Cytokine release during hypoxia reoxygenation by Kupffer cells in rats with obstructive jaundice.

The release of cytokine by Kupffer cells during hypoxia/reoxygenation was studied in vitro in male Wistar rats with obstructive jaundice to investigate the kinetics of interleukin-8 (IL-8) release by Kupffer cells during hypoxia/reoxygenation, and to study the influence of endotoxin during the reoxygenation period. The rats were divided into two groups: one that underwent bile duct ligation (group OJ), and one that underwent a sham operation (group C). Kupffer cells were isolated by collagenase digestion and centrifugal elutriation. The cells were first subjected to hypoxia as 95% nitrogen, after which they were given reoxygenation as 95% oxygen. In addition, they were stimulated with lipopolysaccharide (LPS) 0, 1, and 10 ng/ml. In both groups, the levels of IL-8 became increased during the period of hypoxia/reoxygenation, and reoxygenation after hypoxia further intensified IL-8 production. During the period of hypoxia, the IL-8 levels in group OJ were significantly increased compared with those in group C. With the LPS challenge, there was no significant difference in IL-8 levels in either group. In conclusion, obstructive jaundice induces the activation of Kupffer cells, resulting in increased IL-8 production during hypoxia/reoxygenation.

Combined right hepatic and retrohepatic caval resection with reconstruction using a polytetrafluoroethylene graft for primary leiomyosarcoma of the liver: report of case.

We present herein the case of a 53-year-old woman who underwent successful surgical treatment for a leiomyosarcoma of the liver that originated from the posterior hepatic segment and involved the retrohepatic inferior vena cava (IVC). A computed tomographic scan and magnetic resonance imaging demonstrated a large tumor, with rich vascularity, in the liver. The IVC was found to be occluded on these scans, which was confirmed by venacavography. The patient underwent a combined right hepatic and caval resection with reconstruction using an expanded polytetrafluoroethylene graft. The tumor consisted of spindle-shaped cells with cigar-shaped nuclei. It also had a moderate degree of cellularity and ten mitotic figures per ten high-power fields. Immunohistologically, desmin and alpha-smooth muscle actin were stained positive in the tumor cells, implying that the tumor was derived from smooth muscle cells. The patient is alive and well 15 months after her operation.

Surgical stress and the development of complications in relation to polymorphonuclear leukocyte elastase (PMNE) levels.

This study was conducted to examine the effects of surgical stress on changes in polymorphonuclear leukocyte elastase (PMNE) levels, and to evaluate the relationship of these changes to the development of postoperative complication. A total of 69 patients who underwent alimentary surgery were subsequently divided into three groups: a complicated group, comprised of 25 patients; an uncomplicated group with a high blood loss (H) of more than 1000 ml, comprised of 18 patients; and an uncomplicated group with a low blood loss (L) of less than 1000 ml, comprised of 26 patients. The changes in the levels of PMNE, fibronectin (FN), and antithrombin III (AT III) were compared among these three groups. In the uncomplicated H and L groups the PMNE levels rose significantly on postoperative day (POD) 1. On POD 3, high levels of PMNE were still evident in the uncomplicated H group, but a decline was observed in the uncomplicated L group. From POD 7 onwards the levels decreased to the preoperative values in both uncomplicated groups; however, the complicated group continued to show high levels even on POD 14. Significantly decreased FN levels were observed for the first 3 PODs in each group. The uncomplicated H and L groups regained their preoperative levels on PODs 7 and 14, respectively, but no recovery was found in the complicated group. The AT III levels showed similar changes to the FN levels in all groups. These findings indicate that monitoring the PMNE levels could be a useful index for the early detection of postoperative complications following alimentary surgery.

Left ventricular dysfunction and acute lung injury induced by continuous administration of endotoxin in sheep.

