Prevalence of dementia of Alzheimer type and apolipoprotein E phenotypes in aged patients with Down's syndrome.

We investigated the exact prevalence of dementia of Alzheimer type (DAT) and apolipoprotein E (ApoE) phenotypes in 106 Japanese Down's syndrome (DS) patients. Among these patients 16 were diagnosed as having DAT. The prevalence of DAT was 0% in the 30- to 39-year-old group, 16% in the 40- to 49-year-old group, and 38% in those over 50 years old. The frequency of the epsilon4 allele in DS patients with DAT was 18.8%, which was considerably higher than that of nondemented DS patients (4.5%) and Japanese nondemented controls (6.7%). Especially, the frequency of the epsilon4 allele in DS patients who developed DAT under 50 years was significantly higher (28.6%). DS patients certainly develop DAT at earlier ages but the prevalence of DAT in each group of patients was lower than previously recognized. It is very likely that the ApoE epsilon4 is a risk factor for DAT even in DS patients with a genetic predisposition to Alzheimer's disease.

Induction of protection against oral infection with cytotoxin-producing Escherichia coli O157:H7 in mice by shiga-like toxin-liposome conjugate.

We have previously reported that purified Shiga-like toxins (SLT), SLT-I and SLT-II coupled with liposomes induced a substantial amount of anti-SLT-I and anti-SLT-II IgG antibody production, respectively, in mice. The levels of anti-SLT antibody in the sera of SLT-liposome-immune mice correlated well with the protection against subsequent challenge with SLT. In this study, mice were immunized intraperitoneally with the mixture of SLT-I-liposome and SLT-II-liposome and protection against oral infection with cytotoxin-producing Escherichia coli O157:H7 was evaluated. All of the mice that received immunization with the mixture of SLT-I-liposome and SLT-II-liposome were protected against subsequent intravenous challenge with 10 LD50 of either SLT-I or SLT-II. Eight weeks after primary immunization, mice were inoculated intragastrically with 10(9) CFU of E. coli O157:H7 strain 96-60. All SLT-liposome-immune mice tested survived without any apparent symptom while control mice died within 5 days. In addition, as shown by other antigen-liposome conjugates, SLT-liposome induced undetectable anti-SLT IgE antibody production while they induced substantial amounts of anti-SLT IgG antibodies. These results suggest that SLT-liposome conjugate may serve as a candidate vaccine that induces protection against cytotoxin-producing E. coli infection.

[Activation of astrocytes and ischemic damage following the transient focal ischemia].

Astrocytes play vital roles not only in the mechanical support of the central nervous system but also in the metabolism of neurotransmitters and in the transfer of nutritive substances to neuron. After ischemic brain injuries, it has been known that gliosis appears around degenerative regions and repairs these regions. Recently, accumulating evidence indicates that overexpression of S-100 protein, astrocyte-derived protein, is detrimental to neuronal cells in various pathological conditions. To confirm the astrocytic activation in cerebral ischemia, we examined immunohistochemical changes in S-100 protein and glial fibrillary acidic protein (GFAP) in the transient focal ischemia. Cerebral infarction determined by hematoxylin-eosin staining was slight on day 1 and further expanded on day 2 and 3. Thereafter, GFAP immunoreactivity was observed in boundary zone of the infarct area at 72 hours after the transient focal ischemia. On the other hand, S-100 protein immunoreactivities were markedly increased at 9 hours after the transient focal ischemia. After the infarct formation, the increase of S-100 immunoreactivity was observed in outside boundary of infarct area. These results suggest that astrocytic activation, which we would like to be called "pre-mitotic S-100 peak (PSP)", precedes the neurodegeneration following the transient focal ischemia, and should be distinguished from so-called gliosis observed in the post-neurodegeneration and GFAP-dependent astrocytic proliferation.

Cervical spinal epidural abscess following acupuncture: successful treatment with antibiotics.

A 67-year-old man with poorly controlled diabetes mellitus (DM) had acupuncture several times a month for chronic shoulder muscle stiffness. A few days after acupuncture in the posterior nuchal region, a low-grade fever and backache developed, and subacutely progressed. Finally he complained of gait disturbance, and then respiratory distress appeared. Magnetic resonance imaging (MRI) demonstrated high cervical epidural abscess with massive soft tissue inflammation and vertebral osteomyelitis. Conservative treatment with antibiotics was effective and it was well documented by following serial MRIs. This case suggested that needle acupuncture should be avoided for immunocompromised subjects such as patients with poorly controlled DM.

