Vincristine, BCNU, doxorubicin, and prednisone (VBAP) combination in the treatment of relapsing or resistant multiple myeloma: a Southwest Oncology Group study.

A total of 151 patients with myeloma who had relapsed with or were resistant to melphalan and/or cyclophosphamide (Cytoxan) with prednisone received vincristine, carmustine (BCNU), doxorubicin (Adriamycin), and prednisone (VBAP) at 21-day intervals; 39 patients (25%) responded. Of the 123 patients with relapsing myeloma, 37 (30%) responded. Only two (7%) of the 28 patients with resistant myeloma responded. The regimen was well-tolerated in patients who had not had prior extensive irradiation. Thrombocytopenia was the most frequent form of toxicity. Survival was significantly longer in those patients responding to VBAP (median, 78 weeks) than in those not responding (median, 33 weeks) (P = 0.002). VBAP is a safe, effective regimen for patients with myeloma who relapse with alkylating agents.

A putative 5-HT central feedback in migraine and cluster headache attacks.

5-HT is considered to play a role in the migraine mechanism; its decrease in the midbrain during migraine attacks has been postulated. CSF TP and plasma and platelet 5-HT levels in migraine and cluster headache sufferers have been evaluated; the values registered were compared to those of neurological and carcinoid patients, respectively. The following results were obtained: (a) no significant difference in the time course of free and total TP after l-TP intravenous loading, (b) increased levels of CSF TP during migraine and cluster headache attacks, (c) impressively increased levels of plasma 5-HT only in carcinoid patients. These findings suggest the existence of a positive 5-HT central feedback mechanism during attacks of migraine and cluster headache and thus exclude peripheral 5-HT effects in the generation of pain.

Chemoimmunotherapy for multiple myeloma.

The effect of chemoimmunotherapy consolidation treatment using alternating courses of alkylating agents and BCG was studied in 105 responding patients with multiple myeloma. The survival time of these patients was similar to that of responding patients treated on previous maintenance programs, indicating no apparent value from BCG for myeloma. The duration of unmaintained remission was longest in those with low numbers of residual plasma cells, and relapse usually developed within one year in patients with persistent serum myeloma peaks. Disease recontrol was achieved in 50% of relapsing patients whose median survival from retreatment was 20 months. Patient follow-up without chemotherapy until relapse was justified mainly for those responding patients with disappearance of myeloma proteins.

Therapeutic strategies in multiple myeloma.

In the past decade a number of single agents and combinations of drugs have been introduced in the treatment of patients with multiple myeloma. In this paper special attention is directed to the analysis of prognostic factors and the identification of subgroups at the time of diagnosis, stable plateau phase following induction and at the time of relapse. A comprehensive approach to the treatment of patients with multiple myeloma is presented.

Combination therapy for multiple myeloma.

The effect of six different chemotherapy regimens were evaluated in 462 previously untreated patients with multiple myeloma. In comparison with other treatments, drug combinations that included vincristine and were given at 3-week intervals were associated with higher response rates and longer survival times. No gain was noted from the use of Adriamycin or from combinations of alkylating agents unless vincristine was given and the treatment intervals were short. Seventy-one responding patients were allocated at random to maintenance treatment with intermittent courses of either azathioprine--prednisone or a combination of melphalan--cyclophosphamide--carmustine (BCNU)--prednisone. The survival time was not prolonged with either maintenance treatment in comparison with that for responding patients continued on other therapies or on no therapy in previous studies. Attempts to reduce tumor was maximally with a change in the therapeutic modality, such as with immunotherapy or radiotherapy, remain to be evaluated.

Activity of netilmicin compared with those of gentamicin and tobramycin against enterobacteria and Pseudomonas aeruginosa.

The inhibitory activity of netilmicin against 500 isolates of gram-negative bacteria was compared with those of gentamicin and tobramycin. Netilmicin was considerably less active than tobramycin and slightly less inhibitory than gentamicin for Pseudomonas aeruginosa but was at least as active against Escherichia coli and Klebsiella pneumoniae as were the other two antibiotics. A few Klebsiella and Serratia isolates resistant to gentamicin and tobramycin were inhibited by netilmicin. All three antibiotics were strongly bactericidal for E. coli, K. pneumoniae and P. aeruginosa but had less lethal activity against the otherwise susceptible Serratia isolates tested. Some necessary precautions in reading minimal inhibitory concentrations on agar media are stressed, and some possible advantages of a 4-h bactericidal test, using a constant antibiotic concentration, are defined.

The causes of heroin addiction. A review of the literature. Part I.

The causes of heroin addiction have been studied and debated, and many theories have evolved. This article reviews the extensive literature on the subject and groups the material under the categories of Psychological, Pharmacological, Genetic, Socilogical, Social Protest, The Family, and Stepping Stone theories.

[Apolipoprotein abnormalities in low and very low density lipoproteins in a type V hyperlipemia (author's transl)].

Abnormalities in VLDL and LDL apolipoproteins have been observed in the serum of an 11 year old girl with a type V hyperlipoproteinemia (VLDL + chylomicrons). This was shown with polyacrylamide gel electrophoresis and also with two dimensional immunoelectrophoresis. The LDL contain peptides which are not found in normal apo LDL. In the VLDL, heterogeneity of the lipopeptides is more marked that in normal VLDL, and there is an abnormal distribution of the apolipoproteins C or D.

[An immunoelectrophoretic study of plasma lipoproteins in a case of familial deficiency of lecithin-cholesterol acyltransferase (author's transl)].

The lipid composition and apopeptide patterns of plasma LDL and VLDL have been studied in a patient with LCAT deficiency, using two-dimensional immunoelectrophoresis. After absorption of a fat meal an elevation of VLDL apopeptides C was observed. Apo-LDL also showed an abnormal relative increase of apo-C. After heparin-induced lipolysis, a shift of apo-C from chylomicra and VLDL to LDL was noticed by the striking increase of apo-C in this last lipoprotein class. In view of the present and previously published results the possible mechanisms involved in lipoprotein catabolism in LCAT deficiency are discussed.

[Immunologic study of human low density lipoproteins].

The human serum Low Density Lipoproteins (d : 1,020 - 1,055 g/ml) is composed of lipoproteins B and small amount components which can be precipited during an immunological reaction specifically with lipoproteins C antibodies. Moreover the ethyl-ether treatment induces a dissociation of the LDL into several subunits which have different immunological specificities.

[Two-dimensional immunoelectrophoresis of spontaneous dissociation of low density lipoproteins].

Plasma-lipoproteins isolated between d 1.020 and d 1.055 g/ml were partially delipidated with ethyl ether at 4 degrees C. This treatment induces a transformation of the lipoproteins which is evolutive during several days. Bidimensional immunoelectrophoresis reveals the lipoproteins dissociation and the appearance of 4 immunologically different fractions. The time dependent formation of these subunits is slowed down by EDTA and less efficiently by antioxydants. Once started, the dissociation can be accelerated by heating at 37 degrees C or by UV exposition. Another lipopeptide is more easily revealed by anti VLDL antiserum. It can be shown in the native and in the partially or completely delipidated LDL. Its presence does not depend on lipoprotein dissociation.
