Intraglomerular deposition of fibrin/fibrinogen-related antigen in children with various renal diseases.

The localization of intraglomerular deposits of fibrin (Fb)/fibrinogen (Fg)-related antigen (FRA) in children with various glomerular diseases was determined by an immunohistopathologic method using an anti-Fg antibody capable of detecting FRA, an anti-D-dimer antibody capable of detecting crosslinked Fb (XLFb) and its derivatives (XLFbDP), and by a method using the effect of monochloroacetic acid (MCA) treatment on kidney sections. In proliferative glomerulonephritis (PGN), XLFbs were detected within the capillaries and extension beyond the mesangium was seen in severe PGN. The FRA within the mesangium of minimal or mild PGN was composed of the non-XLFb substance. The FRA within Bowman's space of most PGN had disappeared after MCA treatment, suggesting a non-XLFb substance. The presence of FRA within electron-dense deposits (EDD) suggested that FRA deposits are associated with immune-complex deposits in the glomeruli.

Karyotype evolution and multilineage involvement of Philadelphia chromosome-positive clones in blastic transformation of two patients with chronic myelocytic leukemia.

Simultaneous analysis of the cell morphology and karyotypes on single colonies was carried out in two patients with Philadelphia chromosome (Ph1)-positive chronic myelocytic leukemia in blastic transformation in order to clarify the origin of leukemic cells involved. Patient no. 1 was in a typical myeloblastic transformation and patient no. 2 in "basophilic transformation." Both patients exhibited karyotype evolution in blastic phase (BP), so that we could differentiate BP clones with additional chromosomal abnormalities from chronic phase (CP) clones with only Ph1 among single colonies. The number of single colonies yielding two or more analyzable metaphases was 18 in patient no. 1, and 19 in patient no. 2. Among these colonies, only three in patient no. 1 and none in patient no. 2 were from CP clones and 15 in patient no. 1 and 19 in patient no. 2 were from BP clones. Morphological examination revealed that not only blasts but also mature neutrophils, eosinophils, basophils, macrophages, and erythroblasts were derived from BP clones. These results suggested that (1) BP clones developed at the pluripotent stem cell level, (2) additional chromosomal abnormalities were not restricted to occur in a specific cell line representative in BP; and (3) BP clones, if not all, may retain capacity for maturation and differentiation.

[Re-evaluation of determinants of the cardiothoracic ratio using two-dimensional echocardiography].

To evaluate the correlation of cardiothoracic ratios (CTR) on chest radiography with the sizes of the four cardiac chambers, we studied 41 normal persons and 77 cardiac patients using two-dimensional echocardiography (2DE). The transverse cardiac diameter and the CTR were ascertained from their chest radiographs. Using 2DE, each cardiac chamber was evaluated three-dimensionally, by measuring the short and long diameters of each left (LV) and right ventricle (RV), the diameter of the RV outflow, the anteroposterior, transverse, and long diameters of each left (LA) and right atrium (RA). These values were corrected using the square root of the body surface area. The correlation coefficient for the CTR was greatest in the long diameter of the LA (r = 0.84), and was greater in the atrial diameters (r greater than or equal to 0.71) than in the other diameters except for the transverse diameter of the RA (r less than or equal to 0.51). The contribution of each cardiac chamber to the transverse cardiac diameter was evaluated in 34 patients with cardiac enlargement by a method using the null hypothesis, which we originally designed. Consequently, the contribution of the LV to the CTR was definitely proven in two patients, that of RV in two, that of LA in 15 and that of RA in nine. These results indicated that the CTR is strongly influenced by enlargement of the LA.

[Cerebral blood flow and oxygen metabolism in patients with Parkinson's disease].

