Mechanism for the generation of active smooth muscle inhibitory factor (IF) from bovine retractor penis muscle (BRP).

The active smooth muscle inhibitory factor (IF) is generated by acidifying the extracts from the bovine retractor penis muscle (BRP). The activated IF is very similar to endothelium-derived relaxing factor (EDRF). However, the mechanism for the generation of active IF is yet unknown. From reverse-phase high performance liquid chromatographic data, the activities of nitrite loss were closely related to the contents of both sulfhydryl (SH) groups and L-cysteine. Thus, under acidic conditions, RSH and nitrite in the extracts are considered to react as follows: RSH + HNO2----RSNO + H2O, RSNO----(1/2) RSSR + NO, NO + (1/2)O2----NO2, NO2 + (1/2)H2O----(1/2)HNO2 + (1/2)HNO3. In addition to NO, RSNO is also a labile but potent vasodilator. Thus, activated IF seems to consist of RSNO and NO, and thiol-containing small molecules (molecular weight of less than 1,000) from BRP are closely involved in the generation of active IF.

Hepatic arterial infusion of human recombinant tumor necrosis factor-alpha. An experimental study in dogs.

Local and systemic toxicities associated with hepatic arterial infusion of human recombinant tumor necrosis factor (rTNF) were studied in healthy adult mongrel dogs. The animals received saline containing human serum albumin with or without rTNF (0.02, 0.2, or 2.0 mg/m2). Arteriograms were made, and blood samples were collected for hematologic and biochemical analyses at regular intervals. The dogs were killed at 1, 3, and 7 days postinfusion and complete necropsies were performed. Specimens were obtained from various tissues for histopathologic evaluation. Results indicated that all but the highest dose of rTNF were well tolerated. Severe histopathologic changes were found in the liver, spleen, and kidneys of the animals receiving 2.0 mg/m2 rTNF. In addition, focal tubular degeneration was found in one dog administered 0.2 mg/m2 rTNF. These data suggest that the upper dose limit for hepatic arterial infusion of rTNF is between 0.2 and 2.0 mg/m2 and that renal function should be closely monitored after infusion.

A case of primary malignant lymphoma of the brain with high uptake of 123I-IMP.

In general the 123I-IMP SPECT image of a brain tumor is visualized as a defect. Tumor cases with high IMP uptake have been rarely reported. We encountered a case of primary malignant lymphoma of the brain with high IMP uptake on both early and late scans corresponding to the CT lesions. Previously reported positive cases seemed to be confined to the early scan. For the present the exact behaviour of IMP is incompletely understood, but the mechanism of high uptake has been accounted for by increased extraction and increased amine receptors at the tumor sites. We need further information to establish whether these SPECT findings are specific for primary malignant lymphoma of the brain.

Silent myocardial ischemia in the aged: a retrospective study from the evaluation of necropsy hearts.

The diagnosis of silent myocardial ischemia in the aged remains controversial. In this study to evaluate the basic state of silent ischemia in the aged, a total of 972 consecutive autopsy cases (mean 79.0 yrs of age) were analyzed in terms of coronary atherosclerotic diseases. The following results were obtained: (1) Severe coronary occlusive lesions simultaneously detected in the three major coronary arterial branches were found in 85 cases (8.7%), of which 10 (12%) had neither history of angina nor myocardial infarction (MI), which may correspond to one representation of silent ischemia. (2) There were 218 cases of MI (22.4%), of which 106 (49%) were never documented to have MI during life. The latter may correspond to unrecognized MI. It was considered to be caused by 1) small or middle-sized MI in 66 cases and 2) lateral or nontransmural MI in 29 out of 40 large MI cases (more than 4.9 cm in diameter). (3) 30 cases (54%) out of 56 fresh and large MI with a correct diagnosis revealed no chest pain. This is an incidence of painless MI in the aged. This painless group showed a significant difference in the incidence of a correct diagnosis of MI (51% vs 89%), combined arrhythmia (47% vs 79%) and the association of CVD (47% vs 11%) compared with chest pain group.

A clinicopathologic study of the accessory bypass tracts in six cases of Wolff-Parkinson-White syndrome.

