Myeloid antigen expression on common acute lymphatic leukaemia blasts after culture.

Blast cells from seven out of ten patients with common acute lymphoblastic leukaemia (cALL) developed the myeloid antigen MY7 (CD13) after culture, and one of these coexpressed the myeloid antigen MY9 (CD33). CD13 expression appeared to be independent of maturation since it could be induced more readily in cultures which did not contain the differentiation promoter 12-O-tetradecanoyl-phorbol 13 acetate (TPA). CD13 expression in culture was not seen on one null ALL, or 6 B-CLL investigated or on normal tonsillar B cells or PBMC under similar conditions. CD13 expression on cALL blasts probably represents evidence of abnormal gene expression in the leukaemic cells. However the absence of CD13 expression on the earlier B null ALL or the later B-CLL suggests we cannot exclude the possibility that CD13 expression is a feature of normal precursor B cells.

A rapid dot-blot assay to assess chimerism following sex-mismatched bone marrow transplantation.

Mixed chimerism may occur more frequently than previously thought following allogeneic bone marrow transplantation and may have implications in terms of relapse, graft-versus-host disease and immune reconstitution. DNA analysis using single or multilocus polymorphic probes cannot reliably discriminate between donor and recipient cells below a level of 10%. We used probe pHY2.1, a cloned segment of tandemly repeated DNA (2000 copies) on the long arm of chromosome Y. A dot blot procedure allowed us to immobilize DNA directly from 50 microliter of peripheral blood or bone marrow. Cross-reactivity was eliminated by hybridization at conditions of extreme stringency (65 degrees C, 50% formamide). Mixing experiments detected male DNA at a level of 0.1% after 10 h exposure. Five patients were studied serially post-bone marrow transplantation. One patient showed mixed chimerism for 12 months, one had complete autologous recovery and the remaining three showed complete engraftment. All results were verified by standard karyotyping on bone marrow cells. This technique is a simple, rapid and sensitive assay for chimerism following sex mismatched bone marrow transplantation.

Monocytic differentiation in acute myeloid leukaemia: a cause of Fc binding of monoclonal antibodies.

Cells from 17 patients with acute myeloid leukaemia (AML) were investigated using the following IgG2a monoclonal antibodies cALL, B1 and Leu 1. Cells from six patients showed binding of cALL, five showed B1 and three showed Leu 1 positivity. Binding of the monoclonal antibodies was shown to be due to Fc binding as it was blocked by prior incubation with aggregated human IgG. Measures such as prior incubation with AB serum or incubation at 37 degrees C were insufficient to block Fc binding in all cases. Fc binding correlated with evidence of monocytic differentiation. Our findings suggest that unless Fc binding is outruled false positive results may be obtained in the investigation of lineage infidelity in AML particularly in monocytic leukaemias.

The use of intravenous immunoglobulin (endobulin) in patients with immune thrombocytopenia.

Six adults were treated for chronic immune thrombocytopenia (ITP) with high dose intravenous immunoglobulin (ENDOBULIN). Five patients were evaluable. In four patients the platelet associated IgG (PAIgG) levels were increased. All four responded to i.v. Ig and two subsequently underwent splenectomy. One patient had normal PAIgG and had no response to i.v. Ig. Endobulin is useful in the treatment of chronic ITP and is followed in some patients by a prolonged response. High levels of PAIgG may predict a favourable outcome.

Chronic granulomatous disease presenting as an oculomucocutaneous syndrome mimicking Behçet's syndrome.

A female patient who presented for the first time at the age of 19 with oculomucocutaneous syndrome was found to have an absolute deficiency of neutrophil peroxide production. Neutrophil peroxide production as measured by chemiluminescence was zero on stimulation with opsonized zymosan. Direct membrane stimulation with FMLP and calcium ionophore also failed to elicit peroxide production. The diagnosis of chronic granulomatous disease should be considered in young patients with oculomucocutaneous syndrome.

Sequential studies of bone marrow haemopoietic progenitors (CFU-GEM, BFU-E, CFU-E, CFU-GM) following busulfan treatment in Balb/c mice.

The effect of multiple sublethal doses of busulfan on the haemopoietic system of the Balb/c mouse was examined. FBC's bone marrow histology and bone marrow progenitor cell assays (CFU-E, BFU-E, CFU-GM and CFU-GEM) were performed. No effect was seen on FBC's or on marrow histology. However there was a significant depletion noted in the number of haemopoietic progenitor cells as measured by in vitro culture.

Human marrow stromal cells in short-term semi-solid bone marrow culture in aplastic anaemia.

A short-term methylcellulose technique was used to study the proliferation of marrow-derived stromal, erythroid and myeloid colonies from normal controls and from patients with aplastic anaemia. There was a significant reduction in all colony types in patients with aplastic anaemia when compared with normal controls. In 4 patients who achieved remission following treatment with ATG + oxymetholone or with oxymetholone alone there was a return to normal range of stromal colonies and also of CFU-E. There was no change in the numbers of BFU-E's and only a slight increase in the numbers of CFU-C's.

Treatment of severe aplastic anaemia with anti-thymocyte globulin.

Ten patients (9 adults, 1 child) with severe aplastic anaemia (SAA) were treated with anti-thymocyte globulin (ATG). All patients developed serum sickness. No long-term adverse sequelae attributable to ATG were noted. Fifty percent of patients are alive at 1 year post-ATG. One patient received an allogeneic bone marrow transplant because of failure to respond to ATG. One long-term survivor is severely thrombocytopenic and requires support, and one patient has mild thrombocytopenia requiring oxymetholone therapy.

Cholesterol ester storage disease in an adult presenting with sea-blue histiocytosis.

An adult patient is described with hepatomegaly and sea-blue histiocytes in the bone marrow. A diagnosis of cholesterol ester storage disease was established following enzyme and lipid analyses on liver biopsy and cultured skin fibroblasts. Acid esterase activity was deficient (approx. 5% of controls) in liver and fibroblasts using [14C]-triolein or 4-methylumbelliferyl palmitate as substrates. Cholesterol ester levels were raised about 70-fold in liver, whereas triglyceride levels were only marginally raised. Marked accumulation of cholesterol esters was also demonstrated in cultured fibroblasts. Clinically, the patient responded favourably to phenobarbitone treatment. However, this was not reflected in liver acid esterase or lipid levels.

Provision of leucocyte poor blood at the bedside.

The Imugard IG 500 cotton wool filter and the Cellselect cellulose acetate filter were adapted for filtration of leucocytes from packed cell transfusions at the bedside. Sixty five transfusions were given via the Imugard IG 500 filter and 54 transfusions were given via the Cellselect filter. Packed red cell concentrates from the National Blood Transfusion Service provided for routine blood transfusions were used in all cases. No patient in either group of multitransfused patients experienced a febrile blood transfusion reaction during the study. The Imugard IG 500 removed 91% +/- 9 (SEM) leucocytes; the Cellselect removed 96% +/- 7 (SEM) leucocytes. In the Imugard IG 500 group one patient received greater than 0.5 X 10(9) leucocytes. In the Imugard IG 500 group one patient received greater than 0.5 X 10(9) leucocytes, but no patient in the Cellselect group received greater than 0.5 X 10(9) leucocytes in any single transfusion. This is a safe method of providing leucocyte poor blood at the bedside.
