Histologic features and bile duct lesions in the alcoholic.

Histologic features of 112 liver biopsy specimens from patients with alcoholic liver injury were studied. Alcoholic hyalin was found in 6 specimens (5.4%). Spherical giant mitochondria were detected in 9 specimens (8.0%). Fatty metamorphosis, infiltration by polymorphonuclear leucocytes, and pericellular fibrosis were observed in many specimens. Zonal or submassive necrosis was also found in 20 specimens (17.9%). Cholestasis was seen in 32 specimens (28.6%). Histologic changes in interlobular bile ducts were investigated in 103 specimens containing such ducts. Among 31 specimens with cholestasis, changes in interlobular bile ducts were detected in 19 specimens (61.3%), whereas biopsy specimens without cholestasis showed the changes in only 4 of 72 (5.6%). Changes observed in interlobular bile ducts included inflammatory cell infiltration and vacuolation of epithelium in most specimens. Swollen or flattened epithelial cells, karyopyknosis and eosinophilic degeneration were observed in many. These results suggest that the changes of interlobular bile ducts may be partially related to cholestasis in alcoholic liver injury.

Mechanical energy changes during treadmill running.

The purpose of this study was to investigate mechanical energy changes during treadmill running, using two biomechanical models of the human body: the multi-segment model and the center of mass model. Two university distance runners ran at 3.33 and 2.67 m/s on the treadmill, while the right side was filmed by a 16-mm high-speed movie camera (100 fps). The instantaneous mechanical energies of the multi-segment model composed of 11 rigid segments (ME total) and those of the C.M. model (ME C.M.) were calculated for two strides for each trial. The following results were obtained: the trunk segment was the major contributor to the body energy variations, compared to the four limbs; internal kinetic energy, which was the difference between the energy amounts of the two models, was found to have a phase relationship directly opposite to those of the C.M. model. These findings were contrary to the previous studies on walking and were seen to be important characteristics of running energetics.

[Noninvasive estimation of pressure gradient in the left ventricular outflow tract: an experimental study].

The relationship between systolic anterior motion of the mitral valve (SAM) and left ventricular outflow pressure gradient (PG) was examined in five dogs with experimentally-produced SAM. A total of 155 heart beats including 29 post-extrasystolic beats with various PG were analyzed. Correlations of PG with the time from the onset of left ventricular ejection to the onset of SAM-septal contact (SSC), SSC divided by ejection time (ET) (SSC/ET), and SSC/ET multiplied by the pre-ejection period (PEP) (PEP X SSC/ET) were obtained. The relation between the natural logarithm of PG (InPG) and SSC/ET was expressed by the linear regression equation: InPG = -5.16X + 5.19, with the correlation coefficient (r) of -0.88 for total 155 beats (r ranged from -0.75 to -0.96 for each dog), and the relation between InPG and PEP X SSC/ET by the formula of InPG = -0.075X + 5.35, with the r of -0.91 for total 155 beats (r ranged from -0.84 to -0.95 for each dog). These results indicated that the time from the onset of aortic ejection to the onset of SAM-septal contact is strongly dependent on the degree of PG.

Strong association of idiopathic membranous nephropathy with HLA-DR2 and MT1 in Japanese.

HLA-A, B, DR, and MT antigens were examined in 50 patients with idiopathic membranous nephropathy (IMN) by the standard microlymphocytotoxicity technique. The frequency of HLA-DR2 was significantly increased in the patients in comparison to the controls. Significant decreases in the frequencies were seen for HLA-DR4, DRw8 and DRw9 in the patients. The linkage disequilibrium between HLA-Bw52 and DR2 was noted in the controls, but it was significantly weaker in the patients. The frequency of MT1 was significantly higher, and that of MT3 was significantly lower in the patients as compared with the controls. The findings suggest that an immunological disturbance under the influence of genetic factors may confer a susceptibility to IMN.

Suppression by phospholipase A2 inhibitors of secretion of catecholamines from isolated adrenal medullary cells by suppression of cellular calcium uptake.

The involvement of phospholipase A2 in the secretion of catecholamines and cellular uptake of 45Ca2+ was investigated in isolated bovine adrenal medullary cells. In these cells, stimulation of cholinergic receptors by carbamylcholine causes the activation of receptor-linked Ca-channels and influx of Ca2+ is known to trigger the secretory process. Phospholipase A2 inhibitors, such as quinacrine, chloroquine, quinine and p-bromophenacyl bromide, all inhibited the secretion of catecholamines evoked by carbamylcholine in a dose-dependent manner. These phospholipase A2 inhibitors also inhibited the cellular uptake of 45Ca2+ evoked by carbamylcholine with similar dose-response curves to those for inhibition of catecholamine secretion. The inhibition by phospholipase A2 inhibitors was found to be distinct from inhibition by d-tubocurarine which competitively blocks acetylcholine receptors, and from inhibition by diltiazem which acts as a Ca-antagonist at Ca-channels. Phospholipase A2 inhibitors seem to suppress the secretion of catecholamines by interfering with the linkage between acetylcholine receptors and Ca-channels by the membrane effects including the inhibition of endogenous phospholipase A2 activity of the adrenal medullary cells.