Sixteen sheep were surgically prepared for chronic study. Seven days later, Escherichia coli endotoxin (10 ng/kg/min, lipopolysaccharide (LPS) group, n = 10) or an equivalent amount of 0.9% NaCl (Control group n = 6) was administered. Between 1 and 8 h post-LPS, there was a hypodynamic state with low cardiac index (CI, LPS 5.0 +/- 0.2; sham 6.3 +/- 0.4 liters/min/m2 at 4 h). During this period, the left ventricular end-systolic pressure-diameter relationship (ESPDR), a sensitive index of myocardial contractility, was also lower (LPS 10.4 +/- 1.2; sham 17.2 +/- 0.8 mmHg/mm). Mean pulmonary arterial pressure (PAP) and pulmonary vascular resistance index (PVRI) were remarkably increased 1 h after the administration of LPS (PAP:LPS 37.5 +/- 1.9; sham 21.8 +/- 0.9 mmHg, PVRI: LPS 600 +/- 58; sham 158 +/- 23 dynes x s x cm-5 x m2). The early changes in cardiopulmonary function occurred concomitantly with an elevation in tumor necrosis factor (LPS 1221 +/- 520; sham 0 +/- 0 pg/ml) and thromboxane B2 (LPS 1382 +/- 266; baseline 82 +/- 20 pg/ml) in arterial blood. Following this first phase, the sheep presented a persistent hyperdynamic state characterized by a significant increase in CI. The ESPDR continued to fall. By 24 h post-LPS the CI was 10.1 +/- 0.5 liters/min/m2 (sham, 6.3 +/- 0.3) but the ESPDR had fallen to 8.2 +/- 2.3 mmHg/mm (sham 16.0 +/- 3.0). Left ventricular performance during continuous endotoxin-induced hyperdynamic endotoxemia in sheep.

Cardiac function was studied in an unanesthetized ovine model of hyperdynamic endotoxemia. Sixteen sheep were instrumented with ultrasonic crystals on the left ventricle to measure changes in its external diameter, a pressure transducer in the left ventricle, and aortic and Swan-Ganz catheters. The animals received either Escherichia coli endotoxin [lipopolysaccharide (LPS), 10 ng.kg-1.min-1; LPS group, n = 10] or an equivalent amount of 0.9% NaCl (sham group, n = 6). Between 1 and 8 h after LPS, a hypodynamic state with low cardiac output ensued (LPS 5.0 +/- 0.2 vs. sham 6.3 +/- 0.4 l.min-1.m-2). During this period, the end-systolic pressure-diameter relationship, a sensitive index of myocardial contractility, was reduced (LPS 10.4 +/- 1.2 vs. sham 17.2 +/- 0.8 mmHg/mm). After this first phase, the sheep developed a persistent hyperdynamic state characterized by a significant increase in cardiac output. By 24 h after LPS administration, the cardiac output was 10.1 +/- 0.5 l.min-1.m-2 (sham 6.3 +/- 0.3). Despite the marked elevation of cardiac output, the end-systolic pressure-diameter relationship had fallen to 8.5 +/- 0.9 mmHg/mm (sham 16.0 +/- 1.2). In a model of hyperdynamic state, an increased cardiac output occurs despite a significant depression in myocardial contractility.

Pulmonary transvascular fluid flux and cardiovascular function in sheep with chronic sepsis.

We studied the mechanisms responsible for the changes in lung lymph flow (QL) in chronic sepsis induced by the continuous infusion of endotoxin [lipopolysaccharide (LPS), 10 ng.kg-1.min-1]. Sheep (n = 11) were studied in the unanesthetized state 7 days after preparation, and cardiopulmonary variables were measured. In the control group (n = 5) given lactated Ringer solution, no significant changes were observed in any measured variables. In the LPS group (n = 6), QL increased from 11.7 +/- 3.8 to 54.0 +/- 15.0 (SE) ml/h 24 h after LPS infusion had begun. This elevation in QL was associated with little or no change (P > 0.05) in reflection coefficient (0.80 +/- 0.03 to 0.87 +/- 0.05) or pulmonary microvascular pressure (14.3 +/- 0.4 to 16.7 +/- 1.2 mmHg). The filtration coefficient, however, was significantly elevated (0.018 +/- 0.006 to 0.083 +/- 0.024 ml.min-1.mmHg-1). In association with changes in QL that occur as a result of LPS administration, there was a significant increase in cardiac index (6.1 +/- 0.5 to 10.2 +/- 0.3 l.min-1.m-2) and a reduction in mean arterial pressure (90.2 +/- 4.4 to 73.7 +/- 7.3 mmHg) and systemic vascular resistance index (1,229 +/- 134 to 583 +/- 62 dyn.s.cm-5.m2), findings similar to those noted in septic humans.

Effect of prostacycline analogue OP-41483 and perfusate on liver preservation by simple hypothermia in rats.

Changes in rat livers preserved by simple cooling immersion in modified Sacks' solution, Ross' solution and Ross' solution with OP-41483, a PGI2 analogue, were compared after 12, 24, 36, and 48 hours. The viability of the preserved livers was assessed by measuring the tissue ATP concentrations and quantifying morphologic changes in the mitochondria observed by transmission electron microscopy using a 4-point scale, the mitochondrial score. A general trend towards progressive mitochondrial degeneration was observed in all groups. After 12 hrs of preservation, there was no significant difference between the groups. From 24 to 48 hours, the mitochondrial scores for livers preserved with OP-41483 were significantly higher than in the two other groups. No difference among the groups in tissue ATP concentration was observed at any time. Thus, OP-41483 was considered to retard mitochondrial degeneration in liver preservation by simple hypothermic immersion and does so by a mechanism not directly related to intracellular energy stores. The possibility that OP-41483 acts by stabilizing the lysosomal membrane is discussed.

