[Mental health in software engineers. II. Classification of occupational stressors, and relationship between occupational stressors and psychiatric disorders].

Occupational stressors and psychiatric disorders were studied in a sample of 101 male software engineers. Psychiatric diagnosis was based on DSM-III (Diagnostic and Statistical Manual of Mental Disorders, Third Edition). The results can be summarized as follows: 1) More than 30% of the subjects were not satisfied with their work. 2) Occupational stressors ascertained in the interview could be classified into "quantity of job," "quality of job," "role in organization," "relations with others," "work conditions," "career development," "reward," "organizational structure and climate," "low social support," and "lack of decision making." "quality of job" (35%), "quantity of job" (30%), and "role in organization" (33%) were the most common stressors for the subjects. 3) Among the occupational stressors, only "quantity of job" was significantly related to psychiatric disorders (p less than 0.05). The results suggest that many software engineers are not satisfied with their work and that "quantity of job" has the greatest influence on their mental health. Some measures should be taken to avoid potential health hazards, for the research findings indicate a relationship between occupational stressors and mental health in software engineers.

Ureteral action potential and histological changes in the upper urinary tract receiving the formalin.

The present study investigates the ureteral action potential and histological changes of pelvi-ureteral system after injection of the formalin into the obstracted ureters or the renal pelvis. In the first experiment, formalin was injected into the obstructed ureters of 18 dogs for 30 minutes. In the second, formalin was injected into the obstracted renal pelvis of 13 dogs for 30 minutes using ureteral balloon catheters, and then renal pelvis were released from obstraction and catheters were removed. In these experiments, ureteral electromyogram were recorded and histological changes of pelvi-ureteral system were also observed microscopically. Results 1. After injection of formalin into the ureters, ureteral action potential disappeared and had not restored. Histologically, damage was observed in the ureteral smooth muscle as well as in the mucosa. 2. After injection of formalin into the renal pelvis, ureteral action potential disappeared in 46% of the ureters. In 54% of the ureters, action potential had not disappeared, however discharge interval became irregular. Histological changes of the renal pelvis was not related to the presence or absence of ureteral action potential. The results of the present study show that ureteral smooth muscle play part in the conduction of the ureteral excitation, and have an irregular autonomic discharge.

[A study on LH- and FSH-RIA kits by immunoradiometric assay].

An evaluation of LH- and FSH-RIA BEAD kits based on IRMA was carried out. The results obtained with the methods characterized by the use of monoclonal antibodies, i.e., one is linked to solid phase, and another is isotopically labeled, were compared with those determined by the Daiichi LH- and FSH-kits. Intra- and inter-assay precision, recovery, linearity, and specificity of both methods were favorable without exceptions. The cross reactivity of the LH kit to 5,000 mIU/ml hCG revealed within the range of less than 3 mIU/ml. Significant correlations were observed between the results derived from conventional Daiichi LH- and FSH-kits. The results from the conventional kits exhibited 30 to 40% of those from the Daiichi kits, considered to be mainly due to the difference in standard calibrations used. Among the individuals within the normal menstrual cycle, the serum LH- and FSH-levels determined by the present kits gave a typical pattern with a peak in the preovulatory phase. On the other hand, the LH- and FSH-values of individuals in normal pregnancy revealed strikingly decreased in comparison with those of non-pregnant women.

[Studies on pyospermia in male infertility].

Among 670 infertile men, 72 were diagnosed as pyospermia according to our criteria. i.e., WBC greater than or equal to 10/hpf semen. The sperm motile efficiency index (SMEI) which indicates the rate of progressively motile sperms, was significantly low in pyospermic group compared with that of non-pyospermic men (WBC less than 5/hpf semen). From the result of split ejaculation, a major cause of pyospermia was supposed to be chronic prostatitis. The SMEI was decreased immediately after addition of the neutrophils and granulocyte elastase to semen. The mean value of granulocyte elastase in pyospermic group was 2859.6 micrograms/L, whereas that of non-pyospermic men was 131.6 micrograms/L. In summary, granulocyte elastase in seminal plasma may be a cause of inhibition of sperm motility in pyospermic state.

Inhibitory effects of diltiazem, verapamil, nifedipine, and nicardipine on sympathetic tachycardia in decentralized hearts of anesthetized dogs.

