[Angiogenesis in head and neck tumor].

To clarify the correlation between tumor angiogenesis and tumor growth in head and neck carcinomas, we investigated the number of microvessels, using immunohistochemical factor VIII. No correlations among this number, differences in the primary lesion, histological differentiation and T classification were detected. The incidence of neck lymph node metastases increased as microvessel numbers increased in tumor sites. The number of microvessels increased as N and Stage classification progressed. The number of microvessels in CR cases after induction chemotherapy were increased. The numbers of microvessels in patients without recurrence were apparently greater than those in patients with recurrence. The results of this study suggest that the number of microvessels in a primary tumor correlates with the metastatic ability of the tumor.

[A case of isolated coronary ostial stenosis in a middle-aged woman].

Coronary ostial stenosis with otherwise normal coronary vessels occurs in patients with syphilis or Takayasu's aortitis. Iatrogenic ostial stenosis may develop as a complication of coronary angiography or after coronary perfusion at the time of cardiac surgery. Isolated ostial stenosis in the absence of these factors has been reported infrequently and its unique clinical and angiographic profile has been noted recently. Hence, it is proposed that this type of isolated ostial stenosis may represent a clinical entity distinct from the usual atherosclerotic coronary disease. We report the case of a middle-aged woman with this type of isolated coronary ostial stenosis. A 55-year-old female was admitted with the complaint of exertional chest pain, which had appeared 2 months admission and which had gradually become more frequent. The ECG on admission was normal. She had no coronary risk factor. Treadmill exercise test was stopped at stage 2 of Bruce protocol because ischemic S-T segment depression appeared in II, III, aVF, V4-V6 and she complained of a chest pain. Coronary angiography showed a 90% stenosis of the left coronary ostium with normal distal vessels. The right coronary artery was normal. A coronary artery bypass graft to the left anterior descending branch was performed uneventfully and the patient remains asymptomatic.

[A case of angina pectoris and aortic regurgitation combined with thymoma].

A case of a 72-year-old female with obstruction of LAD, stenosis of RCA, aortic regurgitation in moderate degree and an anterior mediastinal mass of 7 cm in diameter, is reported. Coronary artery bypass grafting, aortic valve replacement and thymectomy were simultaneously performed. On microscopic examination, the tumor was diagnosed as a mixed typed thymoma in the first stage of Masaoka's classification. She was making satisfactory progress without recurrence, after the operation. There was no case has been reported previously in the world in which simultaneous operations were performed for angina pectoris, aortic regurgitation and thymoma.

A bradycardic agent, UL-FS 49, increases atrial force and decreases ventricular force in isolated, perfused heart preparations of dogs.

We investigated the effects of a bradycardic agent, UL-FS 49 (1,3,4,5-tetrahydro-7,8-dimethoxy-3[3-[[2-(3,4-dimethoxyphenyl) ethyl]methylimino]propyl]-2H-3-benzazepin-2-on hydrochloride), on the sinus rate and atrial contractile force and the ventricular force in the isolated, blood-perfused right atrial and left ventricular preparations of the dog and the inhibition by UL-FS 49 of the negative cardiac responses to acetylcholine, adenosine and cromakalim. UL-FS 49 (0.003-3 mumol) decreased sinus rate, increased atrial force and decreased ventricular force in a dose-dependent manner. The threshold doses of UL-FS 49 (0.03 mumol) for atrial responses were smaller than that (1 mumol) for a ventricular response. The duration of bradycardia induced by UL-FS 49 was longer than the intropic one. Propranolol and atropine did not block the positive inotropic and negative chronotropic responses to UL-FS 49, respectively, in isolated atria. UL-FS 49 (0.1-3 mumol) dose-dependently inhibited the negative chronotropic and inotropic responses to parasympathetic nerve stimulation and acetylcholine exogenously administered. UL-FS 49 at higher doses (1 and 3 mumol) attenuated the negative inotropic but not chronotropic responses to adenosine and cromakalim.(ABSTRACT TRUNCATED AT 250 WORDS)

[Sensitivity test to chemotherapeutic agents and cytotoxicic test against immuno-effecter cells of established malignant fibrous histiocytoma cell line].

