Myofibrillar protease activity in muscle tissue from patients in catabolic conditions.

Myofibrillar alkaline protease activity was shown to be present in human skeletal muscle. Endogenous myofibrillar proteins and 14C-labelled, exogenous haemoglobin were both active as substrates, and the enzymic activity appeared to be similar to the myofibrillar protease previously described in rodent muscles. The activity of this enzyme was determined in patients undergoing surgery for a variety of diseases. Significant elevations in proteolytic activity were found in the abdominal wall muscle of patients in wasting conditions as compared with non-catabolic diseases. In cachectic patients on total parenteral nutrition the protease activity was similar to reference values. The results imply that increased activity of the myofibrillar alkaline protease plays a role in the development of cachexia in human wasting diseases by prompting degradation of muscle proteins.

Orchidopexy in cryptorchidism assessed by clinical, histologic and sperm examinations.

A study was undertaken of 187 patients with unilateral and 41 patients with bilateral cryptorchidism before and after orchidopexy. The mean tubular diameter and the mean tubular fertility index were used as quantitative criteria for assessment of the development of the testes. Before orchidopexy, no significant differences were found between scrotal and cryptorchid testes in patients up to six years of age. Thereafter, the scrotal testis showed marked development in distinction to the cryptorchid testis. After orchidopexy, follow-up examinations were carried out when the patients had reached the age, at least, 18 years. Based on the results of the mean tubular diameter and the mean tubular fertility index, a reasonable degree of fertility would be expected in both the unilateral and the bilateral cryptorchid testes. However, histologic assessment of spermatogenesis in the unilateral cryptorchid testes after orchidopexy showed spermatogenic arrest in 81 per cent and in all patients with bilateral cryptorchid testes after orchidopexy. In the unilateral orchidopexy patients, in distinction to the histologic assesment of spermatogenesis, sperm counts gave good results in 80 per cent; evidently, in these patients, the source of the spermatozoa was the contralateral scrotal testis. That spermatogenesis was defective in 20 per cent of the patients after unilateral orchidopexy suggests and underlying systemic factor affecting both testes.

Demonstration of rectourinary fistulas by pressure gastrografin enema.

Rectourethral and vesical fistulas in patients with high anorectal atresia can be demonstrated by using a pressure Gastrografin enema through the distal colostomy opening. A technique is described where contrast medium is forced by gentle pressure, filling the fistula and the urethra. This picture is of great value during the repair of these anomalies.

Endesophageal resection of short benign strictures of the esophagus.

A relatively simple operation for the treatment of short esophageal strictures that do not respond to dilatations has been described. This operation was successfully carried out at all levels of the esophagus. The strictures treated were caused by lye burns, peptic esophagitis and previous anastomoses.

Torsion of the spleen with incomplete infarction: case report.

Torsion and infarction of a "wandering spleen" is a rare disease which is often confused with other acute abdominal crises. A correct preoperative diagnosis, when made, has usually been determined by arteriographic studies. A child is described in whom changes in the TcSC scan made a correct diagnosis possible by non-invasive methods.

A method for reducing the incidence of infusion phlebitis.

A departmental routine has been evolved wherein intravenous infusions are administered for a maximum daily period of 12 hours. Exceptions are made for patients with poor peripheral veins. Results of a prospective study of 376 patients for a period of one year demonstrated a markedly reduced incidence of thrombophlebitis, namely, 4.4 per cent in 297 patients treated by the intermittent method versus 20.3 per cent in 79 patients who received continuous intravenous infusion therapy for more than 24 hours.

Spigelian hernia.

Nine cases of Spigelian hernia treated during a period of five years are presented. The literature is reviewed and the importance of recognizing the condition is stressed. The main pathological and clinical features are discussed.
