Case report: Spontaneous isolated mesenteric fibromatosis presenting as megaduodenum.

A case of spontaneous isolated mesenteric fibromatosis presenting as megaduodenum is reported. The lesion was small, only 2 cm in its greatest diameter. However, because of its location near the ligament of Treitz and because of its characteristic infiltrative growth pattern, the tumour involved the distal duodenum and resulted in acute angulation and obstruction early in its course. We successfully resected the mass and the patient is in fair physical condition 6 months after operation.

Investigation of senile dementia in Qingdao.

According to the popular spot check method we investigated 14,684 partial community persons. The screen instrument used was the MMSE. Positive cases were through ADL, HDS and detailed psychical examination by two doctors according to the CCMD-2 to establish a definite diagnosis. Differential diagnosis between AD and MID was carried out according to HIS result. A total of 1448 persons were over 60 years, which amounted to 9.98% of the entire population. Of a total of 672 men and 776 females, 13 suffered senile dementia, which made up 0.995% of the aged. There were seven MID (5 males, 2 females) and six AD (2 males, 4 females). Family histories showed that one AD had the same symptoms as her twin sister. Two AD and four MD drank Chinese spirit 2-4 liang per day for 20 years, 80% of patients were nursed by the son or daughter-in-law, but they believed wrongly that 'It is necessary to cure the old'. Six of 13 had a long period of drinking history, four of six for MID. This has shown that dementia partly relates to drink especially for MID. Early case can be detected through daily life activities such as buying food, playing cards etc. Activities of daily living may avoid the pseudo-positive which is caused by educational level.

[Emergency measures on drinking water sanitation for mitigation of flood and waterlogging disasters].

This article reports the emergency measures for mitigation during flood and waterlogging disasters to ensure drinking water sanitation and to prevent infectious disease outbreaks. Five preparatory and preventive measures for flood and waterlogging disasters include the construction of dual-purpose water supply installation for ordinary and disaster use, the storage of qualified technicians and materials (or their inventories), and the formulation of predetermination programme for disaster relief, ect.

Avascular necrosis of the femoral head: MR imaging with radiological and histological correlation.

OBJECTIVE: To assess the value of MR in the diagnosis of avascular necrosis (AVN) of the femoral head. METHODS: MR images in 34 consecutive patients (26 men and 8 women) with AVN (57 hips) were reviewed. All lesions were confirmed by radiographic, radionuclide, computed tomographic, and/or histologic examination. Eleven specimens were obtained after total replacement of the hip. Four hips underwent biopsy. All MR images were obtained using a 0.35 T superconductive imaging unit with SE sequence. Specimens were cut coronally into 5 mm thick section and radiographs were obtained. RESULTS: There were four types of MR patterns of AVN. Type one appeared liner or patchy low signal area in the superoanterior portion of the femoral head. In type two, a band or ring of low signal intensity was found surrounding a central area of high signal intensity on T1WI and intermediate signal intensity on T2WI. The low signal band or ring consisted of thickened trabecular bone, mesenchymal and fibrous tissue, and amorphous acidophilic cellular debris. The central zones within the ring were composed of necrotic bone and marrow that had not been reached by the repair process. In type three, the focal subchondral region showed intermediate signal intensity on T1WI and high signal on T2WI surrounded by a low signal ring. The low signal ring consisted of thickened trabecular bone and little mesenchymal tissue. The central area was composed of mesenchymal tissue rich in capillaries and cystic necrotic zones. In type four, the signal intensity of femoral head was inhomogeneous on both T1WI and T2WI. There were low signal bands in the femoral neck surrounding the necrotic zone. Only limited areas of some lesions had signal intensity isointense with fat on T1WI and T2WI. The inhomogeneous area of low signal intensity consisted of a mixture of necrotic bone and marrow, amorphous cellular debris. The first type of MR pattern corresponded to the early stage of radiograph, and the third type of MR pattern to stage 5. The second and fourth type of MR patterns correlated less with the radiographic stage. CONCLUSIONS: MR imaging plays an important role in the diagnosis of AVN of femoral head especially in the early detection of AVN. The MR patterns of AVN is not correlated with radiographic stages exactly.

[Radionuclide pulmonary perfusion imaging in evaluating pulmonary hypertension in patients with congenital heart disease].