Analysis of pulmonary microvascular permeability after smoke inhalation.

We estimated the reflection (sigma) and filtration coefficients (Kf) in a chronic sheep lung lymphatic preparation after smoke inhalation. Group I (n = 7) sheep were insufflated with cotton smoke and group II animals (n = 5) with room air. After inhalation injury, the lung lymph flow increased nearly four times the baseline value by 24 h after injury. There was a concomitant reduction of sigma (0.81 +/- 0.02 to 0.64 +/- 0.02) and elevation of Kf (0.020 +/- 0.002 to 0.042 +/- 0.009 ml.min-1.mmHg-1); pulmonary capillary pressure was also elevated (13 +/- 1 to 17 +/- 1 mmHg). By 48 h postinjury, sigma and Kf returned toward baseline but pulmonary capillary pressure was still elevated. We determined that 34% of the increase in capillary filtration was attributable to increased capillary pressure and 66% to increased permeability 24 h after inhalation, but 48 h after injury, 75% of the increase in capillary filtration was attributable to increased capillary pressure and 25% to increased permeability. We conclude that the lung edema formation following smoke inhalation is the result of marked increases in both capillary pressure and permeability.

[Polymorphonuclear leukocyte elastase (PMNE) levels in surgical patients with postoperative infections].

To study the role of lysozomal enzyme released from polymorphonuclear leukocytes during postoperative infections, plasma levels of PMNE were measured serially in 53 patients who underwent digestive surgery. The patients were divided into two groups, 18 patients with postoperative infections (infected group), and 35 patients without infections (non-infected group). The PMNE levels in the non-infected group elevated during one week with peak value in one postoperative day, but returned to preoperative levels in two weeks. The levels of fibronectin (FN), C3, C4 and antithrombin III (ATIII) were decreased after surgery, but returned within seven postoperative days. On the contrary, the PMNE levels in the infected group remained high two weeks after operations and elevated higher than those of non-infected group before clinical signs of infections. In infected group, the levels of FN and ATIII were significantly decreased two weeks after surgery, compared to those of non-infected group. Correlations were demonstrated between the amount of PMNE and the decrease in the levels of FN and ATIII. It is suggested PMNE might play a role in the pathobiochemical alterations including of acceleration of coagulation during postoperative infections. The measurement of the levels of PMNE in the postoperative course seems to be an useful index to early diagnosis of postoperative infections.

A case of MALT (mucosa-associated lymphoid tissue) lymphoma occurring in the rectum.

A case of mucosa-associated lymphoid tissue (MALT) lymphoma of the rectum is reported. A 64 year-old woman was referred to us for the evaluation of occult blood in the stool. A hard mass was palpable on digital examination. Biopsy specimens revealed a histologic picture compatible with MALT lymphoma. Abdominoperineal excision of the rectum was carried out. Chemotherapy was not performed, and the post-operative course was uneventful, with no evidence of recurrence for 2 years and 11 months. Surgical resection is an effective therapy for MALT lymphoma of the rectum.

Detection of 20q13 gain by dual-color FISH in breast cancers.

An increase in the 20q13 copy number has been reported in a wide range of solid tumors including breast cancers. However, the biological significance of this 20q13 gain has not yet been defined. We examined the 20q13 gain in breast cancer cells by dual-color FISH using two different DNA probes specific for the region of 20q13 and chromosome 20 centromere to investigate the relationship between 20q13 gain and the clinicopathological features of breast cancers. DNA measurement by LSC revealed DNA diploidy in 14 tumors and DNA aneuploidy in 24 tumors. Although the modal number of chromosome 20 was 2 in most of the tumors, the average fraction of cells with the modal chromosomal number was significantly different in the diploid and aneuploid tumors (p < 0.01). A gain in the 20q13 copy number was detected in 9 of 38 breast cancers, 2 of which showed a high-level gain. The gain in 20q13 was associated with negative expression for the progesterone receptor, but it was not linked to estrogen receptor expression. A 20q13 gain tended to be seen in DNA aneuploid and/or scirrhous carcinomas, but the increase in the 20q13 copy number did not affect either the nodal state or the disease stage in this series.
