Multiple methylation-free islands flank a small breakpoint cluster region on 11p13 in the t(11;14)(p13;q11) translocation.

The t(11;14)(p13;q11) translocation is one of the most frequent chromosomal abnormalities in T-cell acute lymphoblastic leukemia (ALL). Ten different leukemias carrying this translocation have been analysed and all 10 breakpoints fall within a region of less than 25 kb on chromosome band 11p13. We have used PFGE and cosmid cloning to assess the presence of potential genes by analysing methylation-free islands in the vicinity. Four methylation-free islands, within 270 kb, flank the t(11;14)-associated breakpoint cluster region (T-ALLbcr), one occurring about 25 kb on the telomeric side and one about 100 kb on the centromeric side of the T-ALLbcr. Evidence for eight further methylation-free islands on both sides of the T-ALLbcr region is also presented. Thus multiple methylation-free islands exist on 11p13 flanking the t(11;14)(p13;q11) translocation-associated breakpoint cluster region, representing multiple potential transcription units whose chromosomal environment is altered by chromosome translocation.

Molecular characterization of the t(10;14) translocation breakpoints in T-cell acute lymphoblastic leukemia: further evidence for illegitimate physiological recombination.

The t(10;14)(q24;q11) translocation is a non-random chromosome change seen in the leukemic cells of 5-10% of patients with T-cell acute lymphoblastic leukemia (T-ALL). Recent studies support the hypothesis that the translocation occurs in the course of aberrant physiological recombination and results in the juxtaposition of a T-cell receptor (TCR) gene in 14q11 with a putative oncogene, TCL3, in 10q24. We cloned and sequenced the translocation breakpoints on both derivative 10q+ and 14q- chromosomes from a patient with t(10;14)(q24;q11) T-ALL. Two distinct diversity segments of TCRD, D delta 2 and D delta 3, were identified at the two translocation breakpoints on chromosome 14. The 9.5 kb DNA that separates these two subunits in the germline was deleted, possibly in the course of a D-D joining event. The two chromosome 10 breakpoints were 10 nucleotides apart and occurred in the immediate vicinity of a pseudo-heptamer signal motif. N-region addition is also evident at the breakpoint on the derivative chromosome 10. Our observations strongly suggest that the IG/TCR recombinase normally involved in V-(D)-J joining was involved in the process of the t(10;14)(q24;q11) translocation.

[Dosimetry comparison of pelvimetry methods using conventional radiologic technic and x-ray scanner].

The aim of this study is to compare conventional and CT-scan X-ray pelvimetry. Doses have been measured on the surface and in deep tissues as well as total doses received by the patients. With CT the fetus and the mother were exposed to 1/10th of the total dose delivered using conventional X-rays. The method is acceptable and the high quality of the films achieved make CT-scanning pelvimetry safer than X-ray pelvimetry.

[Brief intracavitary radiation (137 cesium) of the prostatic site in localized cancer of the prostate: preliminary results after 9 years].

Incidental detection of an adenocarcinoma's microscopic focus inside a piece of adenoma or TURP-chips is not rare. Generally, these hidden or "latent" carcinomas without any clinical event, are staged T0 (UICC) or stage A (Whitmore). In reviews, we can find justification of two contrasting attitudes: radical prostatectomy or therapeutic abstention. For the last nine years, we have proposed a third method: endocavitary curietherapy by 137 Cs of prostatic lodge, whose first results, in terms of morbidity and mortality, are discussed.

[Report on the conservative treatment of melanoma of the uvea at the Lausanne University Ophthalmologic Clinic].

A great number of techniques are currently available for the conservative treatment of uveal melanomas: ocular applicators emitting gamma rays 60Co, 125I) or high-energy beta rays (106Ru/106Rh), light photocoagulation, surgical excision, and accelerated proton beam irradiation. Life expectancy following conservative treatment is equal to or better than that following enucleation. This is demonstrated by nonrandomized comparative studies, and by the authors' own long-term results following the conservative treatment of melanomas by 60Co applicators: mortality due to metastases of small melanomas was 3% (V = smaller than 10 x 10 x 3 mm), with medium-size melanomas it was 12% (V = 10 x 10 x 3-15 x 15 x 5 mm), and with large melanomas 21% (V = larger than 15 x 15 x 5 mm). Accelerated proton beam irradiation of uveal melanomas is currently the method of choice for the conservative treatment of uveal melanomas. The sharp boundaries of the irradiated zone, the uniformly distributed irradiation dose, and beam-splitting are the main advantages of this technique. During the last three years, 310 cases of uveal melanoma have been treated in Switzerland with an accelerated proton beam. Of these, 214 were followed up for more than one year. Eight patients (3.9%) died of metastases. Visual acuity was identical or superior to initial visual acuity in 60.3% of the cases, while 39.6% exhibited a deterioration of vision or a functional loss. Favorable results achieved by conservative treatment of uveal melanomas considerably limited the indications for enucleation, which is now only performed in exceptional situations.

