One hundred consecutive patients with cerebral metastases from malignant melanoma were studied in relation to survival from the time of diagnosis of central nervous system (CNS) involvement, response to treatment, characteristics of their primary lesion, and the course of the disease from initial diagnosis to the development of intracranial tumor. After treatment, clinical and investigational evidence of objective regression of cerebral lesions was demonstrable in ten patients who survived with a median of 11.5 months from diagnosis of CNS involvement. In 11 additional patients, CNS disease remained clinically stable for a median period of 7 months. Median survival for the entire group of 100 patients was 2.5 months. However, eight patients (8%) survived longer than 1 year from the time of diagnosis of cerebral metastases, four of whom (4%) survived longer than 2 years; the longest survivor being disease-free and incomplete neurologic remission at more than 82 months. Patients with malignant melanoma metastatic to the brain should be informed of the therapeutic options available for their disease.
Seven patients with Heredopathia Atactica Polyneuritiformis are described. There were 3 groups of clinical characteristics: first, congenital abnormalities such as skeletal deformities, unrelated to phytanic acid levels; second, signs and symptoms such as retinitis pigmentosa which come on slowly and are not directly related to the plasma phytanic acid level; third, lesions such as the neuropathy, rash and cardiac arrhythmias which can change quickly and are linked to the plasma phytanic acid level. Exacerbations of the signs in the third group are precipitated by a low calorie intake and mobilisation of phytanic acid from the adipose tissue. Poor vision is due not only to retinitis but also to small pupils which fail to dilate in the dark. The use of plasma exchange is discussed. With good dietary supervision it is possible to produce a considerable clinical recovery for the neuropathy.

Seventeen patients with metastatic disease from malignant melanoma received treatment with injections of Human lymphoblastoid interferon. Fifteen of these had the drug by intramuscular injections. One started treatment by the intramuscular route but later proceeded with slow intravenous infusions. The remaining patient had interferon injected into the tumor only. With one exception, all patients had been pretreated with other methods for advanced regional or blood-borne metastases. A partial response (UICC criteria) lasting six months was seen in one patient who was treated with intramuscular injections for intransit cutaneous metastases in one leg. Human lymphoblastoid interferon appears to have minimal activity relating only to skin metastases in advanced and previously treated malignant melanoma. This activity is not superior to existing treatments.

A patient with severe heredopathia atactica polyneuritiformis (Refsum's disease) has responded well to treatment by large-volume plasma-exchange. The dietary control of the disease has been shown to depend not only on a diet low in phytanic acid, but also on one high in calories, which prevents mobilisation of phytanic acid from fat stores.
The antitumour effect of vindesine (desacetyl vinvlastine amide sulfate) in disseminated malignant melanoma has been investigated in our oncology service in a phase-II study. Of 25 patients exposed to the drug, 23 were evaluable, and seven of these (30%) were considered objective responders. The observed response of malignant effusions is of particular interest. Side effects related to vindesine are similar to those of the other vinca alkaloids, although alopecia may be more pronounced with the doses used in this study.