Intravenous diltiazem (10-300 micrograms/kg), verapamil (10-300 micrograms/kg), nifedipine (1-100 micrograms/kg) and nicardipine (1-100 micrograms/kg) inhibited the tachycardia caused by cardiac sympathetic nerve stimulation (SNS, 0.5-4 Hz) in decentralized hearts of anesthetized dogs. The dose range of each drug required to inhibit the SNS-induced tachycardia was almost equivalent to that required to produce the increase in coronary blood flow and the decrease in blood pressure. Nifedipine and nicardipine were equi-active and about 10 times more potent than diltiazem and verapamil in inhibiting the SNS-induced tachycardia. They produced a slight but dose-dependent slowing of the resting heart rate. The negative chronotropic potency was approximately nicardipine, verapamil greater than nifedipine, diltiazem. Bay K 8644 (30 micrograms/kg) antagonized the inhibitory effects of diltiazem (100 micrograms/kg) and nifedipine (30 micrograms/kg) on the SNS-induced tachycardia. Tachycardia induced by intracoronary norepinephrine (0.03-0.3 micrograms) was suppressed by diltiazem (30-300 micrograms/kg) and nifedipine (10-100 micrograms/kg). The inhibitory effects of calcium entry blocking drugs on the sympathetic tachycardia appear to be explained by the interference of the beta-adrenoceptor-mediated increase in slow inward current in the sinoatrial (SA) node. It is also suggested that other mechanisms different from calcium entry blocking action contribute to the negative chronotropic response to these calcium entry blocking drugs.

Safety of cordocentesis under ultrasound guidance for fetal blood sampling.

In the present study, we attempted to assess the safety and the risk of cordocentesis under ultrasound guidance for fetal blood sampling. Forty-five cordocenteses in 43 cases were carried out. These forty-three cases included 23 nonimmunologic hydrops fetalis, 12 congenital fetal malformations, 3 chromosome aberrations, 3 Rh isoimmunization, and 2 idiopathic thrombocytopenic purpura. Gestational age when cordocentesis was performed ranged from 17 to 39 weeks of gestation. In 42 (93.3%) out of 45 procedures, pure fetal blood was obtained. Postpuncture bleeding was observed in 17 cases (37.8%). In 12 (70.6%) out of 17 cases, bleeding stopped within 2 minutes. In only one case, bleeding continued for 11 minutes and cesarean section was performed due to fetal distress. Cardiotocogram obtained after cordocentesis revealed no ominous signs in the other 44 procedures. The other complications, including premature rupture of the membranes, premature labor, and intrauterine infection, did not occur during or after this procedure. There were no cases in which any kind of injury to the placenta or umbilical cord was observed.

[Immunohistochemical studies of the localization of prolactin in human term decidua].

Human amniotic fluid possesses a considerably higher concentration of PRL. And it is generally accepted that decidual cells are the source of amniotic PRL and, decidual PRL transfers into amniotic fluid across the fetal membranes. Because few studies on the fine morphological structure of decidual tissue have been reported, with the aid of electron microscopy, we noted the number of secretory granules in the partial decidual cells which were obtained at spontaneous term vaginal delivery. 1 Using immunohistochemical procedures and the PAP complex method, immunized anti-PRL revealed PRL reactivity on the numerous granules or globules. 2 Incubation for acid phosphatase (AcP-ase) activity was done after the tissues were fixed. The medium used was Gomori lead medium, with glycerophosphate as substrate. Lysosomes are present in decidual cells. In lysosomes, numerous residual bodies with reaction product are evident as electro-opaque black bodies. 3 In staining of tissues for the study of lipid granules, Sudan III solution (Sudan Schwarz B dissolved in 70% alcohol) was used. Decidual cells contain fewer and bigger cytosolic lipid spheres.

[Apnea and oxygen desaturation following nitrous oxide inhalation].

Apnea and desaturation following nitrous oxide inhalation were studied in seven adult volunteers breathing spontaneously. Arterial oxygen saturation (SpO2), end-tidal CO2 concentration in the nasal cavity and respiratory patterns were measured in volunteers breathing air after N2O (50% or 67%) + O2. SpO2 was measured with Biox 3700 and end-tidal CO2 concentration was measured with Normocap, and respiratory patterns were recorded with RESPIGRAPH. After breathing N2O, two volunteers had frequent apnea (greater than 20 sec) accompanied by desaturation (SpO2 less than 90%). The lowest value of SpO2 was 82%. When the apnea occurred, the airway seemed to be open and end-tidal CO2 concentration values were lower than those before N2O inhalation. The authors considered that this kind of apnea was due to several factors, such as hypocapnia caused by hyperventilation during N2O anesthesia, dilution of alveolar O2 and CO2 during N2O excretion, loss of consciousness by N2O, and depression of CO2 ventilatory response by N2O. Inhalation of O2 at high concentrations for five minutes could improve the hypocapnia and prevent the apnea.

