Demographic and social characteristics of university students attending a psychiatrist.

A survey was made of demographic and social characteristics of patients referred to a Student Health psychiatrist over a two year period. These patients were compared with a group attending Student Health physicians. Significantly over-represented in the psychiatric group were: medical and dental students; students who had repeated an academic year; students over 30 years of age; Roman Catholic students; and students who were totally financially dependent on their parents. Findings by others suggesting that first-year and female students would be over-represented in the psychiatric group were not borne out.

Characterization of the WIDR: a human colon carcinoma cell line.

We describe the establishment and characterization of WiDr, a cell line derived from a human colon carcinoma. It produces carcinoembryonic antigen in culture, and has a doubling time of 15 hr with plating efficiency of 51%. The HLA antigenic profile and the allozyme genetic signature (composed of eight gene-enzyme systems) of WiDr cells are different from those of HeLa cells. Furthermore, WiDr cells possess three marker chromosomes, again distinct from the HeLa marker chromosomes. Finally, it is highly tumorigenic in four different xenogeneic animal models. Based on these studies, WiDr represents a useful model cell line for tumor cell biology investigations.

Helix handedness of Leptospira interrogans as determined by scanning electron microscopy.

Representative serovars and strains of the seven genetic groups of Leptospira interrogans, and two previously studied serovars, were all found to form exclusively right-handed helices as determined by scanning electron microscopy. No change in handedness occurred in cells grown in a minimal medium (Tween-80 albumin) compared to cells grown in a rich medium (rabbit serum). The right-handedness of the organisms was related to the evolution, cell wall structure, and the mechanism of motility of L. interrogans.

Physical properties of some ribosomal proteins in solution and evidence for molecular interactions between isolated ribosomal proteins.

Many previous studies have been directed toward obtaining a physical visualization of the relationship between the protein and RNA in the ribosomal subunits isolated from Escherichia coli. The current study is the first report where an attempt has been made to directly assess interactions between a pair of isolated ribosomal proteins separate from the intact system by means of sedimentation equilibrium analysis. The molecular weights of the proteins S3, S4, S5, S6, S7, S8, and S20 from the 30S subunit of the E. coli ribosome were determined under conditions of assembly of the subunit by sedimentation equilibrium. All of the proteins exhibited molecular weights consistent with monomeric behavior (i.e., in agreement with the measurement of the ultimate molecular weight in denaturing solvents as reported in other studies as well as in the current study) except S8 which indicates a tendency to self-associate. Hydrodynamic measurements on the proteins indicate that these proteins are not completely disorganized in solution such as a random coil, although not as compact as globular proteins. The frictional coefficient ratios found for these ribosomal proteins range from 1.4 to 1.9. The hydrodynamic data are discussed as containing some evidence that stable interaction sites could exist in the proteins. The molecular weight data are considered pertinent to a sedimentation equilibrium study of protein-protein interactions that may be occurring in the ribosomal subunits. Two proteins, S3 and S5, considered in this investigation were found to exhibit no tendency to self-associate under conditions of reassembly. When the two proteins are mixed under those same conditions, however, a species with a molecular weight greater than that of either S3 or S5 is observed to be formed. The interpretation is presented that a molecular interaction between S3 and S5 is the cause. The system is described as containing S3, S5, and a complex between S3 and S5 with a stoichiometry of 1:1 and an association equilibrium constant of 5.7 times 10-5 l./mol (delta G-o equals minus 7.25 kcal/mol). Since the association appears to be specific and of moderate strength, it is concluded that the interaction could have some pertinence with respect to conferring a structural arrangement in the ribosomal subunit. Moreover, it is concluded that protein-protein interactions, in general, must be considered in addition to the well documented significant RNA-protein relationships when models for ribosome structure and assembly are formulated.

Clinical neurophysiology referral patterns to a tertiary hospital--a prospective audit.

BACKGROUND: Cork University Hospital (CUH) provides a tertiary service for all neurophysiology referrals in the Southern Health Board region. AIM: To ascertain the number, source, symptoms and diagnosis of neurophysiology referrals at CUH. METHODS: We did a prospective audit of the referral patterns to the neurophysiology department over a 12 -week period. RESULTS: Of 635 referrals, 254 had electromyograms (EMG), 359 had electro-encephalograms (EEG), 18 had visual evoked potentials (VEP), three had somato-sensory evoked potentials (SSEP) and one had multiple sleep latency tests (MSLT). We analysed the demographic pattern, reason for referrals, the average waiting time for neurophysiology tests and the patterns of diagnosis in this audit. CONCLUSIONS: Patients from County Cork are making more use of the neurophysiology services than patients from other counties within the Southern Health Board. The average waiting time for an EEG was 32 days and for an EMG was 74 days. However, more than 35% of those patients waiting for an EEG or an EMG had their tests done within four weeks of referral. The appointments of EEG and EMG were assigned on the basis of clinical need.

Trends in alcohol consumption in undergraduate third level students: 1992-1999.

BACKGROUND: Alcohol consumption has increased in the Irish population in recent years. It is not known to what extent the student population has been affected by this increase. AIM: To determine levels of alcohol consumption among undergraduates in one Irish university and identify changes in drinking patterns in the years 1992-1999. METHODS: Information on alcohol use was obtained by anonymous self-completed questionnaire in a stratified random cross-faculty sample of undergraduates in 1992 and 1999. The CAGE questionnaire to determine problem drinking was included in both surveys. RESULTS: A statistically significant (p=0.01) drop in weekly alcohol consumption by males was found, although the proportion of male problem drinkers increased. Consumption for females remained the same. CONCLUSIONS: Findings are contrary to recent figures for drinking patterns in young Irish people in general. The fall in alcohol consumption in male students may be linked to improved male insight into the negative effects of alcohol or to the substitution of cheaper available substances.