The distribution of cerebral blood flow and metabolism is related to neuronal activity. Cerebral blood flow (CBF), cerebral metabolic rate of oxygen (CMRO2), and oxygen extraction fraction (OEF) in ten patients with Parkinson's disease and five age-matched normal control subjects were measured with positron emission tomography (PET) using 15O2, C15O2 steady state inhalational technique to investigate functional changes of the cortex and the basal ganglia in Parkinson's disease. All patients had no history of cerebrovascular disease and CT scan showed no abnormal findings except for moderate cerebral atrophy in only one patient. When the level of clinical disability was related on the scale of Hoehn and Yahr, one patient was stage I, four were stage II, four were stage III, and one was stage IV. Psychic symptoms which include hallucination, depression, and dementia were recognized in four patients. One of these four patients was mildly demented. Four patients were newly diagnosed and had never been treated with antiparkinsonian medication before. Before the patients had their PET study their antiparkinsonian medication was discontinued for more than three days. But in two patients PET study was performed without discontinuity of antiparkinsonian medication. The values for regional CBF and regional CMRO2 were lower in the patients than in the normal control subjects, especially in the frontal cortex there was a significant decrease of CBF and CMRO2. There was no discrepancy between CBF and CMRO2 both in the patients and the normal control. CBF and CMRO2 in the cortex and the basal ganglia were not correlated with the severity of tremor, bradykinesia, and rigidity.(ABSTRACT TRUNCATED AT 250 WORDS)

[A case of advanced gastric cancer responsive to intratumoral OK-432 injection].

An 83-year-old patient with advanced gastric cancer was given repeated endoscopic administration of OK-432 combined with systemic immunochemotherapy. The cancerous lesion gradually regressed following the treatment, and remarkable degeneration was observed in many tumor cells. Detailed immunohistochemical examination revealed that Leu 7-positive cells had markedly infiltrated the cancerous tissue. This finding suggests that NK cells may play an important role in tumor regression induced by intratumoral OK-432 therapy.

Electrofocusing pattern of fucosyltransferase activity in human leukemic cells.

Fucosyltransferase (FT) activity of normal lymphocytes, normal granulocytes, and various types of human leukemic cells and electrofocusing pattern of FT activity in human leukemic cells and normal lymphocytes were examined using asialofetuin as an acceptor. Levels of FT activity in normal lymphocytes were higher than those of normal granulocytes in which FT activity was almost undetectable. The FT activity was higher in blast cells of acute myeloblastic leukemia and chronic myelogenous leukemia in blast crisis than in blast cells of acute lymphoblastic leukemia and the chronic phase of chronic myelogenous leukemia. The level of FT activity was lower in cells of chronic lymphocytic leukemia than that of normal lymphocytes, but it was higher than that of normal granulocytes. Three main isoelectric forms of FT in leukemic blast cells were identified by isoelectrofocusing, and they each had a characteristic focusing point: around pH 4.5 (peak 1); pH 4.9 (peak 2); and pH 5.2 (peak 3). In blast cells of myeloid leukemia, the activity of peak 3 was markedly higher than those of peaks 1 and 2. In blast cells of lymphoid leukemia, the activity of peak 3 was also the highest, but the activity of peak 2 was higher than that in myeloid blast cells. In normal lymphocytes, the major isoelectric form of FT was focused at around pH 4.9 and peak 3 was undetectable. These results indicated apparent differences not only in FT activity but also in isoelectric forms of FT between myeloid leukemic cells and lymphoid leukemic cells.

Purification and characterization of high molecular weight human epidermal growth factor from human urine.

High molecular weight human epidermal growth factor (HMW-hEGF) was purified to homogeneity from human urine. The purification was achieved exclusively by the use of immunoaffinity chromatography and reverse phase high performance liquid chromatography (HPLC). The purified HMW-hEGF is composed of a single polypeptide chain with an apparent molecular weight of 30,000 and has pI of 4.0. Its N-terminal sequence was determined as Val-Ser-Asp-Gln-Asp-Asp-( )-Ala-Pro-Val-Gly-( )-Ser-Met-Tyr-Ala-Arg-( )-Ile- Ser-. The trypsin treatment of this protein gave hEGF like fragments. These results suggest that the obtained HMW-hEGF may play an important role in biosynthetic precursor of hEGF.