UNLABELLED: A cell line was established from a patient with malignant fibrous histiocytoma, which had originated in the maxillary sinus. Using this cell line, sensitivity to chemotherapeutic agents and cytotoxicity against various immunoeffecter cells were tested. RESULTS: This MFH cell line was sensitive in some degree to adriamycin, 5-fluorouracil, cisplatin, peplomycin, and methotrexate at high doses, but insensitive to mitomycin-C, vincristine and cyclophosphamide. Furthermore, this cell line showed no sensitivity to cytolytic cytokines, such as tumor necrosis factor-alpha and interferon-gamma. Lymphokine activated killer (LAK) cells and lymphokine activated tumor infiltrating lymphocytes (LA-TIL), induced by culture of peripheral blood lymphocytes and TIL respectively with rIL-2 showed high NK and LK activities and remarkable anti-autologous tumor ability.

Blocking effects of V1 (OPC-21268) and V2 (OPC-31260) antagonists on the negative inotropic response to vasopressin in isolated dog heart preparations.

We investigated the effects of V1 IOPC-21268, 1-(1-[4-(3-acetylaminopropoxy)benzoyl]-4-piperidyl)-3,4-dihydro-2( 1H)- quinolinone) and V2 (OPC-31260, 5-dimethylamino-1[4-(2-methylbensoylamino)benzoyl]-2,3,4,5-tetrahy dro-1H- benzazepine) vasopressin receptor antagonists on the negative inotropic response to arginine vasopressin (AVP) in the isolated perfused heart preparations of the dog. AVP (7.5-750 pmol) decreased the atrial and ventricular contractile force when the preparation was perfused with constant pressure or constant flow. AVP induced a small increase in sinus rate. Desmopressin, a selective vasopressin V2 agonist, did not change the sinus rate and atrial or ventricular contractile force. OPC-21268 (0.01-3 mumol) and OPC-31260 (0.01-1 mumol) induced a small negative inotropic effect. Both OPC-21268 and OPC-31260 inhibited the negative inotropic response to AVP in a dose-dependent manner. The doses of 50% inhibition (ID50) for OPC-21268 and OPC-31260 on the inotropic effect were 0.30 +/- 0.16 mumol and 0.084 +/- 0.034 mumol, respectively. Neither OPC-21268 nor OPC-31260 affected the acetylcholine-, adenosine- or norepinephrine-induced inotropic and chronotropic effects. It has been reported that the concentration of OPC-21268 that displaced 50% of specific AVP binding is 0.4 microM for V1 receptors and > 100 microM for V2 receptors and the concentration of OPC-31260 is 0.01 microM for V2 receptors and 1 microM for V1 receptors. We, therefore, suggest that AVP directly causes negative inotropic effects mediated at least in part by V1 receptors in the dog heart.

A negative inotropic effect of omega-conotoxin GVIA in isolated, blood-perfused dog right atria.

Using an isolated, blood-perfused canine atrial preparation, we investigated the effects of omega-conotoxin GVIA (omega-CgTX), a blocker of N-type voltage-operated calcium channels, on the atrial contractile force and sinus rate. Omega-CgTX in a dose range of 0.3-3 nmol gradually attenuated the atrial contractile force in a dose-dependent manner, but did not affect the sinus rate, indicating that it can depress contractility but not pacemaker activity.

[The characteristics of nasopharyngeal carcinoma cell strain against chemotherapeutic agents and cytokines].

