Researches on antibacterial and antifungal agents, X. Synthesis and antifungal activities of 1-(p-methyl-alpha-[4-(1H-pyrrol-1-yl)phenyl]benzyl)azoles and some related products.

The synthesis and antifungal activities against Candida albicans and Candida spp. of some pyrrole analogues of bifonazole are reported. 1-(p-Methyl-alpha-[4-(1H-pyrrol-1-yl)phenyl]benzyl)imidazole was found to be equipotent or sometimes superior to bifonazole and ketoconazole, and lightly inferior to miconazole. Substitution of the imidazole moiety with other azoles retained some activities. No activity was shown when the azole aromatic rings were replaced by the heteroalicyclic ones.

Research on antibacterial and antifungal agents. XI. New antibacterial quinolones related to pirfloxacin.

The synthesis and antimicrobial activities against Gram-positive and Gram-negative bacteria of some 1-substituted quinolones related to pirfloxacin are reported. Compounds with a benzyl moiety at 1-position were generally found to be less active than the analogues bearing an unsaturated alkyl chain at the same position. Introduction of fluorine or chlorine in the benzyl moiety did not increase the antibacterial activity. All derivatives here reported were inferior to pirfloxacin as regards antibacterial power, but always superior in comparison with nalidixic acid and pipemidic acid against Gram-positive microorganisms.

Calcium-rich foods and blood pressure: findings from the Italian National Research Council Study (the Nine Communities Study).

The present report analyzes the relation between different categories of consumption of varied calcium-rich foods and blood pressure in a population sample of 5,049 Italian men and women aged 20-59, examined between November 1978 and May 1979. The consumption pattern of the various foods was estimated with the use of a food frequency questionnaire. The findings are consistent with an inverse association between whole milk consumption and systolic, but not diastolic, blood pressure. Daily consumption of cheese, on the other hand, is not associated with reduced blood pressure levels. The possibility of an adverse effect of the high saturated fats and sodium content on blood pressure that could counteract the beneficial effect of calcium is discussed. Daily skim milk consumption is surprisingly associated with higher systolic, but not diastolic, pressure, compared with infrequent consumption. The possibility of these findings being the result of pre-existing health problems leading to the selection of skim milk over whole milk is discussed. Daily consumption of legumes is associated with lower systolic pressure than infrequent consumption, but the differences do not reach statistical significance, probably because of limited statistical power. These associations are similar in both males and females.

[A substance with antibacterial and antifungal activity. IV. Synthesis and microbiological activity of new 1,5-diarylpyrrole derivatives].

The synthesis and antifungal activities of new 1,5-diarylpyrrole derivatives are reported. Antimicrobial data in comparison with pyrrolnitrin show that only carboxamide derivatives exhibit satisfactory antifungal activity. By contrast all tested compounds show very poor antibacterial activity. The displacement of NO2 group from para to meta or ortho position of the aryl at C5 of the pyrrole ring affects the antimicrobial activity.

Researches on antibacterial and antifungal agents. IX--Pyrrole analogues of bifonazole with potent antifungal activities.

The synthesis and antifungal activities of pyrrole analogues of bifonazole are reported. Reduction of 4-nitrobenzophenone to the corresponding alcohol, reaction with phosphorus tribromide of the latter compound and condensation of the bromonitroderivative with imidazole led to 1-[alpha-(4-nitrophenyl)-4'-benzyl]-1H-imidazole. Hydrogenation of the nitro group to amino and reaction with 2,5-dimethoxytetrahydrofuran according to the Clauson-Kaas procedure afforded the pyrrole analogue of bifonazole. This compound and the related chloroderivative were also prepared by a similar pathway starting from 4-(1H-pyrrol-1-yl)benzophenone and its 4'-chloroderivative. Microbiological screening against Candida albicans and Candida spp showed 1-(alpha-[4-(1H-pyrrol-1-yl)phenyl]benzyl)-1H-imidazole to be the most active compound among the tested derivatives.

Physical activity and its relationship to blood pressure in school children.

