[Late occlusion at the anastomosis site of an extended polytetrafluoroethylene (E-PTFE) graft with small caliber].

Recently expanded polytetrafluoroethylene (E-PTFE) graft is often used in arterial bypass for atherosclerosis obliterans. In our series of 38 patients (48 legs) undergoing femoropopliteal bypass with E-PTFE graft of 6mm-caliber, a cumulative patency rate was 40.8% at 36 months after operation and a mean duration of patency was 26.3 months. To clarify the cause of occlusion, the diseased grafts were removed and histopathologically examined. Macroscopically, the E-PTFE graft was occluded by the intimal hyperplasia, so called pannus ingrowth from the recipient artery into the graft. The pannus formation of the graft was more remarkable in the distal than in the proximal site of anastomosis, suggesting the influence of blood flow on its formation. Microscopically, no endothelial cells were observed inside the E-PTFE graft except for the surface of pannus. Thus, less extension of endothelialization was thought to be a cause of late occlusion in E-PTFE grafts. Most desirable is the autologous saphenous vein for femoropopliteal bypass. However, when it is not available, E-PTFE graft with a larger caliber must be used in combination with postoperative anticoagulant therapy.

[Development of a small caliber vascular graft using a connective tissue tube with temporary antithrombogenicity].

A connective tissue tube containing collagen fibrils and capillary blood vessels is an ideal material for artificial artery, since the collagen fibrils compose an excellent supporting framework for cell migration and proliferation and the endothelial cells of the capillary can be the origin of the endothelialization of the graft. However, its thrombogenicity has remained a problem. This experiment was designed to develop a small caliber (3mm) vascular graft using an autogenous connective tissue provided with temporary antithrombogenicity. A tube knitted of ultra-fine polyesters was implanted subcutaneously in the back of mongrel dogs for three weeks. Thus accomplished connective tissue grafts were heparinized ionically in vitro, using cross-link agents. Then the treated grafts, 5-6 cm in length and 3mm in diameter, were segmentally replaced with bilateral carotid arteries. During the follow-up period up to 55 days, 7 of 8 grafts (88%) were patent. Histological examinations revealed that there was no thrombus and the neointima formation with a lining of endotherial cells was completed, extending overall length of the grafts. Thus, newly devised hybrid type of connective tissue graft is thought to be usefull for a replacement for arteries with a small caliber.

Actions of angiotensin II and noradrenaline on smooth muscle cells of the canine mesenteric artery.

The actions of angiotensin II (AngII) and noradrenaline (NA) on smooth muscle cells of the canine mesenteric artery were studied by measurement of isometric contractions recorded from muscle strips and the intracellular Ca2+ concentration monitored with quin2-fluorescence from dispersed suspensions of single cells. The Ca2+ transients provoked by the two agonists were monophasic in shape, i.e., after application of each agonist, [Ca2+]i rose immediately within 1 s and decreased to near-basal level within 5 min. The contraction induced by NA was maintained for several minutes whilst that induced by AngII was short-lasting. When NA was repetitively applied to the strip in Ca2+ -containing solution, the same amplitude of contractions was always obtained. In contrast, after initial exposure to AngII, subsequently-applied AngII generated small contractions. In Ca2+-free solution, either agonist could induce the large contraction. After initial exposure to NA or AngII in Ca2+ -free solution, subsequently-induced contractions by either agonist were reduced. The response induced by AngII was blocked by [Sar1, Ile8]-AngII and that of NA was blocked by phentolamine. Pertussis toxin inhibited contractions induced by both agonists but not those induced by caffeine and high K+. An activator of protein kinase C, 12-O-tetradecanoylphorbol 13-acetate (TPA), produced a slowly-developing contraction without any change in [Ca2+]i, and this agent inhibited the contractions and Ca2+ transients induced by both agonists. These results indicate that NA and AngII each act on a specific receptor and release Ca2+ from common intracellular storage sites through production of inositol 1,4,5-trisphosphate (InsP3).(ABSTRACT TRUNCATED AT 250 WORDS)

Effect of the calcium antagonist nilvadipine on haemodynamics at rest and during cold stimulation in essential hypertension.

The immediate haemodynamic effects of the calcium antagonist nilvadipine have been studied in ten patients with established mild essential hypertension. Nilvadipine 4 mg p.o. reduced both the systolic and diastolic blood pressures within 60 min, associated with a fall in total peripheral resistance and an increase in heart rate and cardiac index. The peak of blood pressure and total peripheral resistance reached during a cold pressor test were reduced by nilvadipine, but it did not affect the haemodynamic responsiveness to cold stimulation. Plasma renin activity was unaltered and the plasma noradrenaline concentration was increased only slightly. Thus, nilvadipine lowered blood pressure at rest and during cold stimulation as a result of arteriolar dilatation. The hypotensive effect at rest was associated with a reflex increase in heart rate and cardiac index.

