The production of anomalous pancreaticobiliary ductal union in canine models.

For the purpose of resolving the mechanism of the ill-effect of refluxed pancreatic juice, due to the anomalous pancreaticobiliary ductal union, canine models were produced. Mongrel adult dogs and puppies were used. Pancreatico-cholecystostomy was performed on 5 adult dogs and 5 puppies. Pancreatico-choledochostomy was performed on 10 adult dogs and 5 puppies. In both procedures, the cylindrical dilatation of the choledochus resulted. The pathological changes in the choledochus were very similar to the cylindrical choledochal dilatation in the human. Elastic fibers diminished universally and in some cases they vanished completely. The changes in the papilla of the bile duct were very slight. There were no cases with hardened papilla. The pathological changes were greater in the pancreatico-cholecystostomy. Enzymatic analysis of pancreatic juice in bile was very important in explaining the etiology. Amylase was proven to be very high throughout the cases, over 10,000 IU/1 by enzymatic assay. Trypsin and elastase were proven to be activated in pancreatico-cholecystostomy, naturally assuming the existence of enterokinase. They were aso proved to be activated in the pancreatico-choledochostomy, without the existence of enterokinase. Though the mechanism was not clear, the activated proteases were the actual cause of the ill-effect in this anomaly. Thus we have succeeded in producing the ideal animal models of the reflux of pancreatic juice into the bile duct in adult dogs and puppies.

Impending rupture of the stomach associated with malrotation and midgut volvulus.

Two cases of impending gastric rupture, associated with malrotation and midgut volvulus, are reported. The diverticulous mucosal protrusions were characteristically revealed as a box-shaped gastric shadow in the plain X-ray and upper GI series. Two cases were operatively treated with only the seromuscular suture of the defect without excision of the mucosal protrusion. Midgut volvulus and malrotation were reduced. Post-operative course was uneventful.

[Immunohistochemical analysis of tumor cells using choline acetyltransferase (author's transl)].

For an immunohistochemical analysis of cellular function of tumors, as related to acetylcholine, the antibody to choline acetyltransferase from bovine brain was obtained in guinea pigs. The specificity of the antibody was immunohistochemically studied in the cervical spinal cord of the mouse. And the findings coincided well with the biochemically and histochemically data on the distribution of choline acetyltransferase or acetylcholinesterase in the spinal cord. The choline acetyltransferase activity in the tumor cells at 6-10 days after subculture was 2.26 nmol/1 x 10(5) cells/hr in glioblastoma, 1.77 nmol/1 x 10(5) cells/hr in C-1300 and 1.45 nmol/1 x 10(5) cells/hr in sarcoma and the difference was statistical. In the immunohistochemical cell staining of these tumors, the rate of fluorescence-positive cells was 82.0% in glioblastoma, 37.3% in C-1300 and 4.2% in sarcoma. These findings coincide well with data on the enzymatic activity. The antibody is applicable not only in the field of the immunohistochemistry, but also for a mechanical analysis of cells at the single cell level, as demonstrated by Fluorescence Activated Cell Sorter (FACS).

Ultrasonic evaluation of abdominal masses in the pediatric patient.

Ultrasonography was applied to 140 pediatric patients who were diagnosed with histopathological confirmation of masses by either surgical or autopsy methods during the past nine years in the National Children's Hospital and the Hospital of Toho University. The masses were classified into three ultrasonic patterns; solid, cystic, and mixed. A solid pattern was obtained in 47 cases, a cystic pattern in 67, and a mixed pattern in 26. Of the cases of the solid pattern, a benign tumor was seen in 32 per cent, and a malignant tumor in 68 per cent. Each 50 per cent of the cases of the mixed pattern had malignant or benign tumors. All the cases of the cystic pattern had benign tumors. Ultransonics was used as a supplement to routine clinical studies.

Anomalous systemic arterial supply to the basal segments of the lung presenting with a murmur.

A murmur was discovered on routine preschool physical examination of a 5 year and 7 month-old boy. A lower lobe lesion was detected by chest X-ray. Aortography demonstrated that a large systemic artery from the descending thoracic aorta supplied the basilar segments of the left lower lobe, which had no normal pulmonary arterial supply. A clinical diagnosis of intralobar sequestration of the lung was made, and left lower lobectomy was performed.