A clinical and histologic correlation study was conducted to examine the accessory bypass tracts in 6 cases of Wolff-Parkinson-White syndrome. (1) Types A, B (Rosenbaum) and C (Ueda) were found in 2 cases each. From the polarity of the delta waves, the site of the bypass tract was assumed to be in the left posterior and left lateral free wall in type A, the right lateral and right posterior paraseptal regions in type B and in the right posterior paraseptal and right anterior paraseptal areas in type C. (2) In 6 cases 19 bypass tracts were found, consisting of 18 Kent bundles and 1 Mahaim fiber, from a total of 146,400 serial sections. The site of the bypass tract was the left lateral and left posterior walls in 2 type A cases, and the right lateral and right posterior walls in 2 type B cases, with good correspondence to our assumption. In both type C cases it was biventricular, composed of left posterior paraseptal and right lateral regions with an additional Mahaim fiber in 1 case. (3) A total of 46 cases of WPW syndrome were collected from the literature and the present paper; they consisted of 16 cases of type A, 20 of type B, 4 of type A + B and 6 of type C. Seventy % of type A cases showed a bypass tract in the left heart, and 64% of type B in the right heart. In 6 cases of type C, the bypass tract was found in various sites and in various combinations, requiring further investigation.

Stenotic lesions and length of the left main coronary artery in the aged.

To evaluate lesions of the left main coronary artery (LM) in the aged, a pathologic study was performed on a total of 972 consecutive autopsy cases over 60 years of age. The study population consisted of 503 men and 469 women with an average age at death of 79.0 years. In this study the degree of narrowing and length of the LM were evaluated in all cases (T), and the 141 cases with normal hearts (N) were compared with the 202 cases with myocardial infarction (MI). The following results were obtained: (1) the average coronary stenotic index (CSI) was 9.1/15 in T, 5.8/15 in N and 11.2/15 in MI (N less than MI, p less than 0.001), (2) the grade of narrowing in the LM (LMG), when 100% obstruction of the LM is counted as 5, was 1.8 +/- 0.9 in T, 1.3 +/- 0.6 in N and 2.3 +/- 1.1 in MI (N less than MI, p less than 0.001). Cases showing more than 75% narrowing of the LM were found in 58 (6%), of which 35 cases belonged to MI, but development of MI directly due to lesions in the LM was found in only 9 patients (4.5% among MI), (3) the average length of the LM (LML) was 9.4 +/- 4.4 mm in T, 9.0 +/- 4.2 mm in N and 9.3 +/- 3.8 mm in MI (N vs MI; n.s.). Cases presenting left coronary artery dominance (n = 82) or a balanced type (n = 159) showed shorter LML than those with right coronary artery dominance (n = 729) (8.5 +/- 4.0 mm, 8.8 +/- 4.6 mm and 9.7 +/- 4.4 mm, respectively, p less than 0.05), (4) in T there was a correlation between the LMG and CSI, but not between the LML and CSI. In conclusion, lesions of the LM in the aged were generally mild and the LMG showed a positive correlation with CSI and the narrowing of each coronary branch, while no relationship was noted between the LML and CSI.

Isolated right ventricular infarction resulting from thrombotic occlusion of a hypoplastic right coronary artery.

Three cases of isolated right ventricular infarction resulting from thrombotic occlusion of a hypoplastic right coronary artery were found in 4,000 consecutive autopsies performed at Tokyo Metropolitan Geriatric Hospital. The incidence of isolated right ventricular infarction was 0.08%. The clinical profile of the first case was characterized by shock, pulmonary congestion, pleural effusion, decreased V1R and V2R on ECG, a small elevation of CPK and transaminase, elevation of fibrin degenerative products and decreased platelet count. The patient responded to volume expansion, heparin and catecholamines. One year later she died from cerebral bleeding. In the second case, mild aortic regurgitation and atrial fibrillation were present. He died suddenly during an episode of pneumonia. In the third case, there was chronic obstructive lung disease, atrial fibrillation and lung cancer. He died of respiratory failure. On autopsy, the coronary arteries revealed a marked left dominant and right hypoplastic pattern in all cases. The right coronary artery perfused only the free wall of the right ventricle. Complete occlusion of the hypoplastic right coronary artery resulted in isolated right ventricular infarction. In addition, chronic pulmonary disease and arrhythmia may be contributory.