OBJECTIVE: To evaluate the degree of pulmonary hypertension in patients with congenital heart disease (CHD) by using radionuclide pulmonary perfusion imaging. METHOD: The pulmonary perfusion in 81 patients with left to right cardiac shunts was examined by scintigram with macroaggregates of (99m)Tc-labeled human serum albumin (PPS). PPS was analyzed for (1) lung regional radionuclide count (LRC), (2) right upper and lower lung count ratio (RULR), (3) kidney count ratio (KCR). The results were compared with those of cardiac catheterization exam. RESULT: There were correlation between mean pulmonary arterial pressure (mPAP), total pulmonary resistance (TPR), and PPS (KCR, RULR with mPAP gamma = 0.25, 0.35; LRC, KCR with TPR gamma = -0.34, 0.29). Five patients repeated PPS test after early operation. There were no changes in PPS. CONCLUSION: The degree of reversibility of pulmonary hypertension in patients with CHD could be quantitatively analysed by PPS.

[The determination of neuron-specific enolase of serum in the diagnosis and supervision of retinoblastoma].

OBJECTIVE: To investigate the value of determining quantity of neuron-specific enolase (NSE) in the diagnosis and supervision in patients with retinoblastoma (Rb). METHOD: We used rapid electrophoretic and the fluorometer method in the 35 serum specimens from 35 patients with Rb during surgery, after enucleation, respectively, at 3, 6, 12, >or= 18 months and 31 serum specimens from 31 control cases to determine the quantity of NSE and enolase isoenzymes. RESULTS: There were 207 +/- 65.9 u/L of all isoenzymes and 98.4 +/- 40.3 u/L of NSE from 35 patients with Rb. In comparison with the levels of control cases (respectively, 140.7 +/- 46.0 U//L of all isoenzymes, 41 +/- 10.9 u/L of NSE), both serum levels in patients with Rb increased obviously, the difference being significant. At 6, 12, and >or= 18 months after surgery, the determinations of all isoenzymes and NSE from 35 patients with Rb decreased step by step, the difference being significant in comparison with the values before the surgery. After surgery >or= 18 months, the levels of NSE and all enolase decreased to that of NSE and all enolase from control cases. CONCLUSION: The determination of enolase level, especially NSE of serum, is valuable for the diagnosis of patients with Rb.

[Preparation and characterization of monoclonal antibodies against Clonorchis sinensis].

AIM: To develop and identify monoclonal antibody (McAb) against adult Clonorchis sinensis. METHODS: BALB/c mice were immunized with soluble antigen of adult Clonorchis sinensis, the spleen cells of immunized mice were fused with myeloma cell line SP2/0. Then the hybridoma cell lines secreting high titer of McAbs were screened. The Ig subtypes and titer of the McAbs were assayed with ELISA. The antigens recognized by the McAbs were examined with IFAT and identified by immunoblot analysis. The cross reactions between the McAbs and antigens of Schistosoma japonicum, Paragonimus westermani and Cysticercus cellulosae were detected. RESULTS: 5 hybridoma cell lines secreting high titer of McAbs against adult Clonorchis sinensis were established. All five clones of McAbs belonged to IgM subtype. The antigens recognized by the McAbs were located at the intestinal tract wall of adult Clonorchis sinensis. The McAbs had no cross reactions with antigens of Schistosoma japonicum, Paragonimus and Cysticercus cellulosae. CONCLUSION: 5 hybridoma cell lines against adult Clonorchis sinensis secreting high titer of McAbs with high specificity were established.

[The test of mean flow velocity patterns within a new push-plate ventricular assist device].

This article reports the design of Z-4 Push-Plate Ventricular Assist Device and the test of its mean flow velocity patterns. The results showed that during diastole the interaction of the jet formed by incoming fluid within the boundaries of the blood chamber induced a large clockwise vortex, its centre was the centre of the chamber circle; the largest velocity (approximately 125 cm/sec) was generated near the major orifice of the valve in inlet conduit. During systole the centre of the vortex came close to the outlet conduit; the largest velocity (approximately 120 cm/sec) was generated in inlet conduit. There were no turbulent flow and stagnation point during the pume cycle. The mean velocity patterns proved a good wall washing over the entire chamber and the internal surfaces of the pump should be resistant to thrombus deposition.

[Preparation of liposome encapsulated daunorbicine and determination of daunorubicine of aqueous humor in rabbit eyes].

PURPOSE: To investigate the method of determination the concentration of daunorubicine(DNR) and liposome encapsulated daunorubicine(LDNR) of aqueous humor in rabbit eyes. METHODS: After extracapsular lens extraction, 0.1 ml of 1 mg/ml DNA and LDNR were injected into the anterior chamber. At the 12, 24, 48 hours and 1 week after operation, the aqueous humor were aspirated to determine the concentration of DNR by high performance liquid chromatography(HPLC). RESULTS: At the 12, 24, 48 hours after operation, DNR were determined in all eyes, but at one week after operation, the DNR could only be detected in LDNR group. The concentration of DNR in LDNR group were higher than DNR group.

