The results of cholecystectomy from mini-approach (CEMA) in 111 elderly and old patients with acute and chronic cholecystitis living in European North of Russia were analyzed, and also 84 patients were operated by traditional approach (TCE). Duration of CEMA was less than TCE (75 +/- 3.2 and 95.2 +/- 4.6 min respectively; p < 0.05). Sutures after CEMA were removed on day 8.4 +/- 1.2 (after TCE--on day 13.8 +/- 2.4, p < 0.05). Postoperative period after CEMA was 11.4 +/- 2.1 days vs. 18.8 +/- 3.5 days after TCE (p < 0.05). Complications after CEMA were seen in 1.8% patients, after TCE--in 5.0%. Lethality was 0.9% after CEMA and 3.5% after TCE. The main advantages of CEMA are: reduction of surgery time, early activation of patients, decrease of postoperative complications number and reduction of postoperative treatment time (11.4 +/- 2.1 days after CEMA and 18.8 +/- 3.5 days after TCE, p < 0.05).
Clinical trial of well-known and available in Russia enzyme-containing dressings (ECD): dalcex-tripsin and its modification--tripsin-chlorhexidin and tripsin urea containing, profezim, sipralin, lysosorb, collavin and teralgim. The most effective ECD for cleansing of festered wounds, burns and trophyc ulcers are collavin and lysosorb (application wound draining sorbents from helevin and proteolytic enzymes--collagenase of crab and terrylytin, respectively). Combined usage of sorbents and antiseptics substantially increases their curative properties. The other ECD have some shortcomings (low sorption and draining) which makes it possible to recommend them at present time only for limited usage. Draining sorbents and their combinations hold promise for field surgery, pyogenic traumatology, combustiology, cryopathology, gynecology, otolaryngology, neurosurgery, dermatology.

Morphological evaluation of composition and structure of a biologically active compound (BAC) based on sodium alginate, comprising 1% sizomycin and 0.25% protease C, on wound healing in infected rats wounds was made in experiment. The results have clearly shown that for the infected wound treatment during the first period of wound healing BAC of polyfunctional action must be applied. Biologically active substances (sizomycin and protease C) promote removal of inflammatory etiopathogenic factors, i. e. necrotic tissues and microbic bodies, while polysaccharide basis of the composition promotes stimulation of reparative processes in the wounds. Powder with granules size 315 m including 24% potassium gluconate is believed most suitable BAC structure.
Immunohistochemical study of tissues of purulent wounds in rats after application of the collagenase isolated from the king crab Paralithodes camtschatica has been undertaken. The enzyme therapy resulted in a rapid and efficient removal of necrotic debris. It was accompanied by fibrin elimination from the wound bottom and subsequent formation of new capillaries. Cellular fibronectin with ED-A sequence was identified in the newly formed granulation tissue, which points to its active synthesis in situ. Detection of type I collagen in granulation tissue revealed that wound treatment with crab collagenase had no impact on the development process of the tissue. Polyclonal antibodies against two isozymes of crab collagenolytic protease were obtained. It was shown that after application of both isozymes of the collagenase were accumulated in fibrin deposits at the wound bottom but not penetrated in adherent granulation tissue. These processes underlie the therapeutic effect of the crab collagenase.
The paper presents the results of experimental morphological evaluation of the effect of a new proteolytic enzyme collagenase of crab Paralithodes camtschatica on wound healing in infected and aseptic rabbit wounds. The enzyme was applied on wounds using gauze and gelevin. The findings show that this protease is highly effective for debridement of infected wounds, the effect increasing at gelevin addition. To reach maximal therapeutical effect and diminish the inhibition effect of collagenase on granulation tissue, it is recommended to reduce the dose of protease during debridement process. Clinical application of crab collagenase must be individual as well as duration of wound enzymotherapy.

The authors present data on the development and introduction of anesthetic techniques during cardiac surgery at the Institute of Thoracic Surgery, USSR Academy of Medical Sciences, in 1956-1960 and after its reorganization to the Institute of Cardiovascular Surgery, USSR Academy of Medical Sciences, in 1961-1965. It is shown that in the years of introduction of closed operations on the heart, the methods of one- and many component inhalational anesthesia were mastered, its techniques were developed, anesthesia apparatuses and an anesthesia schedule were designed, cardiac anesthesiological studies were conducted, training of physicians from the country's regions was initiated, and the first guidelines for general anesthesia were published. In these years, the firm foundation was laid for the development of cardiac anesthesia. Later on the Institute developed and introduces all basic types of inhalational anesthesia during operations on the open heart under both extracorporeal circulation and hypothermia. The gained experience allowed the laboratory staff to defend several dissertations, to issue two monographs, and to analyze errors and risks of general anesthesia in patients with cardiovascular diseases at surgery.