Multiple inflammatory pseudotumors of the liver associated with acute myeloblastic leukemia.

A 26-year-old man, diagnosed with acute myelogenous leukemia had multiple inflammatory pseudotumors (IPT) in the liver. The patient presented complete remission after remission induction therapy, and then showed right upper quadrant discomfort and intermittent fever. An ultrasonography disclosed multiple hypoechoic nodules in the liver. A biopsy of the nodules showed focal liver cell necrosis with scant inflammatory cells, compatible with IPT. After several courses of chemotherapy, the nodules in the liver increased. The second liver biopsy of the nodule showed fibrosis. Multiple IPTs in the liver should be distinguished from abscess and metastatic nodules.

[A case of systemic sclerosis complicated with pneumoperitoneum and pneumatosis cystoides intestinalis].

Here we report a case of systemic sclerosis (SSc) complicated with pneumoperitoneum and pneumatosis cystoides intestinalis (PCI). A 58-year-old woman who had been treated with oxygen therapy for chronic respiratory failure caused by interstitial pneumonia was admitted to Tokyo Metropolitan Ohtsuka Hospital because of abdominal fullness. On admission, laboratory data showed accelerated erythrocyte sedimentation rate, mild anemia, hypoalbuminemia, and elevated level of lactose dehydrogenase (LDH). Anti-nuclear antibodies, anti-RNP antibodies, anti-Topoisomerase I antibodies, anti-Ku antibodies, and rheumatoid factor were positive. Chest and abdominal roentgenograms showed free air under the diaphragm, dilatation of small intestine and colon, and multiple intestinal cysts. The diagnosis of pneumoperitoneum and PCI was made with laboratory findings and radiographic findings. Her symptoms did not respond to medications altering colonic motility and oxygen therapy. Finally, abdominal symptoms and radiographic findings caused by pneumoperitoneum and PCI disappeared three weeks after treatment with 1,500 mg/day of kanamycin sulfate. PCI is rarely associated with SSc. Possible pathogenic relationship between these diseases was discussed.

[A case of systemic lupus erythematosus associated with spinal epidural hematoma].

A 47 year-old Japanese female who showed transverse myelopathy (TM) due to spinal epidural hematoma diagnosed by MRI in the course of systemic lupus erythematosus (SLE) was reported. She was admitted to Keio University Hospital due to paraplegia, anesthesia of lower extremity, urinary disturbance. Neurological examination revealed transverse disturbance of Th 10. Lumbar spinal cord MRI showed irregular mass that located at epidural region of 9th-11th thoracic vertebrae. When the laminectomy of 9th-11th thoracic vertebrae was performed, hematoma (4.5 cm x 1.5 cm in size) was confirmed and removed completely. Post operative condition was stable and symptoms had been improving gradually. It has been reported that TM associated with SLE was closely related to myelitis. In this case, epidural hematoma was a major cause of TM and MRI was very useful for her diagnosis and treatment. This is the rare case of SLE associated with spinal epidural hematoma and was thought as a important case to consider the cause of neurological complication of SLE.

A new reduction technique for a patellar fracture.

We report 4 cases of patellar fracture with skin injury over the patella which were treated operatively immediately after onset with a new reduction technique using Ilizarov pins. This technique made it possible to perform early operative treatment even in the cases with skin injury over the patella which prevents conventional surgical approach.

Ocular arterial flow hemodynamics in patients with diabetes mellitus.

The purpose of this study was to investigate ocular blood flow hemodynamics in patients with diabetes mellitus. We used color Doppler sonography, in 22 normal subjects and 52 patients with (n = 25) or without (n = 27) diabetic retinopathy, to determine blood flow velocities and the resistive index of the central retinal artery. The resistive index of the central retinal artery in patients with diabetic retinopathy (0.85 +/- 0.09) was significantly greater (P < 0.01) than that in normal subjects (0.72 +/- 0.08) and in patients without diabetic retinopathy (0.81 +/- 0.09). The resistive index of the central retinal artery in the patients without diabetic retinopathy was also significantly greater than that of normal subjects (P < 0.01). The resistive index of ocular arterial flow was increased in the patients with diabetes mellitus and further increased in the presence of retinopathy. Increased resistance in the peripheral ocular vascular bed contributes to diabetic retinopathy, and this change is present before the appearance of overt diabetic retinopathy.