The present report analyzes the relationship between physical activity and blood pressure in 1341 school children aged 7-14 years (girls = 598, boys = 743) participating in a screening for cardiovascular risk factors that took place in the suburban area of Naples. During the baseline examination, systolic (SBP) and diastolic (DBP) blood pressure, height, weight, and resting pulse rate (PR) were measured. The Harvard-Modified Step Test was also performed in order to score both a crude recovery index (RI) and an index corrected for the initial resting pulse rate (RI-PR) used as measures of the level of habitual physical activity. Significant differences have been found between the quartiles of distribution of both the crude recovery index and the corrected index in girls and boys with regard to systolic blood pressure but not diastolic. In both sexes, better responders to the Harvard-Modified Step Test showed on the average lower systolic blood pressure than poorer responders. In boys, the inverse association between systolic blood pressure and the recovery index was independent of the effect of age, body mass index, height, and resting pulse rate, while in girls the same association did not reach statistical significance. Our results support the hypothesis that systolic blood pressure is independently related to the level of habitual physical activity in children.

Alcohol consumption, drinking pattern and blood pressure: analysis of data from the Italian National Research Council Study.

The present analyses addressed the relationship between alcohol consumption, drinking patterns and hypertension in 6699 Italian men and women participating in a national project on arteriosclerosis risk factors. The results of the analyses are consistent with an association between heavy alcohol consumption and systolic blood pressure in both males and females. Higher diastolic blood pressure was associated with heavy alcohol consumption in men but not in women. Drinkers of wine both with and without meals have a higher prevalence of hypertension than abstainers in both sexes. We conclude that heavy alcohol consumption is associated with higher blood pressure values, and that the pattern of drinking could be an important determinant in the association between alcohol and hypertension.

[Research on antibacterial and antifungal agents. VII. Synthesis of (1-pyrryl)methylquinolines acids].

Some (1-pyrryl)methyl derivatives of 1-ethyl-1,4-dihydro-4-oxoquinoline-3-carboxylic acid were synthetized by the standard procedure involving Gould-Jacobs and Lappin reactions. The above derivatives were tested microbiologically as nalidixic acid analogs and compared with some clinically useful 4-oxopyridine-3-carboxylic acids (nalidixic acid, pipemidic acid, norfloxacin, enoxacin and ciprofloxacin). Their antibacterial activities were very weak.

[Antibacterial and antifungal compounds. VIII. Synthesis and antifungal activity of pyrrol derivatives similar to trichostatin A].

Some p-methylbenzolpyrrole acrylic acids and related compounds were synthesized. The new pyrrole derivatives have structural features in common with trichostatin A, an antifungal antibiotic. The above acids and derivatives were tested against Candida albicans and Candida sp in comparison with miconazole, pyrrolnitrin and amphotericin B and showed very weak antifungal activities. Occasionally some activity was found against a few strains of Candida albicans and against Candida pseudotropicalis.

Body fat distribution and cardiovascular risk in middle-aged people in southern Italy.

Skinfold thickness, % body fat content, body mass index, systolic and diastolic blood pressure, plasma glucose, serum cholesterol and triglycerides have been measured in 132 middle-aged men (mean age 39.8 +/- 9.9, range 20-59 yr) and in 114 middle-aged women (mean age 39.1 +/- 9.3, range 20-59 yr). Anthropometric data were related to blood pressure and biochemical parameters by using a straight-line regression analysis. Body mass index, % body fat content, subscapular and suprailiac skinfold thickness, consideral as indicative of splanchnic fat distribution, were positively correlated to blood pressure, plasma glucose, cholesterol and triglycerides. Triceps skinfold thickness, an index of peripheral fat distribution, showed weaker or no correlation with these parameters. These preliminary observations in the Neapolitan area support the hypothesis that, in addition to body mass index and % body fat content, preferential splanchnic fat distribution is associated with cardiovascular risk factors.

[Antibacterial and antifungal agents. VI. Pirfloxacin and related compounds: synthetic and microbiological studies].

