The importance of gastric emptying in reflux esophagitis: an experimental research on pigs.

A study was conducted on pigs to evaluate the importance of gastric emptying rate in reflux esophagitis development. Gastric emptying was previously measured in 25 pigs. Then, the following operative procedures were carried out: Heller's cardiomyotomy, common bile duct ligature, cholecysto-gastric anastomosis, and extramucosal duodenal myotomy on 10 animals (group A); the same procedures except extramucosal duodenal myotomy on another 10 animals (group B); common bile duct ligature and cholecysto-gastric anastomosis on the last 5 animals (group C). Six months later, gastric emptying was measured again; whereas in group A a significant shortening of gastric emptying was found, the other 2 groups remained unchanged. Then, all the animals were sacrificed and the lower third of the esophagus was removed for histologic examination. In all the pigs undergoing cardiomyotomy (groups A and B) appearance of esophagitis was found. No signs of esophagitis were found in group C. The conclusions reached are cardiomyotomy is in all cases responsible for reflux esophagitis development; extramucosal duodenal myotomy is capable of shortening gastric emptying, but this does not affect reflux esophagitis development.

Clinical trial with arginine tidiacicate in symptomatic chronic persistent hepatitis.

In a double-blind study, 50 patients with chronic persistent hepatitis were assigned at random to two treatment groups. One group was given arginine tidiacicate (ATCA) at the dose of 400 mg tablets three times a day and the other given a placebo, both for 30 days. The group of patients receiving ATCA showed a clear-cut improvement of subjective symptoms and of the most important cytolysis and cholestasis parameters studied, such modifications being greater than in the placebo group. Drug tolerance was excellent, since no side-effects were observed.

Chylothorax as a complication of Waldenström's disease.

We describe a case of chylothorax associated with Waldenström's disease, interpreted as a rare complication of this disease. The chylothorax is probably caused by lympho-reticular infiltration of the thoracic duct. In non-traumatic chylothorax, occurring in an adult, Waldenström's disease should be considered as a possible primary cause.

[Guacetisal: a new molecule with balsamic-antiphlogistic action. Clinical study].

An open clinical trial was run on the therapeutic efficacy of a new balsamic-antiinflammatory molecule, guacetisal (presented as 500 mg Broncaspin capsules). The drug was given for a minimum of 7 days to 45 male and female patients with an average age of 64 yr suffering from variously serious diseases of the airways. The concomitant administration of fluidifying, balsamic, steroid, antipyretic and anti-inflammatory drugs was avoided, whereas various combinations of cardiokinetics, diuretics, theophyllines, and antibiotics were used where required. Clinical assessment of the parameters chosen pointed to the good therapeutic activity of the preparation, which is specifically indicated in inflammatory conditions of the airways, especially chronic forms subject to periodic recrudescence. Its equally good tolerability serves to make guacetisal extremely easy to use, with the results that its employment in non-specialist environments can also be recommended.

Practice effects for normal listeners' performance on a nonsense syllable test.

Each of 4 groups (N: 5) of normal-hearing young adults responded to the consonant-vowel-consonant-vowel Nonsense Syllable Test (Edgerton and Danhauer, 1979). Groups I and II on any one day for 5 consecutive days received one presentation of both Lists A and B at 16 and at 56 db re individual SRT, with (Group II) or without (Group I) visual reinforcement as to the correctness of the response. Groups III and IV received 15 randomizations of List A (non-reinforced) in a single session at 16 or 56 db SL, respectively. S's were tested individually diotically; verbal responses were phonetically transcribed using both audio and visual cues, and were scored phonetically. There were no significant differences in means between Groups I vs II as a result of reinforcement, but differences were noted in both groups for presentation levels and trials (days). No practice effects were noted in Groups III and IV. The NST may be used for multiple administrations within a single session, as in deriving performance-intensity functions, or in hearing aid evaluations, without biases due to practice.

[Alkaline phosphatase isoenzymes and alpha-1-fetoprotein in hepatic neoplasms].

A study was made of possible relations between the placental alkaline phosphatase-isoenzyme (PAP), alpha 1-fetoprotein and liver neoplasia. Ninety-six with various liver diseases were examined, including 10 with primary and 16 with metastatic carcinoma. It was found that both PAP and AFP may be present in a certain percentage of subjects in this pathology. PAP (observed in 30% of cases of primary carcinoma and 43.6% of metastatic forms, as opposed to only 5.6% in cirrhotics) proved much more specific than AFP, which was present in both types of cancer (albeit at much lower concentrations) and also in acute, active chronic and persistent hepatitis, and in cirrhosis of the liver. The theories presented in the relevant literature are discussed in an attempt to explain the "reawakening" of both PAP and AFP, especially in liver neoplasias.

[Detection of HBsAg by inverse hemagglutination].

The Authors present the actual knowledge about the hepatitis B associated antigen (HBsAg) and the now more used methodics for detection of its surface component (HBsAg); consequently they present the results of a screening executed between january 1976 and december 1979, directed to research the HBsAg into sera of 655 patients affected by various diseases, particularly liver diseases, using the reverse haemoagglutination test. With this technique, that is of very easy and rapid performance, together with a good sensibility (even though it gives a rather important deal of feigned positivity, yet these can be voidable by means of an adsorption test) the Authors have totally acquired a 8.7% of positive cases for HBsAg (with a majority of patients affected by liver diseases) and a 1.2% chronic carriers for this antigen.

[Epidemiological survey on symptomatology of subject exposed to carbon disulphide (author's transl)].

Some epidemiological parameters were recorded in a group of 185 workers of a rayon viscose factory. An higher prevalence rate of diseases and an higher relative risk for respiratory, digestive, cardiovascular and nervous morbidity in exposed people were found. In order to put into evidence the existence of chronic CS2 intoxication in non-occupationally CS2-exposed people, the authors suggest to investigate morbidity in people who live near these factories.