[Scrotum exfoliative dermatitis with ulcers associated with treatment of acute promyelocytic leukemia with all-trans retinoic acid].

All-trans retinoic acid (ATRA) induces complete remission (CR) in most cases of acute promyelocytic leukemia (APL). Toxicity of ATRA has been shown to be mild and consist of headache, dry skin, dermatitis, gastrointestinal disorders, and hypertriglyceridemia. We report three patients with APL treated with ATRA in combination with chemotherapy, who developed scrotum exfoliative dermatitis with ulceration. Their age was 33 years (range, 25 to 37). All three cases developed scrotum erosions, and many small ulcers after 9 to 17 days of ATRA treatment. The scrotum exfoliative dermatitis with ulceration occurred repeatedly, but gradually resolved in about 8 weeks time. They developed no dryness of the lip or skin apart from the scrotum. All three cases continued to receive 45 mg/m2 of ATRA daily throughout induction therapy, and achieved CR. We suspected the scrotum exfoliative dermatitis with ulceration to be a side effect of ATRA. The scrotum lesions, which have been already reported may be common in patients receiving ATRA.

[Rectal cancer].

We examined the diagnosis and treatment of recurrent rectal cancer with particular regard to localized recurrence. Early diagnosis of localized recurrence is extremely important for the selection of therapeutic strategy. Among strategy options available are surgical treatment, including resection of all pelvic organs, but this should be performed with no decrease in the QOL of the patient.

[Adenosine.amino acid].

Somnogenic effect of adenosine came to be widely known by experiments of the central administration of fowls and dogs in the early 1970s. This effect was subsequently confirmed in rats by the use of metabolically stable analogues of adenosine. Now, there is a hypothesis that adenosine, which is a metabolite of adenosine triphosphate (ATP) and the main source of energy, conveys a feedback signal to check the rise in the consumption of ATP, and then induces the state of sleep with low consumption of energy. Based on the pharmacological and structural characteristics, the receptors for adenosine are now divided into A1, A2a, A2b, and A3. Cholinergic neurons in DBH, PPT, and LDT which are involved in the promotion of wakefulness, are presumed to be the target structure of adenosine to promote sleep by causing a depressant effect on the neuronal excitation through A1 adenosine receptor. On the other hand, it is becoming apparent that A2a adenosine receptors located in the ventral and rostral part of the basal forebrain are also involved in the promotion of SWS and PS with different mechanisms mediated by A1 adenosine receptor.

[Arterial infusion chemotherapy with SMANCS-Lipiodol for multiple hepatocellular carcinoma].

Fifty-five patients with hepatocellular carcinoma were treated with oily anticancer agent SMANCS dissolved in Lipiodol (SMANCS-LPD). The local response rate after the first arterial infusion in all patients was 39%, against 63% in 27 patients with Lipiodol accumulation occupying more than two thirds of tumor areas. The infusion therapy with SMANCS-LPD is adapted for a vascular-rich hepatocellular carcinoma. An infusion of 4 mg of SMANCS was ineffective for patients with tumors distributing in bilateral lobes of liver. Thus, an increase of infusion dosage or repeated infusions were recommended for such cases.

[Superselective angiographic findings of ipsilateral middle meningeal artery of chronic subdural hematoma in adults].

The authors reported the results of continuous superselective angiography of the ipsilateral middle meningeal artery (MMA) in cases of chronic subdural hematoma (CSH) in adults. MMA angiography was performed twice, at an interval of approximately two weeks, in 3 cases of conservative and 1 case of surgical treatment. The features of MMA angiographic findings were diffuse dilatation of MMA and visualization of scattered abnormal vascular networks (VN), which seemed to be macrocapillaries in the outer membrane of the CSH. In two out of the three cases of conservative treatment, these VN revealed a dynamic change temporarily and spatially, i.e. either enlargement or reduction. In one case with a long clinical course a stable MMA angiogram was seen. The operated case showed dramatic change on the second (postoperative) MMA angiogram. The VN around burr hole portion was huge and dark, and several newly visualized small arteries penetrated the enlarged VN, which was thought to have been caused by the operation. The mean blood pressure in the MMA was 103 mmHg. The reason for the acute enlargement of the CSH might be explained as arterial bleeding into the hematoma cavity, caused by rupture of thin walled macrocapillaries by direct arterial pressure.