[A case of renal cell carcinoma with polymyositis].

We report a case of renal cell carcinoma with polymyositis. A 62-year-old female with a diagnosis of polymyositis was referred to our department because of the right renal mass discovered by computed tomography. An ultrasonography and renal angiography showed a solid and hypovascular area at the middle pole of the right kidney. A fine needle aspiration biopsy of the mass was performed and the cytology showed a papillary adenocarcinoma. Therefore, the right radical nephrectomy was carried out and her myopathic symptoms were slightly improved postoperatively. The patient has remained well without recurrence or metastasis for two years.

Limbal anesthesia for cataract surgery.

We present a new technique for the administration of anesthesia in cataract surgery. The technique consists of injecting 0.5 ml of locally-acting anesthetic subconjunctivally (or sub-Tenon's) along the superior limbal border. Using this method, we performed extracapsular cataract extraction (ECCE) with and without posterior chamber intraocular lens (PC-IOL) implantation or secondary PC-IOL implantation on 176 cataract patients. Anesthesia was successfully induced in the majority of these patients and all surgeries were carried out successfully with no major complications. Voluntary eye movements remained but did not interfere with surgery. On the contrary, they helped expose certain surgical sites. This new anesthetic method for cataract surgery is simple and minimizes complications attributed to other blind anesthetic techniques.

[A study of drinking habits of rural middle and old aged residents in Ibaraki Prefecture--relationship between problem drinking, depression and personality (Part 1). On male subjects].

Although there has been considerable research conducted regarding the relationship between alcohol dependency and clinical depression, there have been few reports which have investigated the relationship between alcohol consumption and depression in the general populace of residents of rural communities in Japan. We investigated this correlation in a rural community located in Ibaraki Prefecture, the findings of which are described in this report. The subjects of this investigation were residents of the southern part of the town of K in Ibaraki Prefecture age 40 and over, all of whom had participated in a medical examination program. With respect to the men surveyed, the proportions of problem drinkers and severe problem drinkers were 10.4% and 25.7%, respectively. According to the results we obtained with respect to correlation with personality, problem drinkers were found most frequently in shut-in personality types (37.5%). In regard to the correlation with depression, 7% of the problem drinkers and severe problem drinkers exhibited an SDS score of 50 or higher. In addition, the non-drinking group demonstrated a significantly higher degree of depression in comparison to the normal drinker and severe problem drinker groups.

[A study of drinking habits of rural middle and old aged residents in Ibaraki Prefecture--relationship between problem drinking, depression and personality (Part 2). On female subjects].

In looking at the findings of the women subjects of this investigation, alcohol consumption decreased rapidly in younger women with the borderline being roughly at the age of 50 years. This is consistent with general trends in Japan. In terms of the correlation with personality, the proportion of drinkers in those groups consisting of syntonic, self-revelation, nervous and persistent personality types was high at 34-37%. 3 out of the 7 women who were severe problem drinkers were of the persistent personality type. As such, it is believed that depression contributes to problem drinking in middle-aged and elderly women residing in rural communities. According to the results of this investigation focused on women residing in rural communities, 30% of the severe problem drinkers were also suffering from depression. As this figure closely resembles the proportion noted in clinical investigations, it was suggested that in the process of alcoholism becoming involved with health care institutions, there are other contributing factors besides clinical depression.

[A case of major dehiscence of a prosthetic aortic valve].

A case, 48-year-old man, was presented of dehiscence of an aortic Björk-Shiley prosthesis, which was diagnosed by pendular movement of prosthetic mount in cineangiographic finding. Emergent operation of Re-AVR was performed successfully. Valve dehiscence should be confirmed by cineangiographic examination, in case of progressive congestive heart failure combined with traumatic hemolytic anemia and high grade fever suggestive of prosthetic valve endocarditis.

Differential regulation of the low affinity Fc receptor for IgE (Fc epsilon R2/CD23) and the IL-2 receptor (Tac/p55) on eosinophilic leukemia cell line (EoL-1 and EoL-3).