A new route to pirfloxacin, a fluorinated pyrrylquinolone with high broad-spectrum antibacterial activities, is described starting from 7-amino-1-ethyl-6-fluoro-1,4-dihydro-4-oxoquinoline-3-carboxylic acid and 2,5-dimethoxytetrahydrofuran. When the reaction with the latter compound was carried out using the ethyl ester of the above acid the related pyrryl ester formed, which on alkaline hydrolysis gave pirfloxacin. The synthesis and antibacterial activities of the 1-allyl analogue of pirfloxacin and of 7-[2-(1-pyrrolidinomethyl)-1-pyrryl]-1-ethyl-1, 4-dihydro-4-oxoquinoline-3-carboxylic acid and its 6-fluoroderivative are also reported.

[Substances with antibacterial and antifungal activity. I. Synthesis and microbiologic activity of imidazolylmethylanilines].

The synthesis and antifungal activities of many derivatives of 1-(2-imidazolylmethyl)aniline and of 1-(4-imidazolylmethyl)aniline are reported. Antimicrobial data in comparison with miconazole show that many compounds containing chlorine atoms and nitro group exhibit an interesting antimycotic activity. The results obtained are discussed on the basis of structure-activity relationships.

Chemotherapeutic agents with an imidazole moiety. I. Synthesis and antifungal activities of 1-aryl-4-p-nitrophenylimidazoles.

The synthesis and antifungal activities of 1-aryl-4-p-nitrophenylimidazoles and their 2-mercaptoderivatives is reported. Antimicrobial data in comparison with antifungal antibiotic pyrrolnitrin attributed the best activity to 1-p-methoxyphenyl-4-p-nitrophenylimidazole. The tested compounds were prepared by reacting p-nitrophenacylanilines with potassium thiocyanate in acidic medium to afford 1-aryl-2-mercapto-4-p-nitrophenylimidazoles, which were then transformed into the title compound by treatment with nitric acid.

1-Ethyl-6-fluoro-1,4-dihydro-4-oxo-7-(1H-pyrrol-1-yl)-quinoline-3-carb oxylic acid, a new fluorinated compounds of oxacin family with high broad-spectrum antibacterial activities.

The synthesis of 1-ethyl-6-fluoro-1,4-dihydro-4-oxo-7-(1H-pyrrol-1-yl)quinoline-3-carboxy lic acid, a new fluorinated high broad-spectrum antibacterial agent related to nalidixic acid is described. The title compound has been prepared by the reaction of 4-fluoro-3-(1H-pyrrol-1-yl)aniline with diethyl ethoxymethylenemalonate, cyclization of the malonate obtained to the quinolinecarboxylate ester, ethylation of the ester, followed by hydrolysis with aqueous sodium hydroxide. The new derivative proved very active against both gram-positive and gram-negative bacteria. Its activities in comparison with those of nalidixic acid, pipemidic acid, piromidic acid and enoxacin were found to be greatly superior with regard to the unfluorinated compounds and somewhat superior also to enoxacin. The known 1-ethyl-6-fluoro-1,4-dihydro-4-oxo-7-(pyrrolidin-1-yl)quinoline-3- carboxylic acid, here prepared by catalytic hydrogenation of the pyrrole moiety of the title compound, has been found to be less active as an antibacterial agent.

[Tracking of blood cholesterol and arterial pressure in adolescence].

Early identification of atherosclerosis precursors and assessment of secular trends are key factors in organizing primary prevention programs. Tracking phenomenon for serum cholesterol and blood pressure has been evaluated at two years' interval in a group of students of both sexes, aged 11-15 years, in the municipality of Mugnano, near Naples. Serum cholesterol has been followed in 77 students and blood pressure--systolic and diastolic--in 160. Forty-three % serum cholesterol levels, 44% of systolic blood pressure measurements, and 25% of diastolic blood pressure values, that were in the upper quintile at the first visit, were found in the upper quintile again at the second visit. Our results point out that: there is a chance to identify "high" values, that may persist over time, especially for serum cholesterol and systolic blood pressure; great caution must be exercised in taking isolated measurements of serum cholesterol, systolic and particularly diastolic blood pressure at a young age as indicators in implementing prevention programs.