Noninvasive demonstration of underlying severe coronary disease in patients with vasospastic angina.

Dipyridamole test, isoproterenol test, and treadmill exercise test were performed in two groups of patients with vasospastic angina (Group 1: 10 patients with 70% or greater coronary narrowings, Group 2: 8 patients with narrowings less than 70%. The results were correlated with coronary anatomy. In Group 1, vasodilation of resistance vessels by dipyridamole elicited ischemic episodes in 9 patients and an increase in myocardial oxygen consumption by isoproterenol caused anginal attacks in 7 patients. None of patients of Group 2 showed positive responses to either drug. All patients of Group 1, and 3 patients of Group 2 gave positive responses to treadmill test. These observations show that several different mechanisms are involved in the pathogenesis of myocardial ischemia in patients with vasospastic angina. Pharmacological interventions have higher specificity than exercise tests in predicting coronary anatomy and are useful for the choice of therapy in the patients.

Oxytocin levels in maternal and fetal plasma, amniotic fluid, and neonatal plasma and urine.

Oxytocin was measured in maternal and fetal plasma, amniotic fluid and neonatal plasma and urine using a specific radioimmunoassay, following extraction procedures with Florisil. Maternal oxytocin levels rose progressively with advancing gestation, but there were no significant differences between oxytocin levels around the onset of labor. No diurnal rhythm of oxytocin was evident in maternal plasma during the third trimester. Maternal and umbilical plasma oxytocin levels at spontaneous delivery were significantly higher than those at elective cesarean section. Maternal oxytocin levels in four cases of post-term delivery were lower than those during normal late pregnancy; all four cases experienced uterine inertia. All amniotic fluid samples had detectable oxytocin levels and there were no significant differences between oxytocin levels in the second trimester and those in the third trimester. Oxytocin levels in neonatal urine were higher than levels in amniotic fluid and lower than in the umbilical artery. Neonatal plasma oxytocin levels gradually decreased and oxytocin levels of 7-day-old infants were significantly lower than those in the umbilical artery, but higher than those in adults. In conclusion, it appears that maternal oxytocin levels may not be involved in triggering the onset of labor but may play a role in the maintenance and reinforcement of labor.

Comparative effects of intracoronary vasodilators on restoring coronary perfusion during flow-reducing coronary stenosis in the dog.

The effects of intracoronary injection of nitroglycerin, adenosine, nifedipine and prostacyclin on restoring coronary perfusion during flow-reducing partial coronary obstruction in anesthetized dogs were studied. Coronary obstruction was obtained by inflation of an intraluminal balloon to decrease coronary blood flow and rate of rise in left ventricular pressure (dP/dt) by approximately 30 to 40 and 10%, respectively. Nitroglycerin (0.01 to 10 micrograms/kg per min) increased coronary blood flow and distal coronary pressure and decreased stenosis resistance associated with improved left ventricular dP/dt depending on its dose. In contrast, adenosine (0.3 to 1.0 micrograms/kg per min) decreased coronary blood flow and distal coronary pressure and intensified stenosis resistance associated with depression of left ventricular dP/dt. Nifedipine and prostacyclin caused divergent effects on the coronary circulation related to each dose. Nifedipine (0.01 and 0.1 micrograms/kg per min) and prostacyclin (0.01 micrograms/kg per min) increased coronary blood flow and distal coronary pressure and reduced stenosis resistance. Nifedipine (1.0 micrograms/kg per min) and prostacyclin (0.3 micrograms/kg per min) did not increase coronary blood flow, but reduced distal coronary pressure and intensified stenosis resistance. Thus, the vasodilators produced different effects on restoration of coronary perfusion during pliable severe coronary stenosis. Nitroglycerin and lower doses of nifedipine and prostacyclin improved coronary perfusion due to selective or preferential dilation of large coronary arteries. Adenosine and higher doses of nifedipine and prostacyclin had deleterious effects on the coronary circulation due to potent arteriolar vasodilation.
