Four cases of cholelithiasis in horses were reviewed. Two horses were examined because of recurrent abdominal pain. Clinical laboratory data, results of physical examination, and history indicated biliary obstruction. In 1 case, B-mode ultrasonography aided in the diagnosis, which was confirmed at necropsy. In 2 horses, the biliary calculi were incidental necropsy findings.
Twenty-three horses with clinical signs of gastrointestinal disease were evaluated once or serially during the course of the disease for the presence or absence of circulating endotoxin-like substances (ET+ or ET-) and whether the horses were negative, positive, or very positive for serum fibrin/fibrinogen degradation products (FDP-, FDP+, or FDP++). In addition, platelet count, activated partial thromboplastin time (APTT), and 1-stage prothrombin time (OSPT) were measured. When examined initially, 5 horses were ET-/FDP-, 8 were ET-/FDP+, 7 were ET+/FDP-, 3 were ET+/FDP+, and none was ET+/FDP++. Eight horses were evaluated serially and the following changes were noted: 1 ET-/FDP- horse became ET+/FDP++; 1 ET-/FDP- horse became ET-/FDP+; 1 ET-/FDP- horse became ET+/FDP-; 2 ET-/FDP+ horses became ET+/FDP++; 1 ET+/FDP- horse became ET+/FDP+ (++); and 2 ET+/FDP- horses became ET-/FDP-. At the last examination, 4 horses were ET-/FDP- and all survived; 7 were ET-/FDP+ and only 1 was euthanatized; 5 were ET+/FDP- and only 2 were euthanatized; and 7 were ET+/FDP+ or ++, and 5 horses died or were euthanatized. The relationship between ET and FDP is discussed and a working model of endotoxemia and gastrointestinal disease in the horse is presented.

Volunteers were treated with fibroblast interferon or placebo nose drops for four days. On the second day, they received a challenge with rhinovirus type 4. There was no difference in the clinical symptoms or viral shedding between the two groups. The possible reasons for this result are discussed.

The frequency of American visceral leishmaniasis affecting humans on Margarita Island, Venezuela, has increased in recent years, and infected dogs appear to constitute the principal source of infection. ELISA tests with Leishmania donovani promastigotes and rK39 antigen from L. chagasi in serum from 541 dogs were positive in 33.1% and 21.6% of the samples, respectively. A second blood sample taken from 50 animals after 8-10 months revealed an increase from 24% to 40% of ELISA positivity to both antigens, suggesting high susceptibility and transmission in the canine population. Among 42 serologically positive dogs, 33% of which showed clinical signs of disease, 79% were positive in polymerase chain reactions using primers specific for the L. donovani complex. Control measures including epidemiological hypersurveillance, the humane sacrifice of infected dogs, and rapid diagnosis and treatment of human cases have been initiated.
OBJECTIVE: To describe timing and severity of abuse before and during pregnancy for African American, Hispanic, and white Anglo American women. FINDINGS: Among 199 abused women, 18.1% of the women were abused during pregnancy but not the year before, 30.2% were abused the year before but not during pregnancy, and 51.8% were abused both the year before and during pregnancy. The timing of abuse did not vary by ethnicity. The three (ethnicity) by three (timing) factorial analysis of variance showed severity of abuse to vary by timing of abuse. Women reporting abuse both before and during pregnancy reported greater severity of abuse on each of the five measures than did women abused only before pregnancy or only during pregnancy. CONCLUSIONS: Over half (51.8%) of the women reported abuse before and during pregnancy with these women reporting greater severity of abuse on all five severity scores. Timing and severity of abuse did not vary by ethnic group. The majority of women abused during pregnancy were also abused prior to pregnancy, indicating the need for universal screening of all women during each health encounter.

The authors analysed the visceral leishmaniasis (VL) aspects in the state of Maranhão, Brazil, from 1982 to 1993. The disease happens to occur predominantly in São Luís Island (MA) and during the epidemic period, town of São Luís was pointed out as the main endemic area. The greatest frequency of cases occurred in 1993, despite the use of insecticide and dogs control. There was predominance of age between 0- to 4-year-old population with 58.04% of cases. Neither the human disease nor the rainfall index had significant seasonal variation. However they were correlated moderately, with high number of cases after the period of great precipitation of rain. After this study, the data obtained will allow a better control of the disease, despite some factors such as: the urbanization, localization and dynamic of transmission in endemic areas in the Maranhão state.
