Clinical survey of ischemic cerebrovascular disease in children in a district of Japan.

Ischemic cerebrovascular disease in children is relatively rare. To clarify the clinical features of ischemic stroke occurring in infants and children, we evaluated 54 cases of cerebral infarction, excluding cases of moyamoya disease, in patients less than 16 years old at 24 clinics in the Tohoku (northeast) district of Japan. We observed two incidence peaks, one in little children and the other in junior high school students. Infection and minor head trauma were more frequently seen prior to ischemic strokes than was heart disease. The middle cerebral artery region, including the basal ganglia, was most commonly affected (49 patients, 91%) on computed tomograms. Angiography was performed in 48 patients (89%) and showed various types of occlusive lesions, mostly affecting the middle cerebral artery. Hemiparesis was the most common form of disability following ischemic strokes (48 patients, 89%). Surgical treatment was carried out in seven patients (13%). The clinical course of these cases showed that the recovery of children after a stroke tends to be better than that of adults, but that permanent disabilities, such as hemiparesis or mental retardation, occur commonly. Further investigation of juvenile cerebrovascular disease is important to prevent ischemic strokes in children.

Changes of plasma concentrations of steroid hormones, prostaglandin F2 alpha-metabolite and pregnant mare serum gonadotropin during pregnancy in thoroughbred mares.

Plasma concentrations of estrogens, gestagens, cortisol (F), 13, 14-dihydro, 15-keto PGF2 alpha (PGFM) and pregnant mare serum gonadotropin (PMSG) in 10 Thoroughbred mares were measured for a 11-month pregnancy period. Estrone (E1) and estradiol-17 beta (E2) levels gradually increased as the pregnancy advanced, showing a peak around Month 8 and decreased thereafter. Progesterone (P) levels increased on Months 3 and 11, and 17 alpha-OH-progesterone (17 alpha-OHP) levels peaked on Month 3, whereas 20 alpha-OH-progesterone (20 alpha-OHP) levels increased sharply after Month 6. PGFM indicated peaks on Months 2 and 11. F and PMSG levels peaked on Months 2 and 3. From factor analysis, Month, E1, E2 and 20 alpha-OHP were discriminated as Factor 1, increasing with the progress of pregnancy, PMSG, 17 alpha-OHP and P as Factor 2, showing a relation with the secondary corpus luteum, and PGFM and F as Factor 3, affecting PGFM change on Month 2 by F. P also related to both Factors 1 and 3, showing an inverse relation against PGFM. In conclusion E1, E2 and 20 alpha-OHP contained in Factor 1 were suggested to be important especially as parameters of placental function after Month 6.

Bilirubin conjugation and biliary bilirubin excretion after intravenous bilirubin injection in dogs.

This study was undertaken to investigate the bilirubin clearance, bilirubin conjugation and biliary bilirubin excretion after an intravenous single bilirubin injection (5 mg/kg or 10 mg/kg) in 10 mongrel dogs. Serum total bilirubin concentration increased after the bilirubin injection, and then it immediately decreased. Biliary excretion of loaded bilirubin induced a rapid increase of the bile total bilirubin concentration and bilirubin-monoconjugate. In the 10 mg/kg group, the increase was larger than in the 5 mg/kg group and remained high up to 4 hr after injection, whereas in the 5 mg/kg group these bile components tended to decrease and returned to the preinjection levels within 4 hr. The total bile acid concentration in the bile remained unchanged in both groups. The bile flow did not change in the 5 mg/kg group, but in the 10 mg/kg group it decreased after bilirubin loading. Significantly positive correlations were observed between total bilirubin and bilirubin monoconjugate concentrations in bile both before and 1 hr after injection, but the slopes of regression lines were markedly different from each other. These results suggest that a bilirubin load surpassing the conjugating capacity of the hepatocytes increases bile bilirubin monoconjugate and also enhances the conjugating activity in the liver.

Post-embedding immuno-electron microscopy with rapid freezing and freeze-substitution techniques for the localization of manganese superoxide dismutase.

Dehydration of specimens with ethanol or acetone makes it impossible to detect manganese superoxide dismutase (Mn-SOD) by immunohistochemistry. To circumvent obstacles and demonstrate localization by post-embedding immuno-electron microscopy, a rapid freezing and freeze-substitution technique was employed using Lowicryl K4M embedding medium. This was effective enough to allow the specific observation of immunogold particles for Mn-SOD on the mitochondria of cardiac muscle cells and WI-38 cells (human normal fetal lung diploid cells). This method preserved the antigen-antibody binding activity of Mn-SOD even after dehydration. Therefore, rapid freezing and freeze-substitution is useful for post-embedding immuno-electron microscopy of Mn-SOD and can further be employed for other antigens previously difficult to detect by conventional methods.

