Unusual auricular complications in cutaneous oncology.
The anatomic complexity of the pinna predisposes that structure to a variety of unique, site specific postoperative complications following management of skin cancer. We describe four unusual auricular complications: 1) radiochondronecrosis; 2) autonecrosis of skin during second intention healing; 3) hearing loss secondary to tragal retraction over the external auditory canal; and 4) extension of tumor through fenestrated cartilage. Well-known postoperative auricular complications are reviewed and anticipation and recognition of these unusual complications are emphasized.
Perplexing pigmented papules simulating malignant melanoma.
The clinical presentation of irregularly pigmented papules raises a spectrum of diagnostic possibilities. An eighty-five-year-old black man was referred for evaluation of lesions of fifty years' duration. The lesions were 1 to 2 cm hyperkeratotic, irregularly darkly-pigmented papules limited to sun-exposed areas. The histopathologic findings of large keratinocytes, acanthosis, hypergranulosis, and orthokeratosis were compatible with a diagnosis of large cell acanthoma. We believe large cell acanthoma should be included in the differential diagnosis of keratotic or irregularly pigmented lesions on sun-exposed skin, even in black patients.
Ichthyosiform and morpheaform sarcoidosis.
Specific (usually papules and nodules showing a granulomatous histology) and non-specific (e.g. erythema nodosum) cutaneous lesions presenting manifestations of sarcoidosis have been well described. Two patients with unique presentations are here described; one with ichthyosiform cutaneous lesions and one with not previously described cutaneous lesions which mimicked linear morphea (localized scleroderma). The association of articular manifestations without pulmonary involvement was also unusual.
