[Thromboxane A2 could be involved in bronchial hyperresponsiveness to methacholine in asthmatic subjects but not in bronchitic subjects].

To determine whether the involvement of thromboxane A2 in bronchial hyperresponsiveness is specific to asthma, we examined the effects of a selective thromboxane synthetase inhibitor (OKY-046) and a cyclooxygenase inhibitor (indomethacin) on bronchial responsiveness to methacholine in patients with bronchial asthma and chronic bronchitis. The provocative concentration of methacholine producing a 20% fall in forced expiratory volume in one second (PC20-FEV1) was measured before and after oral administration of OKY-046 and indomethacin in eight asthmatic and 10 bronchitic subjects. Baseline FEV1 value was not altered by OKY-046 or indomethacin. The geometric mean value of PC20-FEV1 increased significantly (p less than 0.005) from 1.78 to 4.27 mg/ml after OKY-046 in asthmatic subjects, but not in bronchitic subjects. On the other hand, PC20-FEV1 was not altered by indomethacin in all subjects. It was concluded that the involvement of thromboxane A2 in bronchial hyperresponsiveness may be specific to asthma.

[A case of chronic traumatic aneurysm of the descending thoracic aorta with the aberrant right subclavian artery].

A 59-year-old man had developed gradual prominence of the aortic knob in a chest X-ray film after a blunt chest trauma. Two and a half years after trauma, an angiogram revealed the aberrant right subclavian artery and a saccular aneurysm at the thoracic descending aorta just distal to it. It was treated with patch closure of the entrance of the aneurysm under a temporary bypass. Pathological findings were tearing off of a medial layer of the aorta. Postoperative course of this case was excellent. It must be noticed that the traumatic thoracic aortic aneurysm had a high possibility of rupture even in the chronic stage.

[Lipoprotein metabolism analysis in arteriosclerotic diseases].

Lipoprotein metabolism was studied by analyses of apolipoproteins, cholesterol content in lipoproteins and electrophoresis. The findings obtained suggested that the apolipoprotein levels such as A1 and B are related with particle number of lipoproteins such as HDL and LDL, while cholesterol content in lipoproteins is affected by qualitative change in particles as well. In patients with diabetes mellitus (NIDDM), LDL cholesterol correlated with HbA1c, and cholesterol and apolipoprotein B in the beta-area on electrophoresis showed accelerated mobilities, which were mimicked by in vitro glycation of LDL. In coronary heart diseases, elevation of apolipoprotein B and a low level of HDL cholesterol were general findings. The B/A1 ratio could be a sensitive indicator for these diseases. Hyper HDL cholesterolemias, excluding the patients with prostatic cancer who had undergone estrogen-treatment, showed elevated levels of apolipoprotein E in alpha 2-area on electrophoresis. Heterogeneity in Hyper HDL cholesterolemia was implicated. Qualitative analysis of lipoproteins by our method is believed to be useful.

[Hepatic functional reserve and tumor size as prognostic factors in patients with primary liver cancer undergoing non-surgical therapy].

A prognostic study on 119 patients with primary liver cancer undergoing nonsurgical therapy was carried out to evaluate the relevance of hepatic functional reserve and tumor size to their cumulative survival rates. All patients were classified into the three groups of Child's classification (A, B and C) according to their hepatic functional reserve and were also divided into the five groups according to their tumor size. The cumulative survival rates of all patients at 1, 2, 3, 4 and 5 years after the diagnosis were 50.9, 28.3, 18.8, 13.5 and 6.7%, respectively. The cumulative survival rates of group V whose tumor occupied more than 40% of the liver area were significantly lower than those of the other tumor-size-groups. The survival rates of Child's group A were significantly higher than those of group B and C. But in those patients who were classified into group V according to their tumor size, there was no significant difference in their survival rates among the three groups of Child's classification. These results suggest that hepatic functional reserve as well as tumor size is an important prognostic factor in patients with primary liver cancer. But if the cancer once develops greater than 40% of the liver area, hepatic functional reserve diminishes in value as a prognostic factor.

[Relationship between cough threshold to inhaled tartaric acid and sex, smoking and atopy in humans].

It has been reported that angiotensin converting enzyme inhibitor (ACE-I) elicits dry cough more frequently in women than in men. This study was designed to evaluate whether airway cough receptors are more sensitive in women than in men. Cough threshold to inhaled tartaric acid was measured in 33 men and 29 women. In non-atopic and non-smoking subjects, geometric mean value of cough threshold in women was 10.0 (GSEM, 1.29) %, which was significantly (p less than 0.02) lower than that in men, 22.5 (GSEM, 1.30) %. In non-atopic men, the cough threshold was significantly (p less than 0.05) lower in smokers (9.3 (GSEM, 1.57) %) than in non-smokers. In non-smoking women, the cough threshold was significantly (p less than 0.02) lower in atopic subjects (4.2 (GSEM, 1.33) %) than in non-atopic subjects. These results demonstrated that airway cough receptors may be more sensitive in women, smoking men and atopic women.

[Immunogenetic factors influencing HBV carrier state, the seroconversion and the development of chronic liver disease].

In order to investigate the immunogenetic factors associated with hepatitis B virus (HBV) carrier state, the HBe seroconversion and the development of chronic liver disease, HLA typing were performed in 278 asymptomatic HBV carriers (ASC) and 110 patients with chronic B type hepatitis (CH). HLA typing was also performed in 178 vaccinees who had received hepatitis B vaccine. The significantly decreased frequencies of DR1 and DRw13 were found in ASC, CH and non-responders to HB vaccine. This suggests that DR1 and DRw13 may be associated with the elimination of HBV. The frequency of DR4.2 was increased in ASC, but decreased in CH. The seroconversion rate of DR4.2 positive CH as well as ASC was high. Therefore DR4.2 may have relevance to the seroconversion from HBeAg to anti-HBe.