Serum-free cultivation of anchorage-dependent cells on microcarrier: effective production of human macrophage colony-stimulating factor.

For the purpose of establishing a large scale production process of biologically active substances by cultivation of anchorage-dependent mammalian cells, basic studies were carried out on the following items; establishment of a new cell line and derivation of high productivity; construction of optimal serum-free medium; optimization of cultivation method using microcarrier in serum-free medium; and establishment of purification process. The cell line, TRC-29SF, used in this study was newly established from human renal carcinoma with a function of producing macrophage colony-stimulating factor constitutively. Improvement of M-CSF productivity upon TRC-29SF cell line was performed by M-CSF gene amplification with dhfr-MTX system and by truncation of membrane-binding amino acid sequence by recombinant DNA technique. Two kinds of serum-free media, IPEG-85 and IREG-89, were formulated for the growth of TRC-29SF cell and its transformant, respectively. A new cell-adhesion method which permits homogeneous attachment to microcarrier in short term was developed by equalizing the sedimentation velocity between cells and microcarrier by addition of 7% Ficoll into the medium. High cell density perfusion culture of TRC-29SF cells was achieved by microcarrier method using IPEG-85 medium, and final cell density reached over 10(7) cells/ml. Based on the results obtained, long-term perfusion cultures were performed using Mn10-5 and Mn10-5/R600 cell lines, which were created by M-CSF gene transfection and amplification. We found that the productivity of M-CSF per cell began to decrease from the end of logarithmic growth phase. Long-term cultivation with high productivity was accomplished by perfusing medium containing 2 mM sodium butyrate. Purification process for M1-CSF from the culture supernatant of transformed cell line was also established.

Effects of pirenzepine, AF-DX 116 and gallamine on the release of catecholamines from the dog adrenal gland in response to splanchnic nerve stimulation: interaction of M1 and M2 receptors with nicotinic receptors.

The present study was undertaken to examine how muscarinic antagonists modify the release of catecholamines evoked by splanchnic nerve stimulation (SNS) from the dog adrenal gland in vivo, in an attempt to elucidate whether muscarinic receptors play a functional role in catecholamine release. Output of epinephrine and norepinephrine was determined from adrenal venous blood by using high-performance liquid chromatography with electrochemical detection. SNS (1 and 3 Hz) produced increases in catecholamine output in a frequency-dependent manner. Intravenous administration of pirenzepine (10-100 micrograms/kg), a selective M1 receptor antagonist, or AF-DX 116 (30-300 micrograms/kg) and gallamine (0.3-3 mg/kg), selective M2 receptor antagonists, did not modify the SNS-induced increases in catecholamine output. C6 (hexamethonium) inhibited the SNS-induced increases in catecholamine output partially in a dose of 1 mg/kg and remarkably in a dose of 10 mg/kg. The combination of C6 (1 mg/kg) with pirenzepine (10 micrograms/kg), AF-DX 116 (30 micrograms/kg) or gallamine (0.3 mg/kg) inhibited the SNS-induced increases in catecholamine output more potently than C6 did by itself. The inhibition by C6 alone was about 50%, but that by each combination reached to about 80%. These results suggest that M2 receptors as well as M1 receptors play a facilitatory role in catecholamine release from the adrenal gland in response to SNS when the nicotinic receptor-mediated mechanism is partially inhibited.

Effects of pirenzepine and AF-DX 116 on ganglionic transmission in the cardiac sympathetic nerves of the dog: interaction of M1 and M2 receptors with nicotinic receptors.

Effects of the selective M1 receptor antagonist pirenzepine and the selective M2 receptor antagonist AF-DX 116 on ganglionic transmission were examined in anesthetized dogs, in order to elucidate a functional role of M1 and M2 receptors. Preganglionic or postganglionic stimulation of the cardiac sympathetic nerves (SNS, 0.5-16 Hz) produced frequency-dependent increases in heart rate. Pirenzepine (3-100 microgram/kg) caused dose-dependent and significant inhibition of positive chronotropic response to preganglionic SNS but not to postganglionic SNS. AF-DX 116 (10-100 micrograms/kg) had no effect on the preganglionic SNS-induced tachycardia. The simultaneous administration of pirenzepine (30 micrograms/kg) and hexamethonium (C6, 1 mg/kg), and the subsequent administration of 10 mg/kg of C6, inhibited more potently the tachycardic responses to preganglionic SNS than each dose of C6 did by itself. The enhancement by pirenzepine of the C6-induced inhibition was evident at high frequencies (8 and 16 Hz) of SNS. In contrast, the blocking effect of C6 (1 and 10 mg/kg) on ganglionic transmission was significantly attenuated by AF-DX 116 (30 micrograms/kg). The attenuation by AF-DX 116 was observed at a wide range of stimulation frequency (0.5-8 Hz). These results suggest that M1 receptors play a facilitatory role in ganglionic transmission but M2 receptors do not contribute to the transmission when nicotinic pathway is intact. However, the activation of M2 receptors would further suppress ganglionic transmission when nicotinic transmission is inhibited. Under these conditions, activation of M1 receptors would mediate the transmission elicited by high frequency of stimulation.

