[AIDS--TB complex in Guinea Bissau: an epidemiological updating].

In Guinea Bissau, West Africa's little country, since 1989 to 1995 June the first 3091 patients with tuberculosis have been tested for HIV-1, and HIV-2 infections. During the first two years of epidemic surveillance the HIV-2 seroprevalence among tuberculotics was respectively 14.89% and 17.26%. Since 1991 it has been noticed besides an increase of seroprevalence for HIV-2 up to 20,27% in 1992, a constant increase for HIV-1 infection, associated or not with HIV-2. The Authors conclude that the epidemiologic pattern of Guinea Bissau is particularly interesting because in this country two epidemics (for HIV-1 and for HIV-2) have been followed since the beginning among tuberculotics allowing to study the natural history and epidemiologic evolution with particular accuracy.

[Mortality rate correlated to AIDS and to other causes in i.v. drug addicts].

The authors observed over time a sample of 1212 drug addicts in the city of Bologna and pointed out that AIDS-related deaths are constantly increasing while deaths due to overdose, although high in the Emilia-Romagna region, are still steady since 1984. The mortality rate AIDS-related is higher in female drug users than in males, whereas males are more prone to die for overdose. HIV positivity is not a relevant factor associated to deaths for overdose and subjects aged between 25 and 44 years show the highest mortality rate. During the period 1984 to 1995, 126 drug addicts died (10.4% of the observed population) and the highest mortality rate was registered after 1989. No death was reported between 1977 and 1983.

[Not Available].

The authors briefly report the epidemiological status of HIV-1 and HIV-2 infections among patients affected by tuberculosis in Guinea Bissau and evaluate the therapeutic follow up in 177 patients contemporarily affected by TBC and HIV-2 infection. The evaluation of the surviving times has been done according to the Kaplan-Meier curve by considering the following determinants: 1) sex; 2) sputum positive for acid-fast bacilli; 3) age range: A) 10-30 years - B) 31-45 years - C) more than 45 years. The total number days/person was 1417. The drop out was moderate. The serum prevalence for HIV-2 in patients affected by tuberculosis in Guinea Bissau is about 20% but in the last two years HIV-1 infection is increasing. There is no significant difference in the surviving curves due to the sex. Tbc-HIV-2+ patients have a longer survival than tbc-HIV-1+ patients reported by European and North America epidemiological data. The present epidemiological data were obtained in an African environment and, due to the low financial support, it was not possible to correlate them with the immunological and microbiological ones.

Perinatal transmission of HIV-2 infection in malnourished children in Guinea Bissau.

Since there have been a few reports of pediatric HIV-2 infection. We therefore investigated the perinatal transmission of HIV-2 in 147 malnourished and 164 well-nourished children attending a health center in the northern part of Guinea Bissau. Specific HIV-2 antibodies were detected in 17 mothers and in 2 malnourished children, one of them with pediatric AIDS. This study demonstrates that mother to child transmission of HIV-2 infection occurs in Guinea Bissau and suggests that there is an increased likelihood of detecting HIV-2 infection in malnourished children. The high seroprevalence of HIV-2 in a rural population without known risk factors may represent a hidden threat to mother/child health.

In vivo genetic variability of the human immunodeficiency virus type 2 V3 region.

The principal neutralizing epitope of the human immunodeficiency virus type 1 (HIV-1) lies between two invariant cysteines in the third variable region (V3) of the viral envelope (gp120), and its amino acid sequence varies among different HIV-1 isolates. HIV-2 carries an analogous amino acid sequence between two cysteines of the V3 regions, but its functional similarity with the HIV-1 principal neutralizing epitope is uncertain. We studied the degree of genetic variation of the HIV-2 V3 region in fresh blood samples from 12 HIV-2-seropositive individuals from Guinea-Bissau. Polymerase chain reaction was used to amplify viral fragments of 465 bp containing the V3 region from cellular DNA. Nucleotide sequence analysis of the entire envelope fragment from each patient revealed that the degree of variation among field isolates of HIV-2 is comparable to that observed in the analogous region of HIV-1. Most of the HIV-2 isolates studied were highly related, suggesting the existence of a limited number of different viral strains in the cohort studied. Thus, the HIV-2 and HIV-1 V3 regions vary to a similar degree and may also have analogous functions.

[Digital radiology with photostimulable image plates and conventional radiology in the study of the interstitial pathology of the lung].

