Ischemic heart disease 31 years after possible Kawasaki disease.

We present a 38-year-old woman who had experienced an acute febrile illness lasting more than 1 week at the age of 6, with erythema on the palms and soles following skin desquamation in the subacute phase and skin eruption. Thirty one years later, she experienced acute myocardial infarction and episodes of angina pectoris. She had no coronary risk factors or autoimmune diseases. Coronary angiography revealed an aneurysm of the proximal left coronary artery with occluded lesions at the distal site. The right coronary artery was also occluded at the proximal site. These findings strongly suggest Kawasaki disease should be considered in the differential diagnosis of early-onset ischemic heart disease in young adults.

Bacillus stearothermophilus cell shape determinant gene, mreC and mreD, and their stimulation of protease production in Bacillus subtilis.

Protease production stimulating genes were isolated from a soybean protein degrading bacterium, Bacillus stearothermophilus HA19. The cloned fragment stimulated production of a 37-kDa protease in B. subtilis. The nucleotide sequence of the genes and their flanking regions were identical to the B. subtilis cell shape determinant genes mreC and mreD [J. Bacteriol., 176, 6729-6742 (1992); J. Bacteriol., 176, 6717-6728 (1992)]. The mreC and mreD genes in B. subtilis stimulate secretion of a neutral protease (37-kDa), and the protease activity in the culture medium reached 2500 U per ml (approximately 10 times higher than the host strain) after 24 h of cultivation in L broth, suggesting the mreCD genes regulate protease expression and the protease is related to the cell shape determination in Bacilli. The protease productions in B. subtilis carrying mreC or mreD deletion plasmids were not elevated, so the 37-kDa protease stimulation requires both mreC and mreD genes. The extracellular protease was purified, and the molecular mass of the enzyme was 37,000 Da by SDS-polyacrylamide gel electrophoresis and gel filtration. The optimum pH and temperature for the enzyme activity were 7.0 and 50 degrees C, respectively, and the enzyme was stable at pH 7-10. The enzyme was inactivated by EDTA, but not by phenylmethyl sulfonyl fluoride and diisopropyl fluorophosphate.

[Drug induced hemolytic anemia associated with agranulocytosis].

A 27-year-old female was admitted to a hospital because of severe anemia (hemoglobin 4.9 g/dl) after taking PL (a drug for common cold consisted of Salicylamide, Acetaminophen, Caffeine and Promethazine methylene di-salicylate) and Cefadroxil (an oral antibiotic) for ten days. History and laboratory data leaded to a diagnosis of drug induced hemolytic anemia. 6 units of concentrated red blood cells were transfused and the suspected drugs were discontinued immediately. Though resolution of anemia and no further hemolysis were observed, progressive leukocytopenia developed since four days after the admission. Bone marrow aspiration revealed marked decrease of granulocytic series. The patient was transferred to our hospital and was isolated under laminar air-flow to prevent her from bacterial and fungal infections. She was treated with prednisolone and granulocyte-colony stimulating factor. She recovered from leukocytopenia in two weeks without suffering from any life-threatening infection. We extensively analyzed the suspected drugs and mechanism of hemolysis and granulocytopenia. Cefadroxil is turned out to be contributed to hemolysis by an immune complex mechanism. Cefadroxil and Salicylamide were suggested to be involved in granulocytopenia by the induction of antibodies against the leukocytes to which these drugs were bound. Thus Cefadroxil was regarded as a causative drug of both hemolysis and granulocytopenia. This case is of interest for analyzing drug-induced blood abnormality because it is very rare that two lineage of blood were injured by one drug at the same time as far as we know.

[Current network in Hokkaido University School of Medicine].

Recently campus LAN (Local Area Network) HINES (Hokkaido university Information NEtwork System) has been popularized rapidly in Hokkaido University. A lot of personal computers have been connected to HINES. Although many people in our school of medicine are coming to be familiar with the Internet, the network has not been utilized sufficiently yet. Establishment of efficient education and research with network, that is the essential purpose of HINES, is the problem to be solved in the near future. In this document, how to set up both modem and ISDN (Integrated Services Digital Network) is also referred for the help of access to HINES from outside of the campus.

[A clinical study on optimization of dose distribution in 60Co RALS for carcinoma of the uterine cervix using X-ray CT images].