Isopropylidine maltoheptosyl fructofuranoside, doubly blocked substrate for determination of endoamylase activity.

We synthesized o-(4,6-o-isopropylidene-alpha-D-glucopyranosyl)-(1----4)- [o-alpha-D-glucopyranosyl-(1----4])5-o-alpha-D-glucopyranosyl-(1----2)- alpha-D-fructofuranoside (IPG7F) and developed an assay for determining the activity of amylase in human serum and urine by using this substrate. Glucoamylase, alpha-glucosidase, and mannitol dehydrogenase are used as coupling enzymes. The coupled reactions are monitored by continuously measuring the oxidation rate of NADH. In this procedure, various substances in the test specimens do not interfere with the detection of amylase activity. Exactly one molecule of NADH is oxidized by one attack of amylase on the substrate, although four products can be produced in the reaction. The within-assay coefficient of variation (CV) ranged from 1.0% to 4.1% and the between-assay CV ranged from 2.6% to 5.3%. The results of our new assay correlate well with those of the amylase assay involving p-nitrophenol maltoheptaoside as substrate (r = 0.978) and with those of the amylase assay involving maltopentaose (r = 0.987).

[A diffuse metastatic leptomeningeal carcinomatosis from gallbladder cancer; case report].

We encountered a 72-year-old woman with diffuse metastatic leptomeningeal carcinomatosis, who first suffered from occipital pain and died about a month after onset. On postmortem examination, gallbladder cancer (adenocarcinoma) was found to be the primary disease. We focused on its frequency and the metastatic route. On the metastatic route, we obtained the following results: tumor cells infiltrated only the cerebrospinal fluid, but not the areas surrounding the gallbladder cancer (spine or spinal cord) or into the brain parenchyma. A comparative study of the state of cerebrospinal fluid between the ventricle and the subarachnoid space disclosed that the cerebrospinal fluid pressure, cell count, and CEA and CA 19-9 levels increased more in the intraventricular cerebrospinal fluid, especially when the CEA level was higher than that in the serum. On histopathological examination, tumor emboli were seen in choroidal vessels in the ventricular wall, and tumor cells existed sparsely around choroidal secretory vessels. These results were thought to support the theory of hematogenous metastasis as Little et al proposed.

[Laparoscopic laser cholecystectomy: a prospective analysis of 70 initial cases].

Laparoscopic cholecystectomy using Nd-YAG laser was performed in 70 consecutive patients with preoperatively symptomatic gallstones. Sixty-eight patients had successful completion of the laparoscopic cholecystectomy. Mean hospital stay was 7.8 days (range: 4-12), and a diet as well as physical walking was tolerated in all 68 patients by the next day following the procedure. Mean operating time was 216 minutes in the initial 10 cases, which improved significantly to 87 minutes in the latest 28 cases. The difference of operative time did not influence any difference on the postoperative course. Nineteen of 68 patients (27.9%) required postoperative pain medication (pentazocine 15 mg, i.m.). Two patients required conversion to open cholecystectomy because of a pin hole injury on the portal vein by laser quartz in the first case, and difficult dissection due to severe adhesion in the 46th case. There is a definite learning curve in this new modality. Laparoscopic cholecystectomy is a safe and effective procedure, significantly improving the quality of life of the patients with gallstones.

[A case with a diagnosis of apparent functional sylvian aqueductal stenosis].

A case with a presumptive diagnosis of a functional sylvian aqueductal stenosis is presented. The patient was a 64-year-old female, who suffered from progressive gait disturbance and dementia. On admission, CT-scan showed lateral and third ventricular dilatations. The pattern of the ventricular dilatation suggested an aqueductal stenosis. But CT-scan showed no tumorous lesion. RI-cisternography, RI-ventriculography, aqueductgraphy, and continuous monitoring of the intraventricular pressure of the third ventricle were performed in order to make a diagnosis. The results of these examinations led to the following conclusion. "Although there was no apparent mechanical obstruction or stenosis, the presence of circulatory failure of CSF, demonstrated apparent functional stenosis of the aqueduct sylvii." A VP-shunt operation was performed a month after her admission. Her symptoms improved remarkably.

[Analysis of muscarinic receptors on rat pancreatic acini by using 125I-quinuclidinyl benzilate and N-[3H]-methylscopolamine].