[Electromyographic study on masticatory function of the telescope denture wearers in the Kennedy Class II mandible].

Masticatory function of 10 telescope denture wearers in the Kennedy class II mandible patients was evaluated with electromyography. Electromyographic activity during mastication with kamaboko and peeled raw carrot was recorded from bilateral masseter and the anterior part of temporal muscles at each interval: Before treatment, immediately after denture insertion and 2, 4, 6, 8 weeks after respectively. Mean of duration, interval and cycle of EMG activities, coefficient of variation and average variation were calculated. The results were as follows: 1. Means of duration and cycle times decreased immediately to 2 weeks after denture insertion. 2. EMG activities during raw carrot chewing became regular immediately to 2 weeks after denture insertion, and within 4 weeks in kamaboko chewing. These results suggest that telescope dentures would be adapted to stomatognathic system within short period after denture insertion.

[Prosthodontic treatment for a partial mandibulectomy patient. A mandibular non-reconstructed case].

One of the most consistently frustrating areas of maxillofacial rehabilitation is the treatment of patients who have had radical cancer surgery of the tongue, floor of the mouth, and mandible. The patient described in this report, was a 66-year-old man who had squamous cell carcinoma of the right jaw. The right body of the mandible distal to the right cuspid was resected, and this part of the mandible was not reconstructed. For this mandibulectomy patient, we have treated with the removable partial denture using 30 degree teeth and TS telescope crown as retainer, registrating maxillomandibular relation with Gothic arch tracer and MKG. In addition, masticatory function in this patient was evaluated on tooth contact, tapping pattern, masticatory efficiency and rhythm of mastication. Consequently, 4 to 8 weeks later, tapping pattern and centric occlusion was stabilized, and masticatory function was improved. We will continue to observe this case in long term.

Clinical evaluation of a new resilient denture liner in long-term use.

The new resilient denture liner "M" was evaluated for adhesion, color stability, surface texture, tissue response, deterioration and denture base stability, at periods of 1 month, 3 months and about 30 months after denture liner application. At the observation of approximately 30 months, the usage of 17 dentures had been discontinued within 2 years, and 6 dentures continued to be used. It could be considered that the main reasons why 17 dentures were discontinued were poor fitting caused by deterioration, and a decrease in the strength of the denture base caused by the denture liner lining. It was suggested that "M" was appropriate for long-term use (about 2 to 3 years), if patients faithfully practice denture cleaning, and if the strength of denture base is considered.

[AE characteristics during compression test of high-copper amalgams and visible light-cured composite resins].

Differences in environmental strength of high-copper amalgams and photo-cured composite resins for posterior after storage in deionized water at 37 degrees C for 1 year were evaluated by examining compressive strength and acoustic emission (AE) characteristics. The results were as follows: Microfailure in amalgams and composite resins was detected from the onset of stress and their microfailures toward catastrophic failure occurred at 70 to 90% of their ultimate strength. One-year compressive strength for amalgam decreased to 80 to 98% of its 24-hour compressive strength and that for composite resin to 65 to 75%.

[Treatment of pathological laughing with amitriptyline].

We investigated the efficacy of amitriptyline in seven patients with pathological laughing caused by cerebrovascular diseases. In a double-blind placebo-controlled cross-over trial comparing amitriptyline with placebo, 5 of these 7 patients were judged to have shown improvement with amitriptyline, and 1 patient improved with placebo. This difference was statistically significant. No significant side effect of amitriptyline was observed. There was no significant change between amitriptyline and placebo in both self-rating depression scale and minimental state test score. These results suggested that amitriptyline was useful in treating pathological laughing.

A case of pacemaker lead fracture associated with thoracic outlet syndrome.

A pacemaker lead fracture in the left subclavian vein was caused by compression of the clavicle and the first rib; subsequent coil elongation at the same site on the right side was observed in the replacement lead in a patient with thoracic outlet syndrome. Venography showed narrowing of the subclavian vein at the site where the lead abnormalities were observed. This case illustrates that a lead in the subclavian vein can be easily damaged when a patient has thoracic outlet syndrome.