[Hereditary macrothrombocytopenia].

A 19 year-old male was referred to our department because of macrothrombocytopenia. His platelet count was 73,000/microliter and giant platelets were observed in the peripheral blood smear specimen. Though he had been suffering from severe atopic dermatitis for four years, he seemed to be healthy without bleeding tendency. When he underwent a shunt operation for tetralogy of Fallot without any complication at nine-years old, thrombocytopenia was allegedly pointed out for the first time. Bone marrow aspiration revealed no abnormal findings with no chromosomal aberration. Normal platelet aggregation responses against adenosine diphosphate, epinephrine, collagen, and ristocetin were observed. The platelet adhesiveness (modified Salzman method) was slightly elevated. Unlike other reported syndromes associated with macrothrombocytopenia, his leukocytes had no inclusion bodies. His mother also had macrothrombocytopenia thus, this disorder was suspected to be hereditary.

Expression of facilitative glucose transporter isoforms in lung carcinomas: its relation to histologic type, differentiation grade, and tumor stage.

Expression of facilitative glucose transporter (Glut) isoforms was studied immunohistochemically in lung carcinomas. Glut-1 was expressed in 45 (74%) of 61 lung carcinomas, including 19 (100%) of 19 squamous cell carcinomas. No Glut-1 staining was seen in normal lung epithelium surrounding the tumors. In squamous cell carcinomas and small cell carcinomas, Glut-1 immunostaining was stronger in the central area of tumor cell nests corresponding to the hypoperfused region. Focal staining was seen in 14 (58%) of 24 adenocarcinomas, and positive staining was correlated to lesser differentiation, larger tumor size, and positive lymph node metastasis. Glut-2 was detected in normal airway epithelium, but no positive staining was seen in lung carcinomas. Glut-3 and Glut-4 were not positively stained in normal lung epithelia, but a few lung carcinoma samples showed positive reaction (9 of 61 in Glut-3; 4 of 61 in Glut-4). Glut-4 immunoexpression was seen in regenerating alveolar and bronchiolar epithelia around and in cancer tissues. Glut-5 expression was not detected in normal and tumor tissues. Reverse transcriptase-polymerase chain reaction for Glut-1, Glut-3, and Glut-4 confirmed the expression revealed by immunohistochemical analysis. Overexpression of Glut could enhance uptake of glucose into lung carcinoma cells, and the increased glucose influx could be involved in cell biologic activities.

[Two cases of polymyositis associated with interstitial pneumonia with anti-OJ (isoleucyl tRNA synthetase) antibodies].

We present two cases of polymyositis (PM) associated with interstitial pneumonia (IP) whose sera contain autoantibodies to OJ (isoleucyl tRNA synthetase). The first patient is a 51 year-old female who was diagnosed as rheumatoid arthritis (RA) and treated with gold and corticosteroid at another hospital. She was admitted to Keio University Hospital due to worsening of dyspnea on exertion and polyarthritis. Laboratory findings revealed elevation of serum CK and LDH. A diagnosis of PM was made based on the myogenic pattern of EMG and pathological feature by muscle biopsy. Chest radiography and CT showed interstitial fibrosis. Because of clinical deterioration, the dose of corticosteroid was increased (prednisolone 50 mg/day) and her symptom was stabilized. The second patient, a 62 year-old male, was admitted to Kawasaki Municipal Hospital because of dyspnea on exertion, polyarthritis, and fever. He was diagnosed as PM associated with IP on the basis of his clinical and laboratory findings, and chest radiography. He was treated with methylprednisolone pulse therapy (800 mg/day for three days) and his symptoms were improved. Both patients were found to have autoantibodies to OJ. Autoantibodies to aminoacyl tRNA synthetase have been described to be associated with myositis and/or IP. In North American, it was reported that all patients with anti-OJ had either myositis or IP or both. This suggests that anti-OJ was commonly associated with the anti-synthetase syndrome observed with other anti-synthetases. This is the first report of Japanese patients with anti-OJ antibody. The clinical features of these patients were likely to be similar to those observed in North American patients. However, further studies are necessary to clarify the precise clinical significance of this antibody.
