Management of the nonvertex second twin.

We reviewed twin deliveries during a five year period at North Carolina Memorial Hospital and Wake County Medical Center in which Twin B presented as nonvertex. We compared three delivery routes: primary abdominal delivery, breech extraction of the second twin and external version of the second twin. There was no difference in neonatal outcome between the delivery routes. Mothers in the external version group were more likely than those in the breech extraction group to undergo abdominal delivery of the second twin. These women also required more emergency anesthesia, had a higher blood loss and stayed in the hospital longer after delivery.

Ovarian suspension in massive ovarian edema.

Massive ovarian edema is an uncommon condition found in young women that is speculated to occur as a result of incomplete ovarian torsion. We present the second patient to our knowledge to undergo ovarian suspension as a treatment for this condition. This approach succeeded after ovarian wedge resection had failed. Our patient remained symptom-free at 1 year of follow-up. Ovarian suspension should be considered when this pathologic entity is diagnosed.

Colchicine toxicity: a case report.

A 59-year-old male was admitted to the Intensive Care Unit from the Emergency Department approximately 12 hr after the ingestion of 30 to 40 mg of colchicine, TYLENOL with codeine, IMODIUM, DESYREL and ethanol. He suffered severe systemic manifestations and succumbed approximately 36 hr post-ingestion. Although this is a case of a mixed ingestion, the complications seen suggest that colchicine was responsible for the fatality.