Studies on human fetal lens crystallins under oxidative stress and protective effects of tea polyphenols.

PURPOSE: To investigate the oxidative modification of water-soluble crystallins of human fetal lens with H2O2 and fourteen metal ions with or without EDTA. Tea-polyphenols (TP) was added to above solutions in order to testing their antioxidative abilities. METHODS: The experiments were performed at 37 degrees C with final concentration of 2.5 mg/ml protein, 0.1 mM metal ions, 0.3 mM EDTA and 1.0 mM H2O2. Then the TP was added to the solution with CuSO4 and H2O2, after 5 or 24 hours, the crystallins were analysed with SDS-PAGE and IEF. RESULTS: There were marked oxidative modifications of lens protein in H2O2 and copper without EDTA. In SDS-PAGE patterns, we found an increase in those species above of bands higher than 30 kD and some diffuse bands from 30 to 17 kD after 5 hours. In IEF patterns, there were a general increase on acidity with loss of the more basic species. When the TP was added, there was not any difference with control group. CONCLUSION: The results indicate that exposure of water-soluble protein to H2O2 and copper leads to covalent crosslinking and cleavage of polypeptides. After 24 hours, the development of the oxidative modifications of crystallins continues, comparison with the catalytic strength, copper ions stronger than the iron ions. On other hand, this work reported that the anti-oxidative action of TP is strong.

Sources of superoxide radicals involved in the pathogenesis of diethyldithiocarbamate-induced gastric antral ulcer in rats.

The production of superoxide radicals as a result of decreased Cu,Zn-superoxide dismutase activity is considered to be the most important factor in the pathogenesis of diethyldithiocarbamate (DDC)-induced gastric antral ulcers in rats. The aim of the present study was to identify possible sources of superoxide radicals and the role of platelet-activating factor (PAF) in DDC-induced ulcer formation. Groups of rats were pretreated with a drug or antiserum before DDC (800 mg/kg) administration. The size of the DDC-induced gastric antral ulcers was measured. Pretreatment with anti-rat polymorphonuclear leukocyte serum or CV-3988 (a specific antagonist of PAF) 20 mg/kg significantly reduced the size of DDC-induced gastric antral ulcers. The results confirmed that superoxide radicals play an important role in the pathogenesis of DDC-induced gastric ulcer in rats and suggested that NADPH (reduced nicotinamide-adenine dinucleotide phosphate) oxidase in PAF-activated polymorphonuclear leukocytes may be involved in the generation of these radicals.

[Parental origin of extra chromosome 21 in Down syndrome detected by using short tandem repeat DNA polymorphisms after PCR amplification].

OBJECTIVE: To detect the parental origin of extra chromosome 21 in Down syndrome, using two (GT)n polymorphic markers-D21S215 and D21S120. METHODS: The alleles of D21S215 and D21S120 were amplified by polymerase chain reaction and identified with denaturing polyacrylamide gel electrophoresis followed with Ag-staining. The parental origin of extra chromosome 21 was determined by comparing genotypes of probands with their parents. RESULTS: The parental origin of extra chromosome 21 was determined in 17 Down syndrome out of 24, with 12 and 5 inherited from mother and father respectively. CONCLUSION: The parental origin of extra chromosome 21 in Down syndrome can be determined by analyzing the polymorphisms of D21S215 and D21S120. Etiological study of trisomy 21 should be focused on maternal meiosis.

Molecular cloning and functional characterization of murine cDNA encoding transcription factor NFATc.

Transcription factors of the NFAT (nuclear factor of activated T cells) family play important roles in immune and inflammatory responses by regulating the expression of genes encoding cytokines and immunoregulatory proteins. Here we describe cloning and characterization of full-length cDNA encoding murine (m) NFATc which predicts that the protein has all the conserved structural motifs of NFAT family members, including the rel homology domain, the NFAT homology domain and the nuclear translocation signals. mNFATc complexed with AP-1 bound specifically to the murine IL-2 NFAT recognition sequence and activated transcription from the co-transfected IL-2 promoter in COS-7 cells. Northern blot analysis showed that the cDNA probe hybridized with a 4.5 kb transcript which is highly inducible in murine T cells. By Northern and in situ hybridization, mNFATc transcript was detected from the early stage of development. In the mouse embryo, mNFATc transcript was strongly expressed in thymus, lung and submandibular gland and weakly in skeletal muscle and heart suggesting that mNFATc may have a role both in embryogenesis and in mature T cells.