To evaluate the diagnostic possibilities of Digital Radiography (DR) and of Conventional Radiography (CR) in the diagnosis of fine chest lesions, 100 patients with AIDS were examined with both techniques. The gold standards were 67Ga scintigraphy and bronchial washing in 100 patients and Computed Tomography (CT) in 50 patients. The data obtained from DR and CR examinations were separately evaluated by 4 readers and subsequently analyzed by ROC curves with Standard Error calculation and by comparing the areas below the characteristic curves with Student's paired t-test (p less than or equal to 0.05). Statistical analysis showed DR and CR to be of equivalent diagnostic value in the study of interstitial lung diseases.

Unusual malignant tumours in 49 patients with HIV infection.

Between December 1986 and December 1988, the Italian Cooperative Group on AIDS-Related Tumours documented 49 HIV-related tumours other than malignant lymphomas (ML) and Kaposi's sarcomas (KS), predominantly among HIV-infected intravenous drug abusers (IVDA). Of 12 germinal testicular tumours collected, six were seminomas, two of which were pure embryonal and the other four embryonal mixed. Cervical carcinoma was observed in nine IVDAs (intraepithelial in eight and advanced, with rapid progression, in one). Lung cancer associated with HIV infection was reported in eight patients, of whom four had an adenocarcinoma, two a small cell carcinoma, one an epidermoid carcinoma and one a mesothelioma. All patients with non-small-cell-lung cancer (SCLC) were at stage III, while those with SCLC and mesothelioma had limited disease. Five out of eight presented with limited disease at onset. The median age was low; lung cancer occurred predominantly in young adults, of whom all but one were smokers. Three patients could not be treated; four died while on treatment because of progression of the neoplasia and one died of an overdose. Acute lymphoblastic leukaemia (ALL) was diagnosed in five patients. The immunophenotype was always Burkitt-like (L3), and acute myeloblastic leukaemia (M2) was diagnosed in one. Of the central nervous system (CNS) tumours, two cases of glioblastoma and one of medulloblastoma were described. Two cases of young adults with multiple myeloma and two cases of colorectal carcinoma were also reported. One case of chronic lymphocytic leukaemia, one anorectal carcinoma, one oral carcinoma, one pancreatic carcinoma, one thymoma, one kidney carcinoma, one malignant melanoma and thyroid carcinoma were also found.(ABSTRACT TRUNCATED AT 250 WORDS)

67Ga citrate scanning, X-ray computed tomography and chest X-ray in the study of interstitial pneumonia in patients with AIDS.

Problems in the investigation of pulmonary interstitial pneumocystis carinii in AIDS patients in the early phase with 67Ga, high-resolution CT scan and chest X-ray are discussed, and a clinical study of 12 individuals is presented. The combination of the three examinations is useful in reducing false negative cases. The use of 67Ga scanning, chest X-ray and high-resolution CT is also proposed in the follow-up of AIDS patients during remission of their lung pathology.

[Diagnostic accuracy of thoracic radiography in infectious pathology].

It is widely recognized that there may be various different reports on the same radiography, because of every single radiologist's subjective interpretation of semiological findings. The authors retrospectively reviewed the X-ray findings of 110 patients (294 chest radiographs) affected by infective pulmonary diseases. A new diagnosis was made, which was compared to the previous one. The quality of each radiograph was judged too, using a grade scale from 0 to 3. The authors confirmed the previous diagnoses in 65.30% of cases. Most of the discrepancies were in case of suspected hilar-mediastinal node involvement, and in case of evident previous pleuropathies. Statistical studies proved the quality of the radiograph not to largely affect the accuracy of the diagnosis in an extemporaneous and discontinuous judgement (P = 0.085).

[Contribution of chest radiology to the diagnosis of HIV virus infection].

After revising literature concerning Pneumocystis Carinii Pneumoniae (PCP), the authors undertook a close examination of the immunological system of the HIVab-positive patients that suffered opportunistic pneumopathy from PCP. Hundred-forty-three cases of HIVab-positive (mean age of 29), prevalently heroin drug-users, were studied. There were 13 AIDS, 26 ARC, 91 LAS cases and 13 patients with only Ab positivity for HIV. Four hundred chest radiographs were examined without previous knowledge of case histories. A small number of hilum and/or mediastinal lymphadenopathies was observed. Moreover, percentage increase in acute lung inflammatory diseases, mainly interstitial, was seen. These data were correlated to the worsening of the HIV infection and to the developing of LAS, ARC and AIDS. In a number of cases there was radiological evidence of progressed inflammatory episodes. The clinical pattern of PCP patients is dramatic and often fatal. Clinical-radiological cases with special reference to acute interstitial disease, caused by opportunistic agents, are presented.