From 1983 to 1992, 248 patients with carcinoma of the uterine cervix were treated with intracavitary radiation therapy using the 60Co remotely controlled high dose rate afterloading system apace (RALS). Five-year survival rates were 92,8 +/- 7 % for Stage I, 72.4 +/- 5% for Stage II and 52.1 +/- 4% for Stage III. The relations among the sequelae in surrounding organs, local control rates and the calculated dose in the rectum, sigmoid colon, bladder and small intestine were evaluated using X-ray CT images. The patients were treated with intracavitary radiation of 6 Gy/fraction at point A. A total of 5 fractions were delivered once a week. The dose calculation was performed by, and dose distribution shown on, a system developed in our hospital with a personal computer. The average values of maximum dose at certain points of the walls of the rectum and sigmoid colon were similar to point A dose. The incidence of late sequelae increased significantly in the group receiving a maximum dose to the rectum and sigmoid colon higher than 8 space Gy/fraction. This report describes the program for automatic optimization of dose distribution by modifying the hot spot (higher than 8 space Gy/fraction) and the cold spot (lower than 6 space Gy/ fraction).

[Improved method of staining RNA in platelets for the evaluation of platelet production in thrombocytopenic patients].

Measurement of RNA stained platelets was proven to be an easy and useful laboratory test to evaluate the state of platelet production in the bone marrow. As the normal value of RNA stained platelet with thiazole orange(TO) in the original method was low, the values of TO-stained platelets in the cases with platelet hypoproduction were within the normal range, but ideally, they should be below the normal range. We modified the original method 1) by using sodium citrate as the anticoagulant instead of EDTA, and 2) by keeping the TO-stained preparation at 4 degrees C until the fluorescence was measured. The normal value of TO-stained platelets was elevated to 22.5 +/- 3.3% (n = 40, M +/- 1SD) or 54 +/- 10 x 10(9)/1. Twenty-seven out of 40 thrombocytopenic case with ITP showed an elevated percentage of TO-stained platelets, 11 cases showed a normal and only 2 cases showed a percentage lower than normal. By contrast, 9 out of 12 cases with platelet hypoproduction showed a lower percentage of TO-stained platelets and no cases showed a value higher than normal. The sensitivity and specificity of this modified RNA staining method for distinguishing thrombocytopenic cases with platelet hyperdestruction from that with hypoproduction were 96% and 75%, respectively.

[Pneumonia caused by varicella-zoster virus in a patient with rheumatoid arthritis].

A 56-year-old woman suffering from rheumatoid arthritis, was admitted to our hospital for evaluation of fever and dyspnea. A month before admission, she had been given a diagnosis of herpes zoster and was treated with an antiviral agent. However, a perineal eruption persisted. A chest X-ray film and a chest CT scan showed many diffuse nodular shadows in both lung fields. With conservative treatment, the shadows regressed along with the skin eruption and other symptoms. Pneumonia caused by varicella-zoster virus was diagnosed from the clinical course, chest roentgenographic and CT scan findings, and serological data. The risk of mortality in varicella-zoster pneumonia is high in adults, especially in immunosuppressed patients. Early diagnosis and effective treatment are, therefore, essential in the management of this disease. Though varicella-zoster pneumonia is rare, chest roentgenographic and CT scan findings are characteristic and suggestive. This case may serve as a reminder of the features of varicella-zoster pneumonia: many nodular shadows on the chest X-ray film and CT scan.

Anatomical and physiological problems of Segment IV: liver transplants using left lobes from living related donors.

We reviewed the problem of Segment IV in using left lobes from living related donors, in 18 left-lobe transplants performed on pediatric patients who ranged in age from 6.0 to 17.3 yr, and in body weight from 19.8 to 58.0 kg. The separate monitoring of oxygen saturation of hemoglobin in the liver sinusoid of segments, using a spectrophotometric technique, demonstrated a delay in re-oxygenation of Segment IV after the portal reflow, and revealed its return to comparable oxygenation with Segments II and III by the re-arterialization. Hemoglobin content, which was determined by the same technique, occasionally increased in Segment IV during the operation, implying sluggish microcirculation caused by inadequate hepatic venous drainage. These characteristic profiles on tissue oxygenation and hemodynamics in Segment IV should be considered when using left lobes as living related liver grafts.

Insulin resistance and growth retardation in mice lacking insulin receptor substrate-1 and identification of insulin receptor substrate-2.

To clarify the physiological roles of insulin receptor substrate-1 (IRS-1) in vivo, we made mice with a targeted disruption of the IRS-1 gene locus. Mice homozygous for targeted disruption of the IRS-1 gene were born alive but were retarded in embryonal and postnatal growth. They also had resistance to the glucose-lowering effects of insulin, insulin-like growth factor-1 (IGF-1) and factor-2 (IGF-2). These data suggest the existence of both IRS-1-dependent and IRS-1-independent pathways for signal transduction of insulin and IGFs. Moreover, we identified tyrosine phosphorylation of a 190-kDa protein (pp 190) as a novel substrate (IRS-2) for insulin receptor kinase in livers of IRS-1 deficient mice which can bind both P13-kinase and Ash/Grb2.