[A case of coronary artery bypass grafting using bilateral mammary arteries in a patient with systemic lupus erythematosus].

A 40-year-old female who had been treated with steroid for 10 years because of systemic lupus erythematosus (SLE) was admitted to our department for triple-vessel disease with LMT lesion. She underwent bilateral internal mammary artery grafting to the right ventricular branch of RCA and the No. 7 segment of LAD. Intraoperative free flow was 70 ml/min in both bypass grafts. She dropped to severe shock with a high fever of 40 degrees C and decreased the white blood cell count of 900/mm3 on the 6th postoperative day. These clinical and laboratory findings were suggestive of rebound phenomenon of SLE or withdrawal syndrome of steroid therapy. She was treated with pulse therapy of methyl-prednisolone and her condition improved in two weeks. Histological findings of the right internal mammary artery revealed stenotic lesion of about 50%, but on June 24, 1988, postoperative angiography showed a good patency of both internal mammary arteries. Only a few reports of successful attempt of coronary artery bypass grafting for coronary lesion due to SLE are available in the literature. Some important problems concerning the surgical treatment as well as the therapy of steroid involved in this case were discussed.

[Effect of deep inspiration on maximum expiratory flow (Vmax) depends on basal bronchomotor tone in young healthy females].

The relationship between the effect of deep inspiration on Vmax and basal bronchomotor tone was studied by partial and maximum expiratory flow-volume curve in 16 young healthy females (20-21 years old). Effect of deep inspiration on Vmax (DI index; (PEF25-MEF25)/PEF25) significantly related to percent increase in PEF25 not only by inhalation of ipratropium bromide (r = 0.81, p less than 0.0002) but also by inhalation of salbutamol (r = -0.62, p less than 0.01). Furthermore, day to day variation of DI index significantly related to day to day variation of PEF25 (r = 0.68, p less than 0.005) but not to that of MEF25. These findings suggest that the bronchodilating effect of deep inspiration in young healthy females may depend on intensity of basal bronchomotor tone caused by tonic vagal nerve activity.

[A case of stage III ovarian carcinoma successfully treated with oral etoposide].

A 49-year-old woman with stage III ovarian cancer (undifferentiated adenocarcinoma), who refused combination chemotherapy for postoperative residual tumor, were successfully treated with daily oral etoposide in the outpatient clinic. The etoposide (50 mg/day) was first given orally and then a dose of etoposide was decreased to 25 mg/day because of alopecia and gastrointestinal toxicity. After seven months of treatment, the complete remission was confirmed by the second look laparotomy, and cytology of peritoneal surface revealed no signs of adenocarcinoma. This case suggests that daily oral etoposide therapy might be the alternative as a first-line and second-line chemotherapy of ovarian cancer in the outpatient clinic.

Mineralization induced by beta-glycerophosphate in cultures leads to a marked increase in collagenase synthesis by mouse osteogenic MC3T3-E1 cells under subsequent stimulation with heparin.

The clonally derived mouse osteoblast-like cell line MC3T3-E1 was shown to produce latent collagenase (approximately 0.2 units/ml) under stimulation with either heparin or parathyroid hormone in confluent cultures. However, it was found that MC3T3 E1 cultures which were first induced to undergo mineralization by the addition of beta-glycerophosphate and were subsequently stimulated with heparin showed an approximately ten-fold increase in collagenase synthesis. MC3T3-E1 cell collagenase from a small sample of serum-free culture medium was purified 49-fold to a specific activity of 200 units/mg protein with a yield of 14% by heparin-sepharose affinity chromatography and ion-exchange high performance liquid chromatography. This new mineralization-primed cell culture system may be a valuable model for the study of osteoblast collagenase.

[Usefulness of bone scintigraphy on diagnosis of malignant fibrous histiocytoma of bone].

We examined the roentgenographical features of 5 patients who were diagnosed histopathologically as malignant fibrous histiocytoma (MFH) of bone, especially on the imaging of bone scintigraphy. Two patients had multiple lesions on the initial examination, and two of 3 patients who showed solitary lesion on the first examination could be detected more pathological sites by bone scintigraphy which was studied within two months of the first investigation. On bone scintigraphy, the mixture of hot and cold lesions which was coincident with the osteolytic change on plan X-P could be noticed, and with the progression of osteolysis it had a tendency to increase the area of cold lesions. These features are similar to metastatic bone tumor which is the most questionable disease on diagnosis, and we conclude that bone scintigraphy will be absolutely necessary when MFH of bone is highly suspected in the clinical course.

Fragments of urinary fibrin/fibrinogen degradation products and cross-linked fibrin degradation products in various renal diseases.

In children with several kinds of glomerular disease, fragments of fibrin/fibrinogen degradation products (FDP) and cross-linked fibrin degradation products (XLFDP) in the urine were investigated by autoradiography using western blotting method. Results were compared with selectivity of proteins observed in cases of proteinuria, or with histological findings. Patients with nephrotic syndrome exhibited slightly increased amount of urinary FDP, consisted mainly of X and Y fragments. On the other hand, in cases of proliferative glomerulonephritis, such as acute glomerulonephritis, purpura nephritis, Ig A nephropathy, systemic lupus erythematosus, or hemolytic uremic syndrome, increased FDP, including XLFDP, was detected in the urine. In these cases, FDP was consisted mainly of fragments X, Y, and D-dimer, and could not be correlated with the degree of mesangial proliferation or with urinary protein selectivity. It was concluded that the increased urinary FDP and XLFDP were derived not only from filtration of plasma fibrinogen or FDP, but also from fibrinolysis of intraglomerular fibrin deposits.
