[Inpatient psychotherapy of anxiety disorders--a comparison of therapeutic effectiveness in patients with generalized anxiety disorder, agoraphobia and panic disorder].

We investigated the effect of psychoanalytically oriented inpatient psychotherapy on three essential forms of anxiety disorders (classified according ICD-10): 1. generalized anxiety disorder (n = 23), agoraphobia (n = 38) and panic disorder (n = 24). Examinations with psychometric test instruments were performed at beginning of inpatient psychotherapy, at dismissal and 6 weeks after dismissal. At the end of inpatient psychotherapy we judged four clinically relevant criteria of therapy success: at the one side the therapist's judgement of symptomatic and structural improvement of the patients, at the other side the patients' judgement of anxiety and depression diminuation. The results demonstrate that summing up these four success criteria 1.) 40.0% of the patients with anxiety disorders improved, 21.2% failed; 2.) 61.4% of the patients with agoraphobia improved, 6.3% failed and 3.) 52.5% of the patients with panic disorder improved, 6.5% failed. An additional analysis of these results showed that the patients with generalized anxiety disorder were significantly (p < 0.01) more disturbed (according clinical assessment) compared to the other two diagnostic groups. About 60% of these patients exhibited increased anxiety symptoms during the course of inpatients psychotherapy, nevertheless relative to the other success criteria these patients achieved satisfactory outcome results. In the paper the consequences of these findings are discussed especially for inpatient psychotherapy of severely disturbed patients with anxiety disorders.

[Depersonalization and self-injury].

Patients with deliberate self harm syndrome and with factitious disorders often describe depersonalisation phenomena, during which they have a diminished pain sensitivity or analgesia. The self-mutilating act can stop the depersonalisation temporarily. Concerning the psychodynamic processes there are common traits between depersonalisation and self-mutilation. The connections between depersonalisation and self-mutilating behaviour are described. Depersonalisation is understood as a defense mechanism, ranging between mature and immature defense mechanisms. An illustrative case is demonstrated.

["Traditional" vs. "modified" approach to research within the scope of psycho-oncologic studies. Theoretical considerations and initial results].

The paper points out two methods of research in the field of psychosomatic resp. psychooncologic research. The "traditional method" of an Inter-Group-Comparison is confronted with the "modificated method" of an Intra-Group-Comparison. The methodological advantage of taxonomic subgroups is demonstrated at a population of 60 breast cancer patients. Two homogeneous subgroups of the investigated population can be distinguished not only by contents criteria but also by means of a cluster-analysis. The prognostic importance of the psychodynamic profile within the different subgroups is discussed. In this context defense mechanisms are particularly relevant.

[Pelvic multivisceral resection from the viewpoint of gynecology].

The pelvic exenteration procedure must be considered a multimodal therapeutic concept from a humanistic-medical point of view. Careful patient selection, radical resection of all tumor-infiltrated organs, complete interdisciplinary organ reconstruction, and optimal perioperative management including full psychosomatic rehabilitation are mandatory. This concept should only be realized in medical centers with highly sophisticated infrastructures.

[The patient with psychogenic and psychosomatic pain. Outline of psychoanalytically oriented nosology].

Starting with the contribution of G. L. Engel (1959) a systematization of the psychodynamic conceptions of pain and pain disorder is given. Four different explanations for the genesis and maintenance of pain states are worked out: (1) The narcissistic mechanism (substitution), (2) the conversion mechanism (symbolization), introduced by S. Freud himself, (3) the psychovegative mechanism (resomatization), and (4) learning principles. These four conceptions are presented. Finally concern is expressed as the conceptual range of three of these explanatory principles is thought to cover a relevant range by far not reached by other pain theories more en vogue.

[Psychoanalytically oriented therapy procedures in pain].

Psychodynamic approaches stress the relevance of family and social factors in childhood for pain proneness in adults. A psychodynamic nosology of psychogenic and psychosomatic pain syndromes is outlined. Starting points of different therapeutic approaches are presented. A review of therapy studies and their results is given. Two case studies illustrate the process of two different psychoanalytically oriented therapies (inpatient therapy, outpatient group therapy).

[Ethology, the real trauma and neurosis. Attempt at an evaluation of the contribution of John Bowlby in understanding the origin of emotional disorders].

This paper tries to trace back the contribution of John Bowlby to the concept of development of psychic disorders. The characteristic features of Bowlby's conception, which centers around the preservation and impairment of the behavioural system "attachment", are presented and their origins especially from psychoanalysis, biology and ethology are shown. This research study was initiated by the psychoanalytic criticism of John Bowlby's positions, and one of its aims is to clarify their concordance with or deviation from psychoanalysis. Finally clinical contributions of Bowlby are presented which are scarcely known in Germany (for instance the discrimination of phobia and pseudophobia). In trying to appreciate the significance of Bowlby for the further scientific development of psychoanalysis an important task is assigned to him, since he insists in providing evidence of those psychoanalytic theories he applies.