Involvement of alpha-adrenoceptors in para-hydroxyamphetamine-induced head-twitch response.

The effect of some drugs with a higher selectivity for either the alpha 1- or alpha 2-adrenoceptors on the head-twitches induced by intracerebroventricular administration of p-hydroxyamphetamine (p-OHA) in mice, have been studied. Pretreatment with yohimbine increased the number of head-twitches induced by p-OHA, whereas pretreatment with clonidine or prazosin reduced the number of responses. The decrease in head-twitches produced by clonidine was completely antagonized by pretreatment with yohimbine. Pretreatment with 6-hydroxydopamine prior to the combined treatment with clonidine and p-OHA, resulted in recovery of the reduced level head-twitches to the level induced by p-OHA alone. Pretreatment with 6-hydroxydopamine alone resulted in a marked increase in the number of p-OHA-induced head-twitches. These results clearly indicate that a noradrenaline system in the brain may, at least in part, be involved in the p-OHA-induced head-twitches in mouse, most probably by modulating a serotonergic system which is responsible for the head-twitch response.

Meningeal chondrosarcomatous tumor associated with meningocytic differentiation.

Most meningeal chondrosarcomatous tumors have been regarded as identical to similar tumors occurring elsewhere in the body. The neoplasm reported here, however, clearly demonstrated transitions between usual meningiomas and chondrosarcomatous tumors. The present case thus provides evidence in support of Cushing's and Eisenhardt's proposal that some meningeal chondrosarcomatous tumors are derived from meningeal cells and are histogenetically related to meningiomas.

A regenerable carbon dioxide removal and oxygen recovery system for the Japanese Experiment Module.

The Japanese Space Station Program is now under Phase B study by the National Space Development Agency of Japan in participation with the U.S. Space Station Program. A Japanese Space Station participation will be a dedicated pressurized module to be attached to the U.S. Space Station, and is called Japanese Experiment Module (JEM). Astronaut scientists will conduct various experimental operations there. Thus an environment control and life support system is required. Regenerable carbon dioxide removal and collection technique as well as oxygen recovery technique has been studied and investigated for several years. A regenerable carbon dioxide removal subsystem using steam desorbed solid amine and an oxygen recovery subsystem using Sabatier methane cracking have a good possibility for the application to the Japanese Experiment Module. Basic performance characteristics of the carbon dioxide removal and oxygen recovery subsystem are presented according to the results of a fundamental performance test program. The trace contaminant removal process is also investigated and discussed. The solvent recovery plant for the regeneration of various industrial solvents, such as hydrocarbons, alcohols and so on, utilizes the multi-bed solvent adsorption and steam desorption process, which is very similar to the carbon dioxide removal subsystem. Therefore, to develop essential components including adsorption tank (bed), condenser. process controller and energy saving system, the technology obtained from the experience to construct solvent recovery plant can be easily and effectively applicable to the carbon dioxide removal subsystem. The energy saving efficiency is evaluated for blower power reduction, steam reduction and waste heat utilization technique. According to the above background, the entire environment control and life support system for the Japanese Experiment Module including the carbon dioxide removal and oxygen recovery subsystem is evaluated and proposed.

Preliminary experimental results of gas recycling subsystems except carbon dioxide concentration.

Oxygen concentration and separation is an essential factor for air recycling in a CELSS. Furthermore, if the value of the plant assimilatory quotient is not coincident with that of the animal respiratory quotient, the recovery of O2 from the concentrated CO2 through chemical methods will become necessary to balance the gas contents in a CELSS. Therefore, oxygen concentration and separation equipment using Salcomine and O2 recovery equipment, such as Sabatier and Bosch reactors, were experimentally developed and tested.

Multilocular cystic nephroma in an adult: immunohistochemical study.

We report a case of multilocular cystic nephroma (cysts of the kidney). The tumor was enucleated surgically. It was well encapsulated, and composed of numerous cysts and cellular stroma. Microscopically, the stromal cells resembled blastematous cells seen in Wilms tumor. Immunohistochemical studies disclosed that the stromal cells resembled leiomyocytes, since vimentin and desmin were positive, and myoglobin was negative. An ultrastructural study supported these findings. The characteristics of these cells were compatible with those of congenital mesoblastic nephroma, which also is considered a metanephric tumor.

Induction and repair of DNA lesions in cultured human melanoma cells exposed to a nitrogen-ion beam.

