[beta2-Microglobulin in synovial fluid (author's transl)].

In a series of patients with rheumatoid arthritis we found higher levels of synovial beta2-microglobulin (5,2 +/- 3 mg/l) than in gouty arthritis (2,4 +/- 0,6) or osteoarthritis (1,2 +/- 0,3 mg/l). The ratio of synovial/plasma beta2-microglobulin was always superior to unity (2,3 +/- 0,7) in the R.A. patients whereas it is always inferior to unity in the other groups. The correlation between plasma or synovial beta2-microglobulin and synovial lymphocyte count was highly significant in all cases in RA patients plasma and synovial beta2-microglobulin are significantly related to the degree of joint involvement as expressed by the joint count.

Treatment of high-renin hypertension with propranolol in children after renal transplantation.

Ten children with hypertension poorly controlled with other drugs and high peripheral plasma renin activity after renal transplantation were treated with propranolol. The mean systolic pressure decreased from 139 to 127 mm Hg (p less than 0.05) and the mean diastolic pressure from 98 to 83 mm Hg (p less than 0.01). Eight children had an antihypertensive response; two did not respond. The maximum dose of propranolol in responders varied from 1.0 to 6.2 mg/kg/day and duration of treatment until response varied from four to 49 days. PRA, repeated in seven responders, decreased in all (p less than 0.01). There was no correlation between changes in PRA and blood pressure. Propranolol was well tolerated and was a valuable antihypertensive drug in these children.

Protein degradation in muscle: response to feeding and fasting in growing rats.

Degradation rates of muscle proteins were determined in young rats allowed access to standard rat chow 12 h/day. degradation was assessed by determination of 3-methylhistidine (3MH) excretion rates. 3MH is a nonreutilized amino acid produced almost exclusively within the actin and myosin of skeletal and cardiac muscle. Because plasma levels of 3MH are low and renal clearance is high, excretion reflects myofibrillar degradative rates. Excretion of 3MH was determined for 4-h periods beginning 12 and 20 h after initiation of feeding and after 24-and 48-h fasts. Excretion of 3MH per 4-h period increased with time after the last feeding. Because creatinine excretion is a function of muscle mass dividing 3MH excretion by creatinine excretion represents myofibrillar degradation per unit muscle mass, the fractional degradative rate. Degradation rates rose from 4.6 to 14.5%/day between 12 and 16 and 60 and 64 h after the beginning of the last meal. These results support the presence of a diurnal pattern of protein degradation as well as increased muscle degradation during starvation.

Automated analysis of total urinary hydroxyproline based on resin-catalysed hydrolysis.

An automated method for the analysis of total urinary hydroxyproline using strong cation resin tablets of Hypronosticon is described for use in a clinical laboratory and the results are compared with those obtained by other methods. Even though good recoveries are obtained using the technique described in the present work by adding the internal standards either before or after hydrolysis of urine, the present method gave consistently lower values of urinary hydroxyproline compared with a manual and an automated method.

Measurement of glomerular filtration rate by fluorescent excitation of non-radioactive meglumine iothalamate.

The clearance of non-radioactive meglumine Iothalamate (Conray 60), assayed by fluorescent excitation analysis, has been shown to be a good indicator of the glomerular filtration rate (GFR). Investigations in rats by the constant infusion technique showed a good correlation with insulin clearance. In a group of ten children and one adult, we obtained excellent agreement between GFR determinations by clearance of Conray 60 and I-125 sodium Iothalamate, using the single injection technique, and taking samples from 10 min to 4 hr after injection. The decay curve was resolved into two exponential components. We have also shown that the 3 and 4 hr samples, fitted to a single exponential function, yield good agreement with GFR determinations using a double exponential model. The GFR determination method presented here is accurate, simple, cost effective, and involves no radiation exposure to the patient.
