Depressed skull fracture in skiing and its experimental study.

During the last seven skiing season, 57 patients were treated surgically for head injuries caused by skiing accidents. A typical type of injury was observed, namely the "depressed fracture" in 84% of the cases, accompanied by brain contusions or hematomas. The mechanism of the accidents was analyzed. A depressed fracture is caused by direct local force application, often as the result of high or not properly adjusted speed. Through this impressive observation of depressed fracture with or without brain damage, experimental impact mechanisms of human skull depressed fractures were studied. In the present study, a tolerance threshold of about 1 to 2 kN (can be compared to an impact of about 16 km/h) was observed for a depressed fracture in the temporal region, which often is the area of severe damage in skiing. This clinical and experimental examination should aid in preventing head injuries in skiing accidents because head injuries are mostly the result of depressed fractures, accompanied by brain contusions or hematomas. From these analyses, it can be shown that the wearing of helmets is in fact an effective possibility for the prevention of head accidents in skiing. It should be recommended particularly for children and young people.

Acrylic fixation in displaced dens fractures.

Non-union of odontoid process fractures with displacement in old patients have been treated operatively stable acrylic fixation. Immediate mobilization without external fixation was achieved.

[A study on luteal function during consecutive cycles (author's transl)].

UNLABELLED: It is conceivable that luteal function is varied with different menstrual cycles. A study was therefore undertaken to investigate for changes in luteal function during consecutive menstrual cycles. METHODS: From those women receiving ambulatory treatment at the hospital 30 displaying a biphasic basal body temperature (BBT) curve were chosen for the study. BBT records were obtained from each of these women for at least 3 consecutive cycles and a total of 194 cycles thus rendered available were evaluated for the pattern of BBT, the duration of high phase (DHP), high phase score (HPS) and planimetric luteal index (PLI). Twenty of these 30 women were subjected further to endometrial dating at the 7th-8th day of high phase with simultaneous determinations of serum estradiol (E2), progesterone (P) and prolactin (PRL) during 2 consecutive cycles. RESULTS: The incidences of abnormal values of the parameters for at least one of the consecutive cycles studied were: DHP; 13.3% pattern; 50-100%, HPS; 40.0%, PLI; 13.3%, E2; 25%, P; 25%, PRL; 30%, endometrial dating; 65%. These results indicate a considerable variability of luteal function.

[Classical low back pain and sciatica syndrome caused by a melanotic nerv sheath tumour of the first sacral root (author's transl)].

This is the description of the case of a 22 year old patient with low back pain and sciatica on the right side investigated by lumbar Dimer-X-Myelography. The result of the examination was an impression in the contrast column at the L5/S 1 level on the right. At operation an intradural tumour, plum-sized, well-defined, and blue-black in colour, was found and removed. Histologically the tumour was melanotic and of high cellular density, with spindle-shaped cells as well as giant cells, with round nuclei, considerable polymorphism, and atypical mitoses. On the basis of the operation finding, the clinical course, and the histology, the diagnosis is melanotic nerve sheath tumour of the first sacral root. For two years following operation the patient has experienced no discomfort.

[Stablization of destructive processes in the cervical spine by means of acrylic prostheses (author's transl)].

Stabilization of destructive processes in the cervical spine by means of acrylic prostheses has hitherto been carried out either by an anterior approach or by a posterior approach. With progressively destructive lesions the stability achieved by either of these methods is unsure, and therefore the author has employed a combination of both approaches. The first two patients treated with this new technique have now been followed up for two years. The position and stability of the prosthesis in each case has remained excellent.

Differential analyses of EMG findings in motor neuron lesions.

The principle in electromyography that positive sharp waves (PSW) and fibrillation potentials (FP) are pathognomonic of lower motor neuron (LMN) pathology is challenged by recent reports that the same potentials are observed in upper motor neuron (UMN) lesions. We have also observed positive sharp waves and fibrillation potentials in patients with UMN lesions, without conditions that involve the LMN. This study was undertaken to ascertain if there are differences in the potentials between the LMN and UMN lesions. Twenty-five UMN and 25 LMN patients were electromyographed. Manual and computer analyses were used to compare parameters from positive sharp wave and fibrillation potentials. Our findings indicate that there are interlesion differences in the mean frequency of occurrence ratio of PSW to FP and in some of the durational wave parameters.

A feasibility study of the AutoPap system location-guided screening.

