Cephalad renal movement during percutaneous nephrostolithotomy.

The change in the position of the kidney during percutaneous nephrostomy is a major concern to urologists who participate in percutaneous stone removal. We report that the majority of kidneys ascend an average of 2.2 cm. when the patient turns from a supine to a prone position. This change in position is more marked on the right side and is more common in male patients. Knowledge of this anatomical variation is important to determine which patients are suitable candidates for percutaneous nephrostolithotomy and to plan a percutaneous approach to a renal or ureteral calculus.

A proposed expert system for nursing practice. A springboard to nursing science.

The knowledge on which nursing practice is based comes largely from traditional sources, expert nurses passing on the wisdom of their experience to novices. Nursing research, although increasing, is usually parallel to nursing practice, and its findings, at best, are implemented only after long delays. Consequently, the most effective nursing responses to a particular client problem may be undiscovered or unknown. Nursing information systems reflect the nature and usage of nursing knowledge. They offer standard care plans, but the knowledge and decision structures for individualizing care remain exclusively in the mind of the nurse. Nurses may have great freedom to enter information into the information system, but the information is rarely retrievable in a form suitable for evaluation or research. Nursing practice, and the knowledge on which it is based, could be enhanced through the use of a novel expert system. This paper describes how such a system could be developed, with examples from the authors' prototype programs. Taxonomies of data, diagnoses, objectives, and interventions would make it possible to compare patients and to determine the relative effectiveness of nursing interventions. A built-in evaluation component would provide feedback and correction. Everyday nursing practice would become a field for research, and the knowledge gained from research would immediately be fed back into practice. In its development and in its implementation, this kind of system would help to build nursing science.

Imaging of an actively bleeding aortoduodenal fistula.

Diagnosis of gastrointestinal bleeding remains a difficult problem. Radionuclides have been used widely to detect active GI bleeding. A case of actively bleeding aortoduodenal fistula detected on radionuclide imaging is presented.

Scintigraphy of bile leakage following cholecystectomy.

Biloma resulting as a complication of cholcecystectomy is rarely reported. Infection and rupture of biloma into the peritoneal cavity is a reported complication that may need aggressive surgical management. A case of infected biloma is presented in which an active biliary leak was demonstrated on a hepatobiliary scan with Tc-99m DISIDA.

Languages, DBMSs, and expert systems. Software for nurse decision making.

This article sheds some light on the task of selecting software for decision making by nurse administrators. The basics of software are described, defining and distinguishing software, firmware, programs, languages, and systems (including database management and expert systems). A discussion of evolving trends and a short discussion of how to go about selecting software systems to meet your needs are included.