The closed relationship between nasopharyngeal carcinoma (NPC) and Epstein-Barr virus (EBV) has been pointed out. However, it still remains unclear whether the EBV becomes a true factor in initiation and promotion in NPC or not, because of difficulty to establish this cancer strain. We succeeded to establish the EBV-genome positive cell line (NPC-TY861) in 1986, which had been derived from the tissues of nasopharyngeal carcinoma. In this study, the characteristics of this cell strain were investigated by tests for sensitivity against several kinds of anti-cancerous agents and immuno-activated cells. The sensitivity tests revealed that NPC-TY861 cell line was extremely sensitive to PEP, ADR, VCR, LPA and CDDP with dose dependent manner. Furthermore, this cell strain had similar sensitivity to CDDP derivatives, such as 254-S, NK121, DWA2114R and CBDCA as KB cell strain did, which had been derived from an oral base cancer. The oral base cancer was a poorly differentiated squamous cell carcinoma histopathologically as same as NPC-TY861 cell strain. On the other hand, NPC-TY861 cell strain showed no sensitivity to cytokines, TNF and IFN, which have direct anti-cancerous actions. NPC-TY861 cell strain had resistant to NK cells and much more resistant to LAK cells derived from IL-2 than Daudi cell strain did. In order to elucidate the oncogeneous mechanism in NPC, it is indispensable to study the character of this cell strain and it would promise to get a new effective therapeutic method for the patient with NPC, who has bad prognosis.

[Comparative studies of diagnosis with US or CT of the cervical lymph node metastases in head and neck cancer].

Correct assessment of lymph node metastasis in the head-neck region is very important for management of head and neck cancer. 233 lymph nodes were removed by radical neck dissection from 12 cases with cancer in the head and neck region, who did not undergo any preoperative treatment, and the materials were histopathologically examined. The histopathological findings were compared with preoperative US and CT findings. Histopathologically, 26 lymph nodes were found positive for metastasis and the remaining 207 lymph nodes, negative. US detected 45 (19%) of 233 lymph nodes before operation, and 40 of the 45 lymph nodes (89% : 40/45) were qualitatively correctly diagnosed. CT detected 21 lymph nodes (19%), 16 of which were qualitatively correctly diagnosed (76% : 16/21). Of 26 lymph nodes which were histopathologically involved, 19 lymph nodes were correctly diagnosed by US and 11 by CT. Possible reasons explaining the superiority of US to CT in terms of diagnostic reliability are the following: 1) US demonstrates more clearly the existence of lymph nodes than CT. 2) US is more reliable for measuring sizes of lymph nodes than CT. In literature, CT has been often reported to be useful to diagnose cervical lymph node involvement in individual cases. However, we insist that it is necessary to diagnose individual lymph nodes strictly for correct assessment of the reliability of image diagnosis.

[A statistical study of clinical cases of malignant tumors first manifested by vocal cord paralysis].

One of the most important problems in dealing with vocal cord paralysis is to clarify the cause of paralysis. In those case where the definite cause is unknown, it is possible that an occult type of malignant tumors exists along the course of the vagus or recurrent laryngeal nerves. For the past 16 years, 62 out of 560 cases with vocal cord paralysis were diagnosed to have malignant tumors first manifested by paralysis of vocal cord. They consisted of 25 cases of thyroid carcinoma, 18 lung carcinoma, 13 esophageal carcinoma, 3 mediastinal tumors and 3 tumors of miscellaneous origin. Among these, 36 were male and 26 were female and the mean age was 63 years old. In 40 cases, paralysis was found on the left side, in 18 on the right and bilaterally in the remaining 4. There was a marked sexual difference in the origin of malignant tumors, i.e. the incidence of lung carcinoma was higher in male with paralysis on the left side, while that of thyroid carcinoma was higher in female. In some cases with thyroid carcinoma, vocal cord paralysis was noted more than a year before the detection of the tumor. Ultrasonography of the neck, chest x-ray, CT scan of the chest, fluoroscopy of the pharyngo-esophagus should be performed for the detection of malignant tumors in cases with left vocal cord paralysis, while ultrasonography of the neck and chest x-ray should be sufficient in right paralysis, although additional fluoroscopy of the pharyngo-esophagus seemed preferable in male.

[Clinical studies of malignant tumors in the nasal cavity].

Nineteen cases with malignant tumors in the nasal cavities have been treated at the department of otolaryngology, Yokoharma City University, during the 10 years from 1978 to 1987. 1. Cases were 8 males and 11 females, and their ages ranged from 27 to 84 years (Mean age: 64.6). 2. In the histological classification, 9 cases were the epithelial malignant tumors (squamous cell carcinoma 5; adenoid cystic carcinoma 2; transitional cell carcinoma 1; malignant pleomorphic adenoma 1), 9 cases were non-epithelial malignant tumors (malignant melanoma 6; malignant lymphoma 2; olfactory neuroblastoma 1), and one case was unclassified malignant tumor. 3. Cases with epithelial malignant tumors showed better prognosis after treatment of surgical and radiation therapy. But those of non-epithelial malignant tumor were worse. 4. A very rare case with malignant pleomorphic adenoma, originated at the lateral nasal wall was reported and its clinical features and treatment were discussed. This tumor has not been reported up to the present in Japan.