Induction and repair kinetics of DNA lesions after exposure to nitrogen ions (N-ions) were studied in comparison to those after 180 kVp X-rays. DNA lesions in human melanoma cells (HMV-I) irradiated with 95 MeV N-ions (0-6 Gy, l.e.t.D = 530 keV micron-1 or with X-rays (0.9 Gy) were assayed by alkaline elution. The N-ion r.b.e. for DNA lesion induction was approximately 0.7. About 85 per cent of the lesions induced by N ions were rejoined with a time-course similar to the rejoining of DNA lesions produced by X-rays. These lesions were considered to be induced by delta-rays around the N-ion tracks. The fraction of residual DNA lesions remaining after a 6 h post-irradiation incubation was higher for N-ions than for X-rays. Unlike the case for X-rays, DNA-protein crosslinks were included in the residual DNA lesions after N-ion irradiation.

Mechanisms of the Ba2+-induced contraction in smooth muscle cells of the rabbit mesenteric artery.

The mechanism of the Ba2+-induced contraction was investigated using intact and saponin-treated skinned smooth muscle (skinned muscle) strips of the rabbit mesenteric artery. After depletion of Ca2+ stored in the caffeine-sensitive site, greater than 0.65 mM Ba2+ evoked contraction in muscle strips depolarized with 128 mM K+ in Ca2+-free solution in a dose-dependent fashion, and the ED50 values for Ca2+ and Ba2+ were 0.5 mM and 1.2 mM in intact muscle strips, respectively. Nisoldipine (10 nM) blocked the contraction evoked by high K+ or 10 microM norepinephrine (NE) in the presence of 2.6 mM Ba2+, but did not block the contraction evoked in the presence of 2.6 mM Ca2+. These results may indicate that Ba2+ permeates the voltage-dependent Ca2+ channel. In skinned muscle strips, the ED50 values for Ca2+ and Ba2+ were 0.34 and 90 microM, respectively, as estimated from the pCa- and pBa-tension relationships. Calmodulin enhanced and trifluoperazine inhibited the Ba2+- and Ca2+-induced contractions. After the application of Ba2+ or Ca2+ with ATP gamma S in rigor solution, myosin light chain (MLC) was irreversibly thiophosphorylated, as estimated from the Ba2+- or Ca2+-independent contraction. Furthermore, both divalent cations phosphorylated MLC, as measured using two-dimensional gel electrophoresis, to the extent expected from the amplitudes of the contraction evoked by these cations. Thus, Ba2+ is capable of activating the contractile proteins as Ca2+ does. The amount of Ca2+ or Ba2+ stored in cells was estimated from the caffeine response evoked in Ca2+-free solution in intact and skinned muscle strips. After the application of 0.3 microM Ca2+ or 0.1 mM Ba2+ for 60 s to skinned muscle strips after the depletion of Ca2+ stored in cells, caffeine produced a contraction only upon pretreatment with Ca2+ but not with Ba2+. When Ba2+ was applied successively just after the application of Ca2+, the subsequently evoked caffeine-induced contraction was much smaller than that evoked by pretreatment with Ca2+ alone. The above results indicate that Ba2+ permeates the voltage-dependent Ca2+ channel but may not permeate the receptor-operated Ca2+ channel, it releases Ca2+ from store sites but is not accumulated into the store site, and it directly activates the contractile proteins via formation of a Ba2+-calmodulin complex.

Effects of changes in afterload impedance on left ventricular ejection in isolated canine hearts: dissociation of end ejection from end systole.

To examine how end systole differs from end ejection and also whether the slope of the end systolic pressure-volume relation can be approximated to that of the end ejection pressure-volume relation, nine isolated, perfused, paced canine hearts ejecting into a hydraulic loading system that simulated the aortic input impedance of a dog's arterial tree were studied. To measure left ventricular volume changes the heart was placed in a plethysmograph. Peripheral resistance (Rp) and arterial compliance (C) were independently varied from 1.9 (Rp = 1.9) to 3.3, 6.4, and 9.6 X 10(8) Pa.m-3.s (Rp run) with a constant value of compliance 1.3 X 10(-9) Pa-1.m3 (C = 1.3), and from C = 0.4 to C = 0.8, C = 1.3 and C = 2.3 (C run) with a constant value of resistance (Rp = 6.4). Five pressure-volume loops were obtained by changing the end diastolic volume at each value of compliance and peripheral resistance. It was clearly shown that ventricular ejection continued after end systole and the time duration between end systole and end ejection became longer with increasing arterial compliance (24(4) at C = 0.4 vs 49(4) ms at C = 2.3, p less than 0.001), while the time duration between end diastole and end systole was constant regardless of afterload impedance change. Regarding the left ventricular pressure-volume relation the end systolic relation was almost linear (r greater than or equal to 0.98) and the slope was not significantly affected by change in any afterload impedance tested. End ejection pressure-volume relation was also linear (r greater than or equal to 0.97) and the slopes in the peripheral resistance and compliance runs were lower than those of the end systolic pressure-volume relation in each corresponding run. The former slopes decreased at smaller values of Rp or larger values of C--namely, 4.4(0.6) at Rp = 9.6 vs 3.6(0.6) at Rp = 1.9, p less than 0.05; 4.8(0.6) at C = 0.4 vs 3.1(0.5) mmHg.ml-1 at C = 2.3, p less than 0.001. Thus it is concluded that end ejection is usually different from end systole and the time difference between them is affected by changes in arterial compliance. In addition, the slope of end ejection pressure-volume relation was dependent on the changes in afterload impedance and cannot be approximated to that of the end systolic pressure-volume relation.

