Acute myocardial infarction mortality in VA versus non-VA settings.

Mortality associated with acute myocardial infarction has declined steadily over the past 25 years. This study was conducted to compare in-hospital mortality among men treated in VA versus non-VA settings. Results supported declines in mortality since 1980 to an overall rate of 11%, as reported in published literature and found in the VA data base. Mortality among men treated in VA settings mirrored rates reported for non-VA settings, both overall and by age groups. Findings were shared with VA Medical Centers for use in identifying opportunities to improve clinical practice.

Prevalence of nosocomial infection in long-term-care Veterans Administration medical centers.

Prevention and control of nosocomial infections are major goals of institutional risk-management programs. However, variations in criteria and denominator parameters make comparison of rates across settings difficult. This study addressed those problems by reporting criteria used to identify infections and applying the same denominator across long-term-care facilities. Findings demonstrated a 9.8% prevalence rate for nosocomial infections in four long-term-care VA facilities. Most of the identified infections were consistent with Centers for Disease Control definitions of nosocomial infections. The most frequent indicators of nosocomial infections that did not fully meet those definitions were (1) documented symptoms, (2) antibiotic therapy, and (3) physician diagnosis.

Nurse involvement in Institutional Review Boards; the service setting perspective.

Access to nursing service settings and protection of human subjects for research purposes are facilitated by the institutional review board if nursing is represented on the board and nurses' research protocols are reviewed for scientific merit and human subject implications. A nurse researcher recounts experiences in achieving these goals within a service setting.

Attitudes and intentions of baccalaureate nursing students toward the elderly.

The primary purpose of this study was to develop instruments that reliably measure belief and behavioral intentions toward the elderly with control for social desirability set within the belief measure. Related purposes involved evaluation of the impact of a gerontologic nursing course on beliefs and behavioral intentions of nursing students toward the elderly. Data were collected over a three-month period from 153 volunteers in a baccalaureate nursing program. One group was tested both before and immediately after a gerontologic nursing course; three other groups were tested immediately, one, and two years after the course. A paired-comparison type belief scale and a multiple-act behavioral intention scale were developed and used in the study. The Kogan Old People and Marlowe-Crowne Social Desirability scales served as outside criteria. Both new instruments were found to be highly reliable. An association between beliefs and behavioral intentions above the .30 level was found between scores on the behavioral intention and Kogan scales, but not between behavioral intention and belief scores.

Urinary incontinence verification in elderly men.

This study considered the problem of verifying the amount and frequency of incontinent urine loss in an outpatient population. The sample consisted of elderly male community-dwelling veterans who experienced urinary incontinence. Two amount tests (60-minute and 3-day) were completed by 46 volunteer subjects and 7-day frequency records were kept by an additional 44 volunteers. Results indicated that 60-minute and 3-day absorbent pad tests for amount of urine loss classified 59% (n = 27) of the subjects the same way (normal or abnormal). Thus, the shorter test appears useful as a screening test and offers advantages of lower cost and more accurate reporting. If abnormal urine loss is not identified with the short test, the long test should be used to detect infrequent episodes. Three-day tests for incontinence frequency classified 86% (n = 28) of subjects the same as 4-day tests. Therefore, a 3-day test appears adequate to determine the number and pattern of incontinent episodes as well as associations with relevant events such as activity levels and fluid intake.
