The probable role of superoxide produced by blast cells in leukaemic cutaneous spreading.

Leukaemic blast cells from 12 patients with acute leukaemia were examined in order to study their capacity to produce anion superoxide in the absence and in the presence of phorbol-myristate acetate (PMA). Blast cells are able to mount an oxidative respiratory burst upon challenge with PMA, as demonstrated by superoxide release. The production of anion superoxide by blasts committed to monocytic differentiation might be an additional factor contributing to the tissue damage observed in leukaemic patients.

Skin involvement in hemopathies: specific cutaneous manifestations of acute nonlymphoid leukemias and non-Hodgkin lymphomas.

The course of a hemopathy may be characterized by the appearance of cutaneous lesions. In such cases a correct histological investigation is needed, especially when the lesions have an aspecific appearance. It is important, therefore, to recognize the cases characterized by cutaneous involvement of typical systemic disease cells. Lymphomas and leukemias are the hemopathies which more frequently present specific and nonspecific cutaneous manifestations (with an approximate incidence of 3-40 and 6-50%, respectively). In the period 1977-1983 we examined 337 patients affected with acute nonlymphoblastic leukemia and 243 patients affected with non-Hodgkin lymphoma, in order to verify the incidence of specific cutaneous manifestations.

Platelet alpha granule release in diabetes mellitus.

We studied beta-thromboglobulin (BTG) and platelet factor 4 (PF4) plasma levels in 90 diabetic subjects; we found high plasma values of these proteins only in patients with complications. To provide further indications regarding the role of platelet activation in the pathogenesis of diabetic microangiopathy, we also tested mobilizable platelet content of BTG and PF4; we found these similar to those of normal subjects.

[Hematologic changes in renal neoplasms].

The main haematological parameters, particularly haemoglobin concentration and the numbers of leukocytes and platelets, were studied in 146 kidney tumour patients. Only 119 of the subjects studied later underwent nephrectomy. In 66 patients the tumours were limited to within the renal capsule (group I) while in 39 other subjects the tumours had gone beyond this limit and had invaded the perirenal tissue (group II). 27 patients had metastasis in one or more sites (group III). In the remaining 14 subjects (group IV) without proven metastasis, the surgical staging was not available since because of cachexia they were not undergone nephrectomy. More frequent signs were: anaemia (Hb less than 12 g/dl), mainly normochromic, found in a high percentage of cases (41% in group I, 59% in group II, 74% in group III, 79% in group IV); leukocytosis (G.B. greater than 8000/mm3: 38% in group I, 51% in group II, 37% in group III, 50% in group IV). Mild thrombocytosis (P = 350-500 X 10(3)/mm3) was found with notable frequency mainly in groups II and III (20% and 18% respectively). On the other hand leukopenia (G.B. less than 3500 per mm3) and thrombocytopenia (P less than 140 X 10(3) per mm3) were rather rare. The results of our study are also discussed in relation to the main findings in the literature.

In vitro fibroblast and dermis fibroblast activation by laser irradiation at low energy. An electron microscopic study.

The effects of laser irradiations have been estimated both from the clinical point of view (rate of reduction of the ulcerated area) and in ultrastructural terms. The modification produced by laser in human fibroblasts in culture have also been investigated. Under our clinical and experimental conditions, laser stimulates the secretion of fibroblasts, both in vivo and in vitro, for reasons that are still the object of research. After irradiation the formation of periodic collagen fibrils is not observed, so that complete ' restitutio ad integrum' of the dermal matrix does not occur.

[Hematological complications of viral hepatitis. Case-list contribution].

The incidence of haematological abnormalities has been evaluated in 1170 patients affected by acute viral hepatitis. Of these patients, 665 were HBsAg+, 427 HBsAg-, 31 with IgM anti-HAV, 26 non A and non B hepatitis. 21 patients were not classified due to lack of data relating to markers for HAV or HBV. Anemia, leucopenia, thrombocytopenia and pancytopenia were found with some frequency at the haemochromocytometric examination. It is emphasized that these haematological abnormalities are often of minor relevance whereas the rarer and more serious complications such as persistent bone marrow hypoplasia or pure aplastic anemia can present difficult clinical problems regarding prognosis and therapy: in 4 cases of bone marrow hypoplasia a recovery was obtained in 1 case only, whereas the remaining cases had a fatal outcome after a period varying from 20 days to 15 months from the appearance of pancytopenia. We suggest the advisability of a closer study of the haematological and immunological aspects in patients with viral hepatitis in order to discover those elements which are useful towards a better understanding of the etiopathogenesis of the haematological abnormalities studied above. Such a detailed study would also enable further improvement in existing therapeutic measures.