Fetal and neonatal cerebral oxygen monitoring with NIRS: theory and practice.

Near infra-red spectroscopy (NIRS) is a comparatively new method for monitoring the oxygenation in blood and tissue in the brain of the fetus and the neonate. Absorption of light in the wavelength range 700-1000 nm through such tissue is measured, which is then used to calculate changes in the concentration of cerebral oxygenated and de-oxygenated haemoglobin (HbO2 and Hb) and hence cerebral blood volume (CBV). Studies carried out on several groups of newborn babies have shown clear changes in HbO, Hb and CBV with hypoxia and bradycardia. These changes may well have implications in the occurrence of hypoxic/ischaemic brain injury. Intra partum NIR measurements on the fetal brain have demonstrated clear changes in HbO2, Hb and CBVm, coinciding with the onset of contractions.

Turf-toe: an analysis of metatarsophalangeal joint sprains in professional football players.

Metatarsophalangeal joint injuries of the great toe (turf-toe) are receiving increasing attention in the literature because of the prevalence of synthetic surfaces and lighter, more flexible shoes. Eighty active professional football players were evaluated. The mechanism of injury was hyperextension in 85% of the players. Eighty-three percent reported their initial injury on artificial turf (P less than 0.05). Other factors significantly related to the incidence of turf-toe included player age (P less than 0.01), number of years in professional football (P less than 0.01), and range of ankle dorsiflexion (P less than 0.05). Turf-toe injury resulted in significantly decreased range of motion of the first metatarsophalangeal joint (P less than 0.01).

Septic arthritis after arthroscopic anterior cruciate ligament reconstruction. Diagnosis and management.

We performed a retrospective study of knee joint infections after arthroscopic anterior cruciate ligament reconstruction at our institution. Two thousand five hundred anterior cruciate ligament reconstructions were performed between 1988 and 1993. Seven (0.3%) patients experienced postoperative deep infections of the knee. All anterior cruciate ligament reconstructions were performed using arthroscopically assisted techniques. Six (86%) of these patients had concomitant open procedures performed, including meniscal repair, posterolateral corner reconstruction, and medial collateral ligament reconstruction. Four patients had acute (< 2 weeks), two patients had subacute (2 weeks to 2 months), and one patient had late (> 2 months) infections. All patients had positive cultures from knee joint aspirates with the organisms Staphylococcus aureus, Staphylococcus epidermidis, Peptostreptococcus, or a combination thereof. All patients underwent immediate arthroscopic irrigation and debridement. All infections were intraarticular; six patients also had extraarticular sites of infection. Four patients underwent repeat irrigation and debridement at approximately 1 week. The anterior cruciate ligament graft was removed from four patients. All patients were treated with intravenous antibiotics for 4 to 6 weeks, protected weightbearing, and physical therapy. At a mean followup of 29 months, mean knee extension was 0 degree, and mean knee flexion was 122 degrees (range, 70 degrees to 135 degrees). Six (86%) patients had minimal to no pain in their operative knee, and they were satisfied with their functional results.

"T-cell-rich B-cell lymphoproliferative disorder" of the bone marrow.

We report four cases of a "T-cell-rich B-cell chronic lymphoproliferative disorder" involving the bone marrow and not extramedullary sites. The neoplastic B-cell proliferation in these cases was composed predominantly of small lymphoid cells with features of both hairy cell leukemia and lymphoplasmacytoid lymphoma. All cases presented with neutropenia and with difficulty in diagnosis. We present the clinical, morphologic, cytochemical, and immunophenotypic findings in these cases and discuss this entity.

Leptomeningeal disease in pre-leukemic syndrome: cytogenetic abnormality versus cellular morphology.

Refractory anemia with excess blasts in transformation (RAEB-T) is a pre-leukemic syndrome that can progress to acute myelogenous leukemia (AML). Although leptomeningeal disease in AML is rare and it is commonly associated with specific cytogenetic abnormalities, it has not been documented in RAEB-T. We report a woman who experienced seizures and had subarachnoid enhancement on MRI suggestive of leukemic infiltrates in the leptomeninges. Her bone marrow aspirate revealed 6.6% blasts and the presence of Auer rods, findings consistent with a diagnosis of RAEB-T. Subsequent cytogenetic analysis detected an uncommon deletion of chromosome 11q23, and the breakpoint on chromosome 11 is identical to that seen in AML patients having leptomeningeal disease and translocation between chromosome 9 and 11. Although RAEB-T is not considered as leukemia and is thought unlikely to involve the central nervous system, the presence of leptomeningeal disease and 11q23 deletion in our patient suggests that cytogenetic abnormality may be an important determinant of neurological complications, regardless of a diagnosis of RAEB-T or AML which is based on cellular morphology.

Deliberate self harm: the hidden population.

Population-based epidemiological surveys of deliberate self harm (DSH) exclude homeless people and do not represent them in their conclusions. However, DSH in the homeless is commonly seen in inner city hospitals and the homeless are probably at higher risk of eventual suicide. We examined the demographic characteristics of all patients who presented to an inner London hospital after an episode of DSH over a period of three years and found that 15% of patients were of no fixed abode. Alternative methods of studying DSH need to be explored if suicide is to be prevented in this group of people.