To analyze muscarinic receptors on rat pancreatic acini, we studied the binding of 125I-quinuclidinyl benzilate (125I-QNB) and N-[3H]-methylscopolamine ([3H]-NMS) to these acini. Binding of 125I-QNB and [3H]-NMS to acini was specific and reversible. 125I-QNB bound to low affinity site, which was not recognised by [3H]-NMS. However, nonspecific binding of 125I-QNB to acini was very high (46%), so 125I-QNB may be inadequate to analyze muscarinic receptor on pancreatic acini. Muscarinic receptors are classified in two groups, M1 and M2, according to affinity of pirenzepine which binds to M1 receptor selectively. Pirenzepine was 530 times less potent than atropine in inhibiting the binding of 125I-QNB, and 250 times less potent than atropine in inhibiting the binding of [3H]-NMS. These results suggest that muscarinic receptors on pancreatic acini are mainly M2 receptors.

[An autopsy case of cerebral sinus thrombosis which showed papilledema and was accompanied by idiopathic thrombocytopenic purpura].

In a 53-year-old woman with papilledema and idiopathic thrombocytopenic purpura (ITP) brain CT examinations failed to detect the cause of papilledema, pseudotumor cerebri was suspected and lumbo-peritoneal shunt operation was performed to decrease the intracranial pressure. Unfortunately the patient died of intracerebral hemorrhage. The autopsy revealed superior sagittal sinus thrombosis to be the cause of papilledema, which indicate that sinus thrombosis can develop with such a hemorrhagic disease as ITP.

Endothelin: a potential modulator of cerebral vasospasm.

1-(5-Isoquinolinesulfonyl)-homopiperazine, HA1077, is a calcium antagonist with anti-vasospastic properties. This compound blocks intracellular actions of calcium in a variety of experiments. In the present study, we examined the effects of HA1077 on the vascular actions of endothelin, an endothelium-derived vasoactive peptide, in dogs in vitro and in vivo. Intracisternal injections of endothelin (0.01 nmol) produced a significant vasospasm, as measured by angiography, similar to that seen in the canine hemorrhage model. Infusion of HA1077 led to a significant dilatation of the spastic basilar artery in endothelin-treated dogs. The rank order of in vitro contractile activity in canine cerebral arteries was a stable thromboxane A2 analog greater than endothelin greater than 5-hydroxytryptamine greater than prostaglandin F2 alpha greater than histamine greater than noradrenaline. HA1077 effectively antagonized the endothelin-induced contraction of canine basilar arterial strips in both calcium-containing and calcium-free medium. The present results indicate that HA1077 is an effective antagonist for endothelin in vitro and in vivo.

Structure of genomic DNA for rat platelet phospholipase A2.

A genomic DNA for rat platelet phospholipase A2 was isolated by screening a rat genomic library with oligonucleotide probes based on its published amino acid sequence. The rat platelet phospholipase A2 gene had a total length of about 2.5 kb and contained five exons and four introns. The intron-exon structure of the rate gene was similar to that of human non-pancreatic phospholipase A2.

[CT and US findings of gauzeoma].

We described the CT and US findings of gauzeoma in 7 patients, in which the masses were histologically proved. In the wide range of the CT findings, whirl-like spongiform pattern and the wavy striped high density area were characteristic of gauzeomas. The typical US features of gauzeomas were a cystic mass with the distinct internal echoes and strong acoustic shadows. The internal echo were occasionally curve-linear in shape. Gauzeomas are not common disease. However, when the findings described above are seen in the patients who have previously had an abdominal surgery, gauzeomas should be suspected.

Effects of a novel alpha 1-adrenoceptor antagonist, SGB-1534, on adrenergically induced renal vasoconstriction in dogs.

The alpha 1-adrenoceptor antagonistic effect of SGB-1534, a novel phenylpiperazine derivative, was examined in the renal vascular bed of pentobarbital-anesthetized dogs. Renal nerve stimulation (RNS, 1 ms duration, 2, 3 and 4 Hz) or intrarenal bolus injection of methoxamine (0.5, 1 and 2 micrograms/kg) or guanabenz (1, 3 and 10 micrograms/kg) produced a frequency- or dose-dependent decrease in renal blood flow (RBF). Both the RBF responses to RNS and methoxamine were inhibited dose dependently by an intrarenal infusion of SGB-1534 (1-30 ng/kg per min) or prazosin (30-300 ng/kg per min). When the equipotent inhibitory doses of the antagonists were compared, the antagonistic potency of SGB-1534 on the RBF responses evoked by RNS and methoxamine was about 30 times greater than that of prazosin. Prazosin also attenuated the RBF response to guanabenz, whereas SGB-1534 had little effect. These results suggest that SGB-1534 has a selective alpha 1-adrenoceptor-blocking property and that it inhibits neurally mediated renal vasoconstriction. The alpha 1-adrenoceptor antagonistic potency and selectivity of SGB-1534 may be greater than that of prazosin.
