Serum muscle-specific enolase in progressive muscular dystrophy and other neuromuscular diseases.

Serum muscle-specific enolase ( MSE , beta beta and alpha beta enolases) levels were determined in 162 patients with progressive muscular dystrophy (PMD) and other neuromuscular diseases by means of an enzyme immunoassay method. The relationships were examined between serum MSE , creatine kinase (CK) and other markers of muscle disease. Serum MSE was strikingly increased in Duchenne muscular dystrophy, and this elevation was more prominent in younger patients. Serum MSE was also increased in other types of PMD and certain other diseases. Serum MSE showed the highest correlation with CK. In the PMD group, the frequency of cases with elevated MSE was the same as in CK. These results indicated that serum MSE may well be a specific marker of muscle disease on a par with CK.

Cancer of gallbladder with severe thrombocytosis.

An autopsy case of severe thrombocytosis associated with cancer of gallbladder is described as the first case in the literature. A 78-year-old male patient was admitted to the hospital with abnormal hepatic functions on August 6, 1975. Laboratory examination revealed platelet count of 5,664,000/cu mm, anemia, leukocytosis and bilirubinemia. Jaundice was progressive and the patient died after a month. Severe thrombocytosis persisted throughout the course. Main autopsy findings were cancer of gallbladder with metastasis to the liver, fibrosis of spleen and purulent bronchopneumonia. A possible mechanism of unusually severe degree of thrombocytosis associated with the present case is discussed in light of the literature.

Serum carbonic anhydrase III in myotonic dystrophy.

Serum carbonic anhydrase III (CAIII) levels were determined by means of an enzyme immunoassay method and were compared with serum creatine kinase (CK) and muscle-specific enolase (MSE) levels in 33 patients with myotonic dystrophy. Serum CAIII levels were elevated in all 33 patients, whereas serum CK and MSE levels were elevated in 12 and 10 patients, respectively. Serum CAIII levels showed a good correlation with CK levels, but a poor one with MSE levels. There was no obvious correlation between the serum CAIII level and the duration of illness or the age of the patient. These results suggest that serum CAIII is probably a more sensitive marker than CK and MSE in myotonic dystrophy and may also reflect the type 1 fiber abnormality more predominantly observed in myotonic dystrophy.
