Scintigraphy of gastrointestinal hemorrhage: superiority of 99mTc red blood cells over 99mTc sulfur colloid.

The relative efficacy of 99mTc sulfur colloid and in vitro-labeled 99mTc red blood cells in detecting and localizing gastrointestinal hemorrhage was evaluated in a prospective tandem study of 100 patients referred for suspicion of gastrointestinal tract hemorrhage. Thirty-eight true-positive scintigrams were obtained with 99mTc red blood cells, whereas 99mTc sulfur colloid detected only five sites of hemorrhage. Scintigraphic findings were corroborated by clinical, endoscopic, arteriographic, and surgical findings. 99mTc red blood cells were clearly superior, with a sensitivity of 93%, specificity of 95%, and overall accuracy of 94% in detecting and localizing gastrointestinal hemorrhage.

Biliary scintigraphy. The "hot rim" sign.

A new cholescintigraphic finding, the "hot rim" sign, is reported in a case of acute cholecystitis. Local inflammation in the gallbladder fossa may be the cause of this phenomenon.

Another use for the seven-pinhole collimator. Routine nontomographic imaging of the thyroid.

Relatively few seven-pinhole collimator systems purchased within the last three years are still in routine use. This paper describes a nontomographic use of the collimator for thyroid scanning. One image is obtained which simultaneously shows seven different projections of the thyroid gland. The diagnosis of thyroid abnormalities can frequently be made from this single composite image without the requirement for additional images. Further information can be obtained when needed by computer processing the image into tomographic slices.

Fourier phase analysis in non-cyclic dynamic studies.

Single harmonic Fourier amplitude and phase (FA/P) analyses of 129 radionuclide angiographic (RNA) studies were found to reflect accurately the sequence and quantity of perfusion determined in the independently interpreted RNA studies. In addition, the anatomic detailing in peripheral studies (hands, feet, etc.) was considered superior to that obtained in RNA studies.

Femoral intraarterial digital angiography: an outpatient procedure.

Selective intraarterial digital angiography was performed in 50 patients with known or suspected peripheral vascular disease. Excellent anatomic detail was provided by low-volume injection of 80 mgl/ml contrast medium. The procedure was performed with a 20- to 21-gauge needle or short 3 French catheter as an outpatient procedure. No complications were observed. Thirty-five patients underwent subsequent surgical repair of abnormalities demonstrated by this direct intraarterial technique.

Scintigraphic detection of occult hemorrhage using RBCs labeled in vitro with technetium Tc 99m sodium pertechnetate.

Scintigraphy with RBCs labeled with technetium Tc 99m sodium pertechnetate effectively located the source of hemorrhage in a patient receiving long-term anticoagulant therapy. (The patient was initially seen with a large hematoma on the flank.) More important, the procedure was used to monitor activity in this otherwise-occult bleeding site. Scintigraphic studies may be useful in the management of these difficult clinical problems.

Computer edge displays for cardiac wall motion evaluation.

Several computer display techniques in both black and white and color are described. Use of such techniques improves the qualitative evaluation of ECG-gated cardiac blood pool studies, especially by less experienced observers. Detection of wall motion at the interface between the myocardial wall and the labeled blood within the cardiac chambers is greatly enhanced. No additional equipment nor computer processing time is required to use these display schemes.
