Characterization of peripheral blood and salivary gland lymphocytes in secondary Sjögren's syndrome.

Secondary Sjögren's syndrome (SS) is defined as a condition of patients with sicca symptoms in association with a connective tissue disease such as rheumatoid arthritis. This study was designed to investigate the peripheral blood and affected minor salivary gland (SG) tissue lymphocytes with monoclonal antibodies in patients with secondary SS having rheumatoid arthritis. Minor SG lymphocytes of the patients and normals were determined in the fresh-frozen sections of the minor SG biopsy samples using monoclonal antibodies with immunoperoxidase technique. Peripheral blood of secondary SS patients revealed significant reduction in CD3+ and CD8+ cells. CD4/CD8 radio, HLA-DR+ cells, and B-cells were unchanged. SG biopsies showed varying degrees of lymphocytic infiltration with predominance of CD3+CD4+ cells located at the periductal areas. CD8+ cells were found to be in low numbers within the infiltrates. IgG- and IgA-producing plasma cells were both numerous in the biopsy samples. Our findings suggest that there is an alteration of lymphocyte subpopulations at the local site of inflammation in the salivary glands without, however, a corresponding alteration in the peripheral blood.

Immunohistological analysis of gingival lymphocytes in adult periodontitis.

Inflammatory periodontal diseases are mediated by interactions between the dental plaque and the components of the host immune system. This study was designed to analyse the phenotypic properties of gingival lymphocytes in adult periodontitis. Biopsies were obtained from 12 patients and aged between 35 and 55 years. The tissues were processed for both histopathological and immunohistological examinations. Gingival tissue lymphocytes were identified using monoclonal and polyclonal antibodies with the immunoperoxidase technique. All specimens revealed a significant degree of CD3(+) cell infiltration beneath the pocket epithelium, which is located adjacent to the bacterial plaque, compared to that on the oral epithelial side. CD4(+) and CD8(+) cells were evenly distributed within these infiltrates. Numerous HLA-DR(+) cells were also noted. The majority of plasma cells in the central lamina propria bore IgG isotypes. These findings suggest that T-cell mediated regulatory mechanisms play an important role in the pathogenesis of adult periodontitis.

Vitamin D-deficiency rickets and myelofibrosis.

This study was aimed at detecting the early appearance of myelofibrosis by bone marrow biopsy examination in children with vitamin D-deficiency rickets. Twelve children, aged 4 to 18 months, were evaluated. Only a minimal increase of the reticulin was shown in rachitic children without anemia in whom no other laboratory evidence of myelofibrosis was present. Early signs of myelofibrosis with increase of reticulin were present in rachitic infants with anemia. In patients of the same age with iron deficiency anemia, the bone marrow reticulin findings were normal. Bone marrow biopsy after successful treatment of rickets could be repeated in one patient with myelofibrosis; results indicated that the myelofibrosis with anemia associated with vitamin D-deficiency rickets is reversible by vitamin D treatment.

Histopathological changes in adult cryptorchid testes.

The histological specimens of 73 patients who presented with undescended testes after puberty were re-evaluated. None of the specimens revealed carcinoma in situ but in the specimens of 5 patients severe atypia was detected. None of the patients, including 5 with severe atypia, has shown any evidence of invasive testicular tumour during follow-up.

Primary salivary gland tumors. A report of 703 cases.

Seven hundred-three cases of primary epithelial tumors of the salivary glands diagnosed over a long period in two Turkish University Hospitals are presented. The patient age distribution, sex, and the locations and histological types of the tumors are discussed and compared with those in the literature.

Gastritis cystica polyposa.

Gastritis cystica polyposa was diagnosed in five patients between ages 29 and 61 years. All five had been operated on for peptic ulcer disease three to 26 years earlier, when gastroenteric anastomoses had been made. The presenting complaint was abdominal pain, nausea and vomiting, or gastrointestinal bleeding. The lesions, located on the gastric side of the anastomosis, consisted of polypoid mucosal changes associated with hyperplasia and cystic dilatation of glands. Radiologic and endoscopic findings were often nondiagnostic, and surgical intervention was necessary to relieve obstruction and to rule out carcinoma. Resection of the gastrojejunostomy site and choosing anastomotic techniques to minimize bile reflux into the stomach appear to be the preferred methods of treatment.