Restrictive practice environment and nurse practitioners' prescriptive authority.

This study analyzes the impacts of several variables, including a restrictive practice environment, on a nurse practitioner's level of autonomy with respect to prescribing selected categories of medicines. A general linear model is applied to data from the 1992 national sample of nurse practitioners. Results show that among the significant independent variables, a restrictive environment of imposed state laws and regulations reduces nurse practitioners' level of autonomy in prescribing medications and acts as a barrier to their practicing to full potential. Results and policy implications are discussed.

Returns to nursing education: rural and nonrural practice.

This study uses data from a national sample of registered nurses to compare earnings of nurses in rural and nonrural practice. The comparisons, conditioned by the nurses' education level, are analogous to the concept of "returns to human capital investment" used in labor economics. A general linear model is applied within a framework of labor economics analysis. Results show that nurses with more education receive less for their investment if they practice in rural areas. Work experience and employment setting are also related to lower annualized earnings for rural practice. One exception to the otherwise consistent findings is that returns to advanced practice nursing are higher in rural areas. Results and policy implications are discussed.

Electrophoretic determination of aqueous and serum neuron-specific enolase in the diagnosis of retinoblastoma.

PURPOSE: Neuron-specific enolase (NSE) of containing gamma-enolase is considered valuable in the diagnosis of tumours of neuroectodermal origin. METHOD: We used rapid electrophoretic method on cellulose acetate plate to determine the pattern of enolase isoenzymes in 21 aqueous humor and 23 serum specimens from retinoblastoma (Rb) and 21 aqueous and 25 serum specimens from 25 control cases to evaluate NSE in the diagnosis of Rb. The assay allowed assessment of all three major isoenzymes (alpha alpha, alpha gamma and gamma gamma), and NSE relative activity and its percentage in the total relative activity of the three enolase isoenzymes were assessed by means of fluorometer. RESULT: Aqueous from all patients with Rb contained alpha alpha, alpha gamma and gamma gamma isoenzymes and presented strong positive, the positive rate of NSE being 100% and its relative activity accounting for 45 +/- 9% of the total relative activity of the 3 enolase isoenzymes; No enolase was detectable in control aqueous with cataract, glaucoma and Coats's diseases (4 cases), but in two patients with traumatic hyphema alpha alpha and faint alpha gamma and gamma gamma were presented in aqueous. The control serum contained only an alpha alpha band, while the sera form patients with Rb contained alpha alpha, alpha gamma and gamma gamma bands in 18 of 23 specimens, the positive rate being 78.2%, and only alpha alpha bands in the rest 5 specimens (21.8%). In patients with Rb, the NSE relative activity and its percentage in the total relative activity of the 3 isoenzymes in serum (36 +/- 9%) were significantly lower lever than those in aqueous. CONCLUSION: The amounts of NSE increase significantly in both serum and aqueous from patients with Rb, and the immunoelectrophoretic assay for determination of enolase patterns is valuable in the diagnosis of Rb and differential diagnosis of Rb. In addition, the method is rapid, simple and requires only a little amount (< 5 microL) of samples.

[Study of the antioxidative effect of tea polyphenol and allitridi on cultured rat lens in vitro].

PURPOSE: To investigate the oxidative modification of lens proteins in the presence of H2O2 in vitro and the protective effect of tea polyphenol(TP) and allitridi. METHODS: Rat lenses were cultured in DMEM medium containing H2O2(1mM) at 37 degrees C in CO2 incubator. Iron and copper salts were added to the medium. At 24 or 48 hours, photographs were recorded the changes in transparency of rat lens and lens proteins were analysed by SDS-PAGE and IEF. TP and allitridi were added to the medium with lens in order to test the antioxidant effects. RESULTS: Lens opacities occurred at the equatorial region by 24 hours, progressing to totally opaque by 48 hours. SDS-PAGE revealed new bands higher than 43KD were found. In addition the 30KD band disappeared in medium containing copper salt. With addition of TP or allitridi to the culture system, all lens remained transparent. CONCLUSION: Exposure rat lens to H2O2 with iron or copper salt resulted in opacification of lens with crosslinking of crystallins and degradation of lens polypeptides. Addition of TP or allitridi prevented the lens from oxidative stress.

[Cultivation of Polygonum multiforum].

The experiments of some cultivated medthods of Polygonum multiforum show that the direct sowing of seeds is better than the cuttage, the index of reproduction and yield are higher, and the cultivated method can decrease expense. To cultivation period of 5 years, high yield and excellent qualty is available.
