Routine preoperative testing on patients undergoing elective and emergency surgery has identified life-threatening illnesses unknown to the patient. Over a six-year period, 34 patients were found to have such medical problems, including cancer, diabetes, heart disease, tuberculosis, pneumonia, Paget's disease, and sarcoid.

A patient had full-thickness loss of abdominal wall from clostridial myonecrosis. Initial care consisted of resuscitation, debridement, and transfer to a hyperbaric chamber facility. After control of sepsis, multiple enteric fistulas were managed by enterotomies, gastric and duodenal defunctionalization with closed-loop gastrojejunostomy, gastrostomy, and end jejunostomy. Good nutritional status was maintained with total parenteral nutrition over a three-month period. Total abdominal wall reconstruction was accomplished by rotation of bilateral tensor fascia lata musculocutaneous flaps. Reconstruction was successful as the patient was able to return to an active life.
Rejuvenation is a subject that has captured the imagination of the public as well as the interest of physicians. Long before plastic surgeons began performing operations, patients sought the help of reputable practitioners and charlatans alike to alleviate the outward manifestations of aging. This paper presents some of the men who offered remedies--some apparently ineffective or even potentially harmful--to those hopeful of restoring youth.

The last 16 years of the life of Sigmund Freud were spent in a brave battle against a massive intraoral cancer to which he finally succumbed at the age of 83. He was subjected to 30 operative procedures and endured daily manipulations of a cumbersome prosthesis worn to replace his resected jaw and palate. This study reveals medical details of his illness and suggests modern options of management of this tumor.