Tissue alpha-fetoprotein and human chorionic gonadotrophin in non-seminomatous testicular tumours. A comparative study with serum levels.

The correlation between tissue localisation and serum levels of alpha-fetoprotein (AFP) and human chorionic gonadotrophin (HCG) was studied in 20 tissue samples from 18 patients with non-seminomatous testicular tumours. Specific immunoperoxidase stains were used for tissue staining and serum measurements were made by radioimmunoassay. There was a close correlation between the tissue demonstration of AFP and HCG and elevated levels of serum markers. The cellular localisations of AFP and HCG were different and no cell stained concomitantly for both AFP and HCG. From these results and from the demonstration of tumour markers in the tissue of some patients with normal serum marker levels, it was concluded that immunocytochemical localisation of the tumour markers was more sensitive than serum measurements.

Peripheral nervous system involvement in Behçet's disease. Case report and review of the literature.

Central nervous system complication of Behçet's disease are well recognized. However, peripheral nervous system involvement has been rarely observed. A case of Behçet's disease with prominent peripheral nervous system involvement is reported in this article. The results of clinical, electromyographic, immunologic and electron-microscopic findings of a nerve biopsy are presented and discussed in the light of the literature.

Unusual metastases of renal cell carcinoma.

The case of a 55-year-old white male with recurrent metastases in the oral cavity and maxillary sinuses, as well as other organs, is presented. Local excision of the lesions, hormonal treatment and palliative measures for other metastases were used. The patient is alive 10 years after the initial diagnosis.

Histopathologic and microdensitometric analysis of craniotomy bone flaps preserved between abdominal fat and muscle.

In this study we have investigated the preservation of craniotomy bone flaps within the abdominal wall and the use of such bone flaps for cranioplasty in the early postoperative period. The craniotomy flaps were implanted into a "pocket" between abdominal fat and muscle of adult rabbits, and were later used as autologous grafts in cranioplasty. The flaps were analyzed by histopathologic and microdensitometric methods. In 36 of the flaps, new bone formation near the periosteum was observed. The bone flap-cranium relationship was supplied by a bony tissue. Microdensitometric analysis showed increased optic densities in the experimental group.

[Primary carcinoma of the choroid plexus. 2 case reports and review of the literature].

Primary Carcinomas of Choroid Plexus are seen rarely. In previous literature there were 37 cases which were reported according to Lewis and Russel-Rubinstein malignancy criterias. In this article, we describe 5 years old boy and 1.5 year old girl who suffered from cerebellar ataxia and intracranial pressure increase syndrome. In case 1, the tumor of the fourth ventricle was diagnosed by brachial angiography and ventriculography. In case 2, the diagnosis was established by CT scan. In case 1, the patient died during the postoperative course. In case 2, the postoperative time was unremarkable. A radiotherapy of 3 000 rads was applied to the posterior fossa. One year later, the patient had only mild ataxia.

[Papilloma of the choroid plexus of the 3d ventricle. Report of a case and review of the literature].

It's rare for CPP to fill the III ventricule. Up to date, 24 cases surgically removed have been reported. In this report, we present a 1,5 year old boy who administered because of severe cachexia and reactive coma. Ct scan revealed dilatation in lateral ventricles and a mass totally filling the III ventricle. The mass has been totally excised through foramen Monro by right frontoparietal craniotomy and transcortical transventricular surgical approach. While a post-operative recovery was being observed, the patient died in the 30 the day because of bronchopneumonia. The treatment of III ventricle CPP as all the other CPP, must be total resection. For this reason, preoperative CT scan carries a great value about giving information of the nature of the tumor.

Biopsy of apparently normal bladder mucosa in patients with bladder carcinoma and its prognostic importance.

A total of 100 cold cup biopsies were obtained from apparently normal vesical mucosa of 25 patients admitted to Hacettepe University Hospital between 1980 and 1981. Dysplasia was found to occur in 40.49, hyperplasia in 17.35, squamous metaplasia in 9.91 per cent. Dysplasia was shown to be increasing parallel with the increase in the grade and stage of the tumour. The frequency of dysplasia and hyperplasia in the recurrent and non-recurrent groups differed significantly (p less than 0.001). In other words, recurrence was seen more frequently in patients with positive random biopsies. Although recurrence phenomena cannot be explained on the basis of mucosal abnormalities alone, this study shows that these subendoscopic lesions do have some prognostic role in recurrences.
