Diastolic bulging of the interventricular septum toward the left ventricle. An echocardiographic manifestation of negative interventricular pressure gradient between left and right ventricles during diastole.

Diastolic bulging of the interventricular septum (IVS) toward the left ventricle was observed by real-time cross-sectional echocardiography in three patients with primary pulmonary hypertension and one patient with secondary pulmonary hypertension after closure of an atrial septal defect. M-mode echocardiography showed a characteristic abnormal pattern of septal motion in diastole and in systole. In two patients, we attempted to correlate M-mode motion to the interventricular pressure gradient. During diastole, the interventricular pressure gradient between the left and right ventricles was negative and the pressure gradient curve was very similar to the M-mode echogram of the IVS. Banding studies in which acute right ventricular hypertension was produced in dogs showed similar shape changes, suggesting that the diastolic shape and motion of the septum are determined by the interventricular pressure gradient between the ventricles. Diastolic bulging of the IVS toward the left ventricle in our patients results from negative interventricular pressure gradient between the left and right ventricles during diastole.

Horizontal and vertical optokinetic nystagmus in man.

Horizontal and vertical optokinetic nystagmus (OKN) was studied in 20 healthy adults. Although the horizontal OKN showed no directional difference, a statistically significant difference was found between horizontal and vertical OKN. Upward optokinetic pursuit was better on average than downward pursuit, but more variable. The inferiority of vertical OKN seems to indicate a suppression of optokinetic pursuit due to a different direction of the rotational axis from that of gravity. Regarding the vertical OKN findings, it is speculated that manifest directionality found in quadrupeds is modified in men by the change of their visual field on forward locomotion accomplished by their upright walking posture.

2-Deoxy-alpha-ecdysone from ovaries and eggs of the silkworm, Bombyx mori.

From ovaries dissected from developing and emerged adults of the silkworm, Bombyx mori, two substances having high molting hormone activity were isolated. One of these was identified as 2-deoxy-alpha-ecdysone by means of high-pressure liquid chromatography and mass spectrometry. Although this compound had previously been isolated from the fern Blechnum minus and postulated to be the precursor of alpha-ecdysone, it had not been obtained from insect material. The compound is also contained in the form of a conjugate in ovaries as well as in diapausing silkworm embryos.

Some methodological considerations for the clinical evaluation of neuroleptics--comparative effects of clozapine and haloperidol on schizophrenics.

For the purpose of evaluating objectively the characteristics of clinical effects of clozapine and haloperidol, a double-blind comparative trial was conducted on 91 schizophrenic in-patients. The changes of symptoms were assessed by means of several psychiatric and behavioral rating scales. The results were analyzed by means of nonparametric statistical methods. Furthermore, application of multivariate analysis was considered. After 12 weeks of treatment, though no significant difference in general improvement rate was noted between the two groups, a marked different spectrum of clinical effects was observed according to the rating scales. Clozapine is superior in anti-delusional, contact-promoting and sedative anxiolytic effect, while haloperidol is superior in anti-autistic effect. Among the side-effects, extrapyramidal symptoms appeared significatly more severe in the haloperidol group. The frequency of appearance of fever, was significantly greater in the clozapine group than the other. The four dropout cases of the clozapine group were all caused by fever. Through this clinical trial, clinical availability and validity of various statistical analytical methods were reconsidered.

Gallium-67 scanning in the evaluation of therapy of malignant tumors of the head and neck.

Gallium-67 scintigraphy was performed on 25 patients with a variety of malignant tumors of the head and neck in order to evaluate the susceptibility of the tumors to irradiation and chemotherapy before treatment and to monitor the effectiveness of therapy. If certain limitations are borne in mind, gallium scanning may prove a valuable tool in planning and evaluating therapy of malignant tumors.

Clinical evaluation of 67Ga-citrate scanning in the oral region.

Gallium-67-citrate was administered to thirty-nine patients who had a variety of benign, malignant, and inflammatory lesions of the oral region. The gallium-67 scintigram was noted to be valuable in the diagnosis of maxillary carcinoma, parotid carcinoma, and malignant lymphoma. Clinically, it is possible to differentiate malignant tumors from benign ones with the aid of 67Ga scanning. It is difficult, however, to differentiate malignant tumors from inflammatory lesions. At present, it is difficult to defect the small malignant lesion by means of a 67Ga scintigram.

Simple and effective method for selecting protein A-dificient mutants by consedimentation with sensitized sheep erythrocytes.

An effective slection method for protein A-deficient mutants was devised by means of cosedimentation with sensitized sheep erythrocytes and anti-protein A serum agar plates. Hitherto, two types of mutants have been successfully isolated by this method. One was deficient in both cell-bound protein A and soluble protein A. The other seems to release extracellularly soluble protein A but does not possess cell-bound protein A. Spontaneous mutants of the former type were contained in the culture of Staphylococcus aureus 248betaH in a concentration of approximately 10-minus-5, which is ten times as much as the latter. Repeated cosedimentation procedure greatly increased the efficiency of isolation of the mutants. As a result, a majority of the bacterial cells remaining in the final supernatant after three cosedimentation procedures were of the two mutant types. Details of the method as well as some preliminary results are described.

Two serotypes of exfoliatin and their distribution in staphylococcal strains isolated from patients with scalded skin syndrome.

Two serotypes of exfoliatin, A and B, previously reported by Kondo et al. (1974) were examined for their presence in 43 strains of Staphylococcus aureus, most of which were isolated from patients with Ritter's disease and impetigo. The tested strains consisted of 24 strains of phage group II and 19 strains not of phage group II. Twenty-four strains were found to produce an exfoliatin of either type A or type B, but 16 strains produced both types and 3 strains produced neither. No relationship was found between the serotype of an exfoliatin and the phage type of the exfoliative strain, although the single producers of exfoliatin A were all found to belong to phage group II and those of exfoliatin B to the other phage group.