[Investigation of UFT and radiation combination therapy of head and neck cancer--mainly in laryngeal cancer].

We studied the effects of simultaneous combination therapy with UFT and radiation for 69 head and neck cases with the same protocol in 6 universities in Kanagawa. Completed cases (cases in which therapeutic effects could be evaluated) totaled 67 patients who could receive more than 40 Gy as a given radiation dose and more than 8.4 g as a total UFT dose. Responder rate at 90th day after completion of radiation in 67 completed cases (CR 57 cases + PR 6 cases/67 cases) was as favorable as 94%. A comparison between 52 cases of the most frequent laryngeal cancer and historical control 113 cases received single radiation therapy by means of Kaplan-Meier method resulted significantly (P less than 0.05) favorable in UFT combination group as for disease free rate in all cases, disease free rate in Stage IV cases, disease free rate in glottic cancer, and survival rate and disease free rate in supraglottic cancer. No severe adverse reactions were observed in all cases. This combination therapy with radiation and UFT was judged effective for head and neck cancer, especially laryngeal cancer.

[A phase 2 study of recombinant interleukin 2 (S-6820) for head and neck cancer].

To evaluate the clinical efficacy and safety of a recombinant interleukin-2 (S-6820) therapy for head and neck cancer, a phase 2 study was conducted in a cooperative endeavor that has involved 23 institutions throughout Japan. Of the 81 patients in this study, 46 were eligible for an evaluation of the efficacy of this therapy. Two patients achieved a complete response (CR), and 4 patients achieved a partial response (PR). The overall efficacy rate was 13.0% (6/46), and good responses were obtained following a local injection either intratumorally or about the tumor. In contrast, a good result was not obtained from systemic administrations. WBC and lymphocyte counts in the peripheral blood increased and NK activity and LAK cells were seen to develop after treatment. Side effects were observed in 70.9% of 55 evaluable patients. These effects included fever (61.8%) or general fatigue (14.5%). In the laboratory findings, abnormalities such as eosinophilia were noted in 65.5% of patients.

Artificial immune system against viral infection involving antisense RNA targeted to the 5'-terminal noncoding region of coliphage SP RNA.

We previously reported the utilization of antisense RNA in the development of a novel immune system against RNA coliphage SP proliferation (Hirashima et al. [1986] Proc. Natl. Acad. Sci. U.S. 83, 7726-7730). We attempted to determine the most effective (i.e., those eliciting antiviral activity) sequences for targeting micRNAs within the 5'-terminal noncoding region of 54 nucleotides (nt). It was found that a 30-nt micRNA against the sequence from base 32 to 61 exhibited nearly complete inhibition of phage production. Upon further dissection of this sequence, it was concluded that the most effective micRNA against phage SP production should contain the sequences complementary to the Shine-Dalgarno (SD) sequence of the first gene and its 13-nt upstream sequence. The addition of downstream sequences had little effect. These results suggest that the micRNA functions by preventing the binding of ribosomes to the SD sequence of the first gene. The addition of further upstream sequences had a significant negative effect on the micRNA function, indicating that the removal of such impeditive sequences from a micRNA is an important strategy for the development of a potent micRNA immune system.

[Combination effect of UFT and gamma-irradiation against nude mice transplanted KB cells].

Combination effect of UFT and gamma-irradiation was examined using KB cell transplanted in nude mice. KB cell line is known as derived from human malignant tumor of the mouth floor. Although treatment with UFT or with gamma irradiation did not suppress the tumor growth, combination of these two treatments caused a marked suppression. Especially, when UFT was administered to the mice successively from the time of transplantation and the irradiation was applied after tumor growth, both suppression of the tumor growth and necrosis of the tumor tissue were observed.
