet symptoms of botulism occur in humans only if toxin A, B, or E is ingested, absorbed in the intestine, and bound and eventually internalized in the neuronal receptors, producing neuromuscular blockade. Clinically, botulism is divided into four types: food borne, infantile, wound, and unclassified. Systemic neurological symptoms occur within 72 hours of gastrointestinal symptoms and can progress rapidly to respiratory paralysis. Diagnosis depends on a high index of suspicion, but cultures and special tests may be helpful. Treatment remains mostly supportive with good respiratory care emphasized. Use of botulism antitoxin and guanidine may be helpful in some cases.

[Suicide in Bavarian prisons (author's transl)].

The rate of suicides of prisoners in Bavaria from 1945-1974 was investigated. It was shown that only for prisoners awaiting trial was the rate higher than in the normal population or of soldiers of the Bundeswehr. A much higher risk of suicide was found for the first day of imprisonment. The most common method was by hanging in solitary confinement. A sure prevention by confinement with more prisoners is not possible.