OBJECTIVE: To study the feasibility of AutoPap System location-guided screening and to evaluate the accuracy of the AutoPap System in selecting locations of potentially abnormal cellular material, the accuracy of triaging slides using the selected locations and the improvement in laboratory accuracy and workload reduction resulting from a combination of location-guided rescreening and AutoPap System primary screening. STUDY DESIGN: For the study, 683 conventionally prepared cervical cytologic smears (263 WNL, 184 atypical squamous cells of undetermined significance/atypical glandular cells of undetermined significance, 173 low grade squamous intraepithelial lesions, 55 high grade squamous intraepithelial lesions and 8 cancer slides) were acquired from nine laboratories. The study slides were independently screened to establish study reference diagnoses. The slides were processed on the AutoPap System and screened by cytotechnologists who were guided by the field of view (FOV) locations provided by the AutoPap System. The location-guided information was used either for initial manual screening (location-guided screening or after initial manual screening of the entire slide (location-guided rescreening). The results were compared to the study reference diagnoses. Sensitivity and workload reduction figures were estimated for different slide populations. RESULTS: The accuracy of location-guided slide triage was high and did not vary significantly over different slide populations. Also, for > 80% of the abnormal slides, the AutoPap System-selected FOV slide locations did contain the most diagnostic cells, which were used to assign an accurate diagnosis to the slide. By combining location-guided rescreening with AutoPap primary screening, the estimated overall location-guided screening false negative fraction (FNF) for LSIL+ slides was approximately 40 times lower than the cytotechnologist-only FNFs, and this improvement was achieved-even at a no review rate of 70%. CONCLUSION: The feasibility study results showed that the AutoPap System location-guided screening process can triage slides accurately and enhance the overall accuracy of slide diagnosis. The combination of location-guided rescreening and AutoPap primary screening could improve the accuracy and workload of the laboratory.

AutoPap system performance in screening for low prevalence and small cell abnormalities.

OBJECTIVE: To summarize the design principles of the AutoPap System evaluation score by evaluating slides having a low prevalence of abnormal cells and small cell abnormalities and assessing the evaluation score as a diagnostic tool. STUDY DESIGN: Data from two clinical studies conducted using the AutoPap System and data obtained from the evaluation score training slides were analyzed to demonstrate the effectiveness of the evaluation score. The clinical studies included a prospective, intended-use study involving approximately 13,000 slides and a comprehensive sensitivity study using approximately 1,200 slides from five laboratories. The evaluation score training set consisted of 4,174 slides from 10 laboratories. RESULTS: The robust design of the AutoPap evaluation score was demonstrated by similar detection capabilities and sensitivities to slides having either a low or high prevalence of abnormal cells. No significant difference in performance was detected between the small cell slides and the comparison groups of carcinoma in situ and invasive squamous carcinoma having normal-sized abnormal cells. In addition, the evaluation scores corresponded well to the diagnostic severity of the slides.

Changing mesophilic wastewater sludge digestion into thermophilic operation at Terminal Island Treatment Plant.

This paper describes the progress up to June 2000 for thermophilic digestion of wastewater sludge at the Los Angeles, California, Bureau of Sanitation's Terminal Island Treatment Plant. The development of the microorganism culture has followed a course similar to that seen at other successful plants for establishment of a stable, well-balanced thermophilic culture in a large digester, but at an accelerated pace. This study began with rapid heating, increasing the temperature of the 4500 m3 (1.2 mil. gal) digester to the target temperature of 55 degrees C at approximately 3 degrees C/d. A method of feeding to maximize the rate of culture development was used as feeding accelerated to approximately 400 m3/d (0.1 mgd). An initial rise of acid concentration (primarily acetate) was seen. Within two weeks, acid concentration declined and stabilized, indicating that acidogenic and methanogenic microbial communities came into balance. Coliform data indicate that digester disinfection was stably effective from the middle of April. The salmonella tests done to date satisfy the U.S. Environmental Protection Agency (U.S. EPA) class A specification. Testing with helminth ova and enteric viruses before and after the digester shows satisfaction of class A standard for those organisms. The present combination of low volatile fatty acids and low hydrogen sulfide is good news for odor control. The data show increases in volatile solids destruction and estimated gas production, compared with the previous mesophilic operation; however, large uncertainties have been calculated from the data. As the digester is now operating successfully at the current feed rate, there seems to be no barriers to processing the entire sludge production of the plant. Other results indicate that the U.S. EPA requirements for exceptional quality class A biosolids are likely to be achieved.