Role of prostaglandin in norepinephrine and renin release in canine kidney.

We investigated renin and norepinephrine (NE) release during electrical renal nerve stimulation (RNS) in relation to prostaglandin (PG) E2 concomitantly produced by the kidney in anesthetized dogs. During 10 min of continuous RNS (2.5-4 Hz), the increases in renin, NE, and PGE2 secretion rates were determined at 1 and 10 min after the start of stimulation. Under control conditions, almost the same extent of increase in the NE secretion rate was observed at 1 and 10 min of RNS, whereas the increase in renin secretion rate at 1 min of RNS was followed by a further increase at 10 min of RNS. On the other hand, an upward but not significant trend of increase in PGE2 secretion at 1 min of RNS was followed by a substantial level at 10 min of RNS. After administration of indomethacin, the increase in NE secretion rates at both 1 and 10 min of RNS were not altered, but the increase in renin secretion rate at 10 min of RNS was suppressed by approximately 50%, without any reduction of the increase in the renin secretion rate at 1 min of RNS. Consequently, the time-related change in the renin secretion rate during RNS was abolished. These results suggest that renin response to continuous RNS is enhanced by concomitantly generated PGs but not by NE, and furthermore, that endogenously generated PGs do not inhibit the release of NE from canine renal nerve endings.

Participation of the prostaglandin system in furosemide-induced changes of renal function in anesthetized rats.

The possible mediation of the endogenous prostaglandin and kallikrein-kinin systems of changes in renal function induced by furosemide was studied in anesthetized rats. Increasing doses of furosemide infusion (0.03, 0.1, and 0.3 mg/kg/min) caused dose-related diuresis, natriuresis, kaliuresis, and decreased renal blood flow and urinary osmolality without any significant changes in mean arterial blood pressure. Pretreatment with the prostaglandin synthetase inhibitor indomethacin resulted in marked reduction of the water and sodium excretion induced by furosemide. It also blunted renal vasoconstriction and renin release by furosemide, but the glomerular filtration rate was not affected. Pretreatment with aprotinin, a kallikrein inhibitor, failed to affect the renal response to furosemide. The results indicate that the renal prostaglandin system, but not the kallikrein-kinin system, participates in the effect of furosemide on renal functions mainly through electrolyte transport inhibition in the renal tubule.

Effects of the pericardium on the diastolic left coronary pressure-flow relationship in the isolated dog heart.

We studied the effects of the pericardium on diastolic left coronary pressure-flow relationships in heart-blocked and isolated canine preparations. In these preparations, the left and right coronary arteries were dilated with adenosine and perfused by means of a pressurized arterial reservoir. The diastolic left heart pressure (LHP) was controlled by the height of a reservoir connected to the left atrium and left ventricle. The right atrial and ventricular pressure i.e., coronary outflow pressure, was kept constant at 0 mm Hg. Before and after pericardiectomy, diastolic coronary pressure-flow relationships were obtained at three values of LHP (0, 15, and 30 mm Hg) with driving pressure decreasing (2 mm Hg/sec or less) from approximately 60 mm Hg to the actual zero-flow pressure (Pf = 0) during a single long diastole induced by cessation of ventricular pacing. The slopes of the coronary pressure-flow relationships were approximated by a linear regression analysis in which the correlation coefficients were greater than .98 in all cases. Before pericardiectomy, with LHP increasing from 0 to 15 and 30 mm Hg, the value of Pf = 0 significantly increased from 7 +/- 1 to 16 +/- 1 (p less than .01) and 28 +/- 2 mm Hg (p less than .01), respectively. After pericardiectomy, it increased from 7 +/- 1 to 14 +/- 1 (p less than .01) and 17 +/- 2 mm Hg (p less than .01), respectively. When LHP was at 0 and 15 mm Hg, the pericardiectomy had no effect on the value of Pf = 0.(ABSTRACT TRUNCATED AT 250 WORDS)