Two types of activation Ag, low affinity FcR for IgE (Fc epsilon R2)/CD23 and IL-2R (Tac/p55), were expressed and differently regulated on human eosinophilic leukemia cell lines (EoL-1 and EoL-3). Because the binding of IgE on EoL-3 cells was completely inhibited by H107 (anti-Fc epsilon R2/CD23 mAb) but not by irrelevant mAb, essentially all the low affinity Fc epsilon R2 on EoL-3 seemed to be the Fc epsilon R2/CD23 molecules. Both IL-4 and IFN-gamma enhanced the surface expression of Fc epsilon R2, whereas IL-1, IL-2, and IL-5 showed no effects, as determined by surface staining with anti-Fc epsilon R2 antibody (H107). In contrast to Fc epsilon R2 up-regulation, IL-4 and IFN-gamma showed a differential effect on the regulation of IL-2R (Tac/p55). Whereas IFN-gamma up-regulated the receptor expression of IL-2R/Tac, IL-4 did not. The result suggests that these lymphokines are involved in the different aspects of the activation pathway of the eosinophils. The possible role of Fc epsilon R2 and IL-2R on the function of eosinophils in allergic reaction is discussed.

Specificity of S-adenosyl-L-methionine in the inactivation and the labeling of 1-aminocyclopropane-1-carboxylate synthase isolated from tomato fruits.

1-Aminocyclopropane-1-carboxylate (ACC) synthase, which catalyzes the conversion of S-adenosyl-L-methionine (AdoMet) to ACC, is irreversibly inactivated by its substrate AdoMet. AdoMet has two diastereomers with respect to its sulfonium center, (-)-AdoMet and (+)-AdoMet. We prepared (+)- and (-)-AdoMet from a commercial source, and compared their activities as a substrate and as an inactivator of ACC synthase isolated from tomato (Lycopersicon esculentum Mill). fruits. Only (-)-AdoMet produced ACC, whereas both (-)- and (+)-AdoMet inactivated ACC synthase; (+)-AdoMet inactivated the enzyme three times faster than (-)-AdoMet. We have previously shown that ACC synthase was specifically radiolabeled when the enzyme was incubated with S-adenosyl-L-[3,4-14C]methionine. The present results further indicate that S-adenosyl-L-[carboxyl-14C]methionine, but not S-adenosyl-L-[methyl-14C]methionine, radiolabeled the enzyme. These data suggest that the 2-aminobutyric acid portion of AdoMet is linked to ACC synthase during the autoinactivation process. A possible mechanism for ACC synthase inactivation by AdoMet is discussed.

Endothelin and the production of cerebral vasospasm in dogs.

We attempted to determine whether or not endothelin (ET) plays a physiological role in cerebral vasospasm. ET (10(-11) - 3 x 10(-8) M) induced a concentration-dependent contraction in isolated canine basilar arteries. Intracisternal injection of 10 to 1,000 pmol ET into dogs induced a dose-dependent decrease in the basilar artery diameter, as measured angiographically on the 1st day. On the 3rd day after the injection of ET (10 pmol), the diameter of the basilar artery had diminished to 76% of the value seen in the control. There were no effects on blood pressure or heart rate. Thus, ET seems to be a potent constrictor of cerebral arteries in vivo, and these effects are long-lasting.

Effects of afterload elevation on the ischemic myocardium in isolated, paced canine heart with partial coronary stenosis.

The effect of afterload elevation on the ischemic myocardium was examined in an isolated, paced canine heart with a partial coronary stenosis. The coronary blood flow of the left circumflex coronary artery was reduced to approximately one-third of the values before stenosis. The left circumflex coronary stenosis produced a decrease in global ventricular function, a decrease in systolic shortening and deviation of the ST-segment of the epicardial electrocardiogram and an increase in myocardial carbon dioxide (CO2) tension of the ischemic region. Then, afterload elevation with constant preload decreased the myocardial CO2 tension and improved the ST-segment deviation of the ischemic myocardium. Mechanical function, estimated by the relation between mean aortic pressure and systolic shortening, also improved with elevation of mean aortic pressure. In contrast, afterload elevation combined with preload elevation did not improve ischemic injury, as estimated by myocardial CO2 tension, and did not improve ST-segment deviation or mechanical function despite an increase in left circumflex coronary flow. These results suggest that the elevation of afterload pressure under constant preload improves ischemia produced by a partial coronary stenosis due to increased coronary blood supply; however, the preload elevation counterbalances the beneficial effects of afterload elevation.