Acute myeloid leukemia with T-lymphoid features: a distinct biologic and clinical entity.

We studied the clinical and biologic features of 10 cases of acute leukemia that met standard French-American-British (FAB) criteria for acute myeloid leukemia (AML) but in which the blast cells also expressed the T-cell-associated CD2 surface antigen. All cases had greater than 3% myeloperoxidase and Sudan black B-positive leukemic blasts, and blasts from seven cases contained Auer rods. Reactivity of the cells with a panel of monoclonal antibodies (MAbs) indicated that leukemic cells in all cases expressed myeloid-associated (CD11b, CD13) surface antigens, further supporting the diagnosis of AML. However, blasts from every patient coexpressed the T-cell-associated surface CD2 and CD7 as well as cytoplasmic CD3 antigens. Blasts from five patients expressed surface CD25, whereas blasts from only one expressed surface CD3. Five patients had rearranged T-cell receptor beta-chain genes, whereas only three had rearranged T-cell receptor gamma-chain genes. This pattern of lineage-related gene expression appears to define a distinct subtype of AML with T-lymphoid features (CD2+ AML) and could reflect either aberrant gene expression in leukemic blasts or transformation of a pluripotent stem cell having a flexible pattern of gene expression. Clinically, these 10 patients presented at an older age with a higher leukocyte count and a higher frequency of lymphadenopathy than did children whose blast cells were characteristic of myeloid leukemia. Patients with CD2+ AML also had poorer responses to remission induction therapy (50% v 80% entered complete remission, P = .05). However, each of the five children who failed induction chemotherapy on AML protocols had a striking response to drug combinations usually reserved for lymphoid leukemia. We conclude that this leukemia with mixed lymphoid and myeloid characteristics is a distinct biologic and clinical entity.

[Large breasts, small breasts; which mammographic technic?].

The variation of dose and image quality parameters in the voltage range 25...33 kV have been studied on 3 mammographic phantoms simulating thin, average and thick breasts respectively. The global image quality has been objectively evaluated by an image quality index. The average glandular dose has been chosen as relevant indicator of risk. The study shows that the surface dose and the midplane dose are not suitable indicators of risk if the breast thickness is variable. For all phantoms, the deterioration of image quality with voltage increase was found to be relatively less important than the decrease of the average glandular dose. Consequently, the following optimum voltages are proposed: 28-29 kV for thin breasts, 29-30 kV for average breasts and 33-35 kV for thick ones. The study showed that breast thickness variation is not suitably compensated by the automatic exposure devices currently available.

Objective assessment of mammography systems. Part I: Method.

The authors have developed an experimental method for simultaneous determination of dose and image parameters in mammography. A global and objective quality concept, the image quality index, is proposed and its reliability demonstrated by tests of reproducibility. Objective quality tests and subjective evaluation by radiologists showed good correlation.

Objective assessment of mammography systems. Part II: Implementation.

A quality control program for mammography units was carried out, based on objective tests of image quality and dose. Results are reported for 31 units, including correlation between various parameters. Satisfactory results were obtained in the case of 21 installations; for the other 10, comparison between measured quality parameters and those of the reference system suggested ways in which quality might be improved.

[Radiotherapy of intraocular tumours (author's transl)].

Ocular radiotherapy has considerably improved during the last 60 years. It allows very often, not only to save an eye, but also to maintain a useful function. But it is only one single element, although important, in our fight against intraocular tumors.

[Diagnosis of intraocular tumors: phosphorus 32 test].

The 32P test occupies an interesting place among the methods used in the diagnosis of intraocular tumours. It must, however, be reserved for certain situations only, and carried out under clearly defined conditions. The authors outline the indications and modalities of this test, and report the results obtained at the clinic of ophthalmology of Lausanne where it has been in use over the last 10 years.
