[Chronic pulmonary thromboembolism revealing beta-thalassemia intermedia after splenectomy].

INTRODUCTION: The clinical expression of beta-thalassemia intermedia is variable and complications are more frequent than in the minor form. Thromboembolism risk increase after splenectomy. Few cases of the type of complications are reported. CASE RECORD: A man was admitted for beta-thalassemia intermedia with moderate chronic hemolysis anemia, complicated by chronic pulmonary thromboembolism and liver iron overload. Post-traumatic splenectomy probably increase the risk of this two complications. The patient's respiratory status improved following bosentan therapy without worsening his hepatopathy. CONCLUSION: The present study also notes that thromboembolism complications can be an indicator of beta thalassemia and interrogate about the risk and the benefit of splenectomy in the treatment of beta-thalassemia intermedia.

Acquired inhibitor to factor VIII in a patient with Hodgkin's disease following treatment with interferon-alpha.

We describe a young woman who developed acquired haemophilia after 18 months of interferon (IFN-)-alpha therapy. This patient had been monitored since 1992 for Hodgkin's disease initially treated by chemotherapy. After two relapses, she received intensive chemotherapy followed by an autologous peripheral progenitor cell graft. IFN-alpha was then administered for 18 months. Bleeding of the limbs and tongue occurred 1 month after withdrawal of IFN-alpha and high titres (123 Bethesda units) of autoantibody to factor VIII (FVIII):C were measured. Prednisone (1 mg kg(-1) day(-1)) achieved rapid cessation of the bleeding and FVIII autoantibodies were undetectable 5 months later. This case report suggests that the activated partial thromboplastin time should be regularly checked in every patient treated with IFN-alpha in cases of unexplained bleeding, together testing for antibodies to FVIII if the bleeding is prolonged.

Intrauterine food restriction as a determinant of nephrosclerosis.

We previously showed that 3-month-old rats subjected to a 50% intrauterine food restriction had a decreased number of nephrons with increased glomerular diameter, which suggests compensatory hypertrophy. Hypertrophy could be the early event of glomerular damage. In this study, we extended our investigation and performed functional, morphological, and immunohistochemical evaluations in 3- and 18-month-old rats that underwent a 50% intrauterine food restriction (RT3 and RT18, respectively) and age-matched control rats (C3 and C18, respectively). Our findings showed that glomerular filtration rate was significant decreased in RT18 rats (2.42 +/- 0.15 mL/min/kg; n = 28; P: < 0.05) compared with C18 control rats (4.19 +/- 0.10 mL/min/kg; P: < 0.05) and the percentage of glomeruli with sclerosis was greater in RT18 rats (13.01% +/- 2.95%; n = 9; P: < 0.01) than in C18 rats (2.71% +/- 0.35%; n = 6). RT18 rats also showed more intense tubulointerstitial lesions and immunohistochemical alterations in the renal cortex. Immunohistochemical studies showed increased fibronectin and desmin expression in glomeruli and tubulointerstitium and increased vimentin and alpha-smooth muscle actin in the tubulointerstitial area from the renal cortex of RT18 rats (P: < 0.05). Desmin was also increased at the edge of glomeruli from RT18 rats, suggesting podocyte injury. Our data show that when food restriction is imposed during pregnancy, permanent damage occurs in the kidney of the offspring. Glomerular lesions were more severe than the tubulointerstitial damage in these animals.

Monoclonal antibodies against laryngeal carcinomas. Preliminary report.

Following types of monoclonal antibodies (MoAb) (HMFG 1, 2 and AI 16.9) were used in twenty patients with laryngeal carcinomas (10 pharyngolaryngeal carcinomas, T4 N+ MO, and 10 vestibulo-glottic carcinomas, T1 T2, N+ N-, MO). The demonstration of MoAb's binding was performed with immunoperoxidase technique. The results demonstrate that the HMFG 1 and 2 antibodies fixed to both T and N, mostly good or very good, while the AI 16.9 antibody fixed in a various way. None of these three MoAb's was specific for the tissue of metastasis-free satellite lymph nodes.

A pilot project on community based rehabilitation in south India--a preliminary report.

A pilot project on Community Based Rehabilitation was launched by the Hind Kusht Nivaran Sangh in South Arcot District of Tamil Nadu with the help of the Hemerijckx Rural Centre, Rawttakuppam on an experimental basis to assess the cost effectiveness and suitability of its application in other districts. Twenty cured disabled leprosy patients with grades 1 and 2 deformities from 17 villages were given training in trades like cycle repairing, tailoring, pesticides spraying, doll making, cane work, cigar making, fish net knitting and incense stick making. The duration of the training varied from two to six months depending upon the trade. Local artisans and craftmen from among the community members were identified, motivated, and utilised as trainers. The travel and maintenance costs paid to the trainees was an incentive to learn the trade and the honorarium paid to the trainers motivated them to spare their time to impart the skill within the specified period. On completion of training, start-up funds needed for purchase of tools and accessories required for pursuing the vocation were arranged through banks under DRI scheme, IRDP schemes from BDO office and from other voluntary agencies. The total expenditure incurred for training 20 cured disabled leprosy patients worked out to only Rs. 25,350/-i.e., approximately Rs.1,250/- per patient. Out of the 20 patients trained, 17 have already started earning through the skills imparted to them.