[Two cases of interstitial pneumonia with anti-PL-12 (alanyl tRNA synthetase) antibodies].

We present two cases of interstitial pneumonia (IP) whose sera contain autoantibodies to PL-12 (alanyl tRNA synthetase). The first patient is a 47-year-old female who was diagnosed as IP and treated with corticosteroid at another hospital. She was admitted to Keio University Hospital due to worsening of dyspnea and polyarthritis. Laboratory studies revealed elevation of LDH and CRP, and her chest radiography showed interstitial fibrosis. Because of clinical deterioration, the dose of corticosteroid was increased (prednisolone 40 mg/day) and her symptom was stabilized. The second patient, a 55 year-old female, was admitted to Tokyo Metropolitan Ohtsuka Hospital because of dyspnea on exertion and polyarthritis. She did not show any symptom of myositis and was diagnosed as IP with arthritis on the basis of her clinical and chest radiography. She was treated with oral corticosteroid (prednisolone 30 mg/day), which resulted in improvement of her respiratory symptom and arthritis. Both patients were found to have autoantibodies to the PL-12. Autoantibodies to aminoacyl tRNA synthetases have been recognized as having a linkage with myositis mainly because of observations of the Jo-1 specificity. There was one report on a North American population that most but not all patients with anti-PL-12 antibodies had myositis. However, the clinical significance of anti-PL-12 has not been examined in Japanese patients. These patients suggested that anti-PL-12 antibodies have a stronger association with IP than myositis in Japanese patients.

Circulating ICAM-1, E-selectin, IL-2 receptor, and HLA class I in human small bowel, liver, and small bowel-plus-liver transplant recipients.

Recently, soluble(s) circulating isoforms of intercellular adhesion molecule-1 (sICAM-1) and sE-selectin (formerly endothelial leukocyte adhesion molecule-1) have been described in normal human serum. Elevated levels have been reported in acute and chronic inflammatory disorders, including allograft rejection. In this study, plasma levels of sICAM-1 and sE-selectin were determined in groups of tacrolimus (FK 506)-treated adult patients following either isolated small bowel (SB), liver, or combined SB plus liver (SB/L) transplantation. Each molecule was measured at 1, 2, 4, 6, 8, and 12 weeks (all patients) and at 6, 9, and 12 months after transplantation (SB and SB/L only) by enzyme linked immunosorbent assay. Levels were compared with those of soluble interleukin-2 receptor (sIL-2R; a marker of lymphocyte activation) and soluble HLA class I (which has been reported to be elevated in liver transplant-related complications). Elevations above normal in mean plasma levels of sICAM-1 (2.4-fold), sE-selectin (1.8-fold), sIL-2R (10.6-fold), and sHLA class I (1.3-fold) were found in patients with stable isolated SB grafts during the first 12 weeks posttransplant. Except for sHLA class I, levels of each protein were subsequently reduced, up to 1 year posttransplant. However, further increases in sICAM-1 and in sIL-2R and sE-selectin levels were observed during episodes of SB rejection compared with stable grafts. Mean levels of all molecules were higher in patients with isolated SB grafts compared with those given liver or combined (SB/L) transplants, either during stable SB graft function (up to 12 weeks posttransplant) or rejection. The data demonstrate increased adhesion molecule production/shedding following SB transplantation and are suggestive of a reduced overall level of immune activation in liver and SB/L compared with isolated SB transplantation.

Circadian variations of prostaglandins D2, E2, and F2 alpha in the cerebrospinal fluid of anesthetized rats.

Circadian variations in the levels of prostaglandins (PGs) D2, E2, and F2 alpha were studied in the cerebrospinal fluid (CSF) of rats. We collected CSF samples from the cisterna magna of anesthetized rats at different clock-hours (1000, 1400, 1800, 2200, 0200, and 0600 hr), and measured the concentrations of the three PGs. PGD2, which appeared to be the most abundant among the three, showed a circadian variation; and the mean of the day-time levels (145 pg/ml) was significantly higher than that of the nighttime ones (111 pg/ml). Day/night variations were also noticed with the levels of PGE2 and PGF2 alpha; however, these levels remained 3-4 times lower than those of PGD2. The general day/night variation seen in the CSF concentration of PGD2 conforms well with the postulated role of PGD2 as an endogenous sleep-promoting factor acting on a certain brain surface area defined as its site of action.
