Multicentric giant lymph node hyperplasia with ascites and double cancers, an autopsy case.

A 70-year-old man, diagnosed to have multicentric giant lymph node hyperplasia (MGLNH) of the plasma cell type by postmortem examination, had double cancers of the thyroid and kidney, as well as a large amount of ascites and persistent serositis. Serum immunoelectrophoresis showed monoclonal IgG (lambda). Using the paired immunofluorescent technique, monoclonal plasma cell proliferation was observed on the section of a lymph node. The cause of the ascites was speculated to be the combined influence of peritonitis, renal dysfunction and obstruction of abdominal lymphatic ducts. Occurrence of double cancers and persistent peritonitis suggest the long-standing faulty immune regulation in MGLNH.

Culture and changes in dynamics of proliferation of pulmonary small cell carcinoma cells.

We had developed a method for the selective cultivation of cancer cells in short-term which was simple and required minimal technical skills. There have been so far 23 cases of pulmonary small cell carcinoma in which long-term passage of more than one year and establishment of a cell line were successful by the culture method. In particular, from November 1982 to March 1987, there were 29 patients who were diagnosed as having pulmonary small cell carcinoma on preoperative cytodiagnosis and whose cells were primary cultured. In 24 (83%) of these, isolation could be carried out in a short period and stable passage was possible. Long-term passage of more than one year and establishment of a cell line were successful in 21 (72%) of the 29 patients. As a result of primary culture of small cell carcinoma cells, a pattern of a suspended colony was observed in 22 (88%) of 25 cases. As passage advanced, the tendency toward attachment to the basal surface increased. The changes in dynamics of proliferation after long-term passage of more than one year were divided into three types: epithelial like cell type was observed in 8 (32%), the neurite like cell type was observed in 4 (16%) and the suspended colony type was observed in 13 (52%) of the 25 cases. Variant clones composed of relatively large cells appeared as a result of long-term passage or the use of carcinostatics in some pulmonary small cell carcinoma cells.

Syringomyelia extending to the basal ganglia. Case report.

A 10-year-old girl was admitted to the hospital with complaints of progressive right hemiparesis and sensory disturbance. Magnetic resonance imaging revealed a Chiari Type I malformation and syringomyelia from T-10 to C-1. The syrinx extended from the medulla to the right putamen along the pyramidal tract.

Age-dependent and tissue-specific changes of DNA methylation within and around the c-fos gene in mice.

The degree of DNA methylation in the c-fos gene and its vicinity in liver, brain and spleen of mice of different ages was examined using methylation-sensitive restriction endonucleases. The gene had an invariable unmethylated domain from 1.8kb upstream of the cap site to the first intron. The domain was flanked on both sides with partially methylated areas, where tissue-specificity and age-dependent changes were observed. The tissue-specificity was found on the 14th day of gestation. Increases of methylation were observed during the late fetal and one month postnatal developmental periods in the three tissues. After that, the methylation states remained unchanged. One exception was seen in the exon 2 area in liver, where methylation continued to increase throughout life. This revealed the importance of the late fetal- and post-natal developmental periods in the establishment of tissue-specificity in DNA methylation profiles and further showed that some of the developmental changes could extend into the senescent phase.

[A technique of end-to-side anastomosis of a prosthetic graft to the ascending aorta: reinforcement of the anastomosis with wrapping of the ascending aorta].

In temporary, permanent or extra-anatomic bypass grafting to the side of the ascending aorta, a technique of prosthetic fabric wrapping of the ascending aorta associated with reinforcement of the anastomosis is devised as a simple and useful method for aortic surgery. With this technique, dislodgement of the vascular forceps can be prevented completely, and the ascending aortic wall may be protected from injuries owing to the vascular forceps.

[Fulminant autoimmune hemolytic anemia with multiple organ failure].

We report a rare case of fulminant autoimmune hemolytic anemia (AIHA) with multiple organ failure (MOF). A 40-year-old man was emergently admitted to our hospital because of conscious disturbance and jaundice. The peripheral blood revealed RBC 68 x 10(4)/microliter, Hb 3.5 g/dl, Ht 8.9%, Ret 30% (20,400/microliter), WBC 20,300/microliter, Plts 16.9 x 10(4)/microliter, indirect bilirubin 9.4 mg/dl. Both direct and indirect Coombs test were positive and the IgG autoantibody was identified. Bone marrow aspiration revealed hypercellularity with increased megakaryocytes and erythroid hyperplasia. The patient was diagnosed as having idiopathic warm type of AIHA and the therapy was started with prednisolone 80 mg/day from the first day of admission but hemolysis with reticulocytopenia was so rapidly progressive that he was in acutely life-threatening state and MOF (acute renal failure, adult respiratory distress syndrome, congestive heart failure, liver dysfunction, rhabdomyolysis) appeared on the third hospital day. Plasma exchange therapy and hemodialysis were started and high dose methylprednisolone was given soon after rapid administration of sufficient blood transfusion. Dramatic improvement of hemolysis was noted and MOF was controlled after starting these therapies, but he died of exacerbation of MOF probably due to sepsis 40 days later.

[A case of hemangioma of the heart].

A-74-year old woman who had suffered from anterior chest pain and palpitation was admitted to our hospital. The chest X-ray photograph showed that the heart was enlarged mildly. We found that the tumor lay in the mediastinum by the echocardiograph and was enhanced at the aortic phase of the radioisotope angiography. In order to obtain the histological specimen, she underwent exploratory thoracotomy. The tumor was diagnosed as a cavernous hemangioma. She has been asymptomatic over 1-year period. The RI angiography seemed to be useful to diagnose the hemangioma of the heart.

[Determination of nicotinic acid in injections by high-performance liquid chromatography].

A high-performance liquid chromatographic method for determination of nicotinic acid (NA) in commercial injections was established. NA was determined by high-performance liquid chromatography on an Inertsil ODS column (4.6 x 150 mm) using a mixture of 0.05M monobasic sodium phosphate solution (pH 3) and methanol (80:20) containing 1.5 mM sodium 1-octanesulfonate as the mobile phase at 35 degrees C and the detection wavelength at 260 nm. The working curve for 0 to 120 micrograms nicotinic acid/ml against 100 micrograms caffeine/ml as internal standard passed through the origin and was linear. On comparing the analytical data obtained by this method and those by the colorimetric method, both data were in good agreement.

[Collaborative study of Japanese Pharmacopoeia Heparin Sodium Reference Standard (Control 871)].

Heparin Sodium Reference Standard for Japanese Pharmacopoeia (JP) is replaced with new material derived from porcine mucosa. The material was dissolved in water, distributed to vials and freeze-dried. The anticoagulant activity of the freeze-dried product was determined in collaboration with four laboratories employing JP method. The weighted mean potency of 101 assays was 1515 international units per vial, and this value was adopted for the potency of new JP Heparin Sodium Reference Standard (Control 871). By the antiheparin test of protamine sulfate, 0.712 mg of the new standard was neutralized by 1 mg of protamine sulfate. Molecular weight distribution of this standard was also measured by means of gel permeation chromatography and compared with those of the former reference standard and bovine heparin.
