A history of recurrent vulvovaginal candidiasis (RVVC) was reported by 102 women, while current vulvovaginal candidiasis (VVC) was diagnosed in 83 of the same 996 women. They had all attended two family planning and one youth clinic, respectively. Two women, without RVVC or VVC, matched for age for each case of RVVC, were selected as a comparison group (COMP). Recurrent, but not current VVC, was associated with a history of sexually transmitted disease. Those with current, but not with recurrent, VVC had significantly more often genital warts and bacteriuria (> 10(5) bacteria/ml), but significantly less often bacterial vaginosis than the COMP women. Both VVC and RVVC were inversely correlated to a vaginal flora change with a mixed anaerobic vaginal flora. Those with VVC had a greater number of lactobacilli on vaginal cultures, than those with RVVC and the women in the COMP group. VVC and a history of RVVC both occurred more frequently in women with a lactobacilli-predominated vaginal flora, as compared with those with a flora change with a mixture of anaerobic and facultative anaerobic bacteria.

The effects of drugs on isolated spleen strips from the cod, Gadus morhua, have been investigated. Affinities and intrinsic activities for various agonists were determined from cumulative dose-response curves. Contractions were produced by acetylcholine, methacholine or carbachol. The response to acetylcholine was considerably potentiated by the acetylcholinesterase inhibitor BW 284 C51. Methacholine-induced responses were competitively blocked by atropine (pA2 = 8.2), indicating the presence of muscarinic receptors. Nicotine did not contract the spleen strips. Adrenaline, noradrenaline or phenylephrine contracted the preparations and so did isoprenaline in high concentrations. Competitive blockade of the noradrenaline-induced contraction was obtained with yohimbine (pA2 = 6.1), phentolamine (pA2 = 6.0) and propranolol (pA2 = 3.4), and non-competitive blockade with phenoxybenzamine (pM50 = 7.0), indicating that alpha-adrenoceptors mediate the contractions produced by the adrenergic agonists. Isoprenaline caused a slight relaxation of preparations precontracted with methacholine. This effect was abolished by propranolol suggesting the presence of beta-adrenoceptors. Judged from the pA-values for the competitive antagonist, arterial and capsular/trabecular smooth muscles have slightly different alpha-adrenoceptor properties.

This paper describes a development process concerning the active involvement of staff of different professions in developing and implementing methods for assessment of activities of daily living (ADL) in home-based geriatric rehabilitation. Although a variety of established ADL instruments exist, at the time for this study no I-ADL (Instrumental Activities of Daily Living) instrument suitable for communication among staff members of different professions was available. The specific aim was to test a new I-ADL instrument for interrater reliability. The developmental process resulting in the Measure of Instrumental Daily Activity (MIDA) is described. The instrument comprises 12 I-ADL items, defined on the basis of practical home rehabilitation experience. The study involved 36 clients with impairments, aged 65+ years. Multi-disciplinary interrater reliability was tested by 67 parallel independent assessments during a 3-month period, performed by pairs of raters of different professions. Overall agreement was very good (mean weighted kappa=0.89). The MIDA fulfils the basic requirements necessary for valid I-ADL assessment of elderly clients in community health care. An important quality is the active involvement of all staff in the assessment procedure, facilitating and stimulating the implementation of a general rehabilitative attitude in everyday practice.
A 47-year old man attending at an in vitro fertilization clinic for infertility was diagnosed with congenital idiopathic hypogonadotrophic hypogonadism. No palpable testes and no spermatozoa in the ejaculate were found. Endocrinologically serum FSH, LH and testosterone was undetectable. A retroperitoneal magnetic resonance imaging confirmed the testicular absence. rFSH/hCG treatment was initiated. At four months almond-sized testes had developed and puncture with testicular sperm extraction (TESE) showed occasional immobile spermatozoa. Six months after initiation of treatment occasional mobile spermatozoa in semen were successfully used for intracytoplasmic sperm injection (ICSI) and one oocyte was fertilized and transferred. After 12 months sperm count revealed 10(5) mobile spermatozoa and three oocytes were fertilized. The embryo transfers did not result in a clinical pregnancy. As far as we know, this is the first time that objectively diagnosed testes atrophy could be successfully treated with FSH/hCG.
Twenty-four patients with a history of recurrent urinary tract infection and in whom residual urine was considered to be a factor of importance for chronicity, have been treated for an average of 16 months with 1 g methenamine hippurate morning and evening. No patient had urinary calculus at the commencement of treatment and neither did any patient have an indwelling catheter. In patients without urinary tract infection or in whom abacteriuria was achieved with methenamine hippurate, the number of reinfections was reduced by approximately two thirds compared to periods prior to treatment. No patient was completely free from infection throughout the whole treatment period. However, in no case did bacteria with extensive resistance appear. When urinary tract infection was treated with methenamine hippurate, abacteriuria was achieved in only 6 of 14 patients. It would therefore seem that this agent is only of limited value for treatment of established infection. In the event of manifest infection it would appear appropriate to treat the infection primarily with antibiotics and to use methenamine hippurate for prophylaxis when abacteriuria has been achieved. No patient developed urinary calculus during treatment with methenamine hippurate and no deterioration of renal function or haematological change was observed.
Tortuosity of an artery can disturb fluid mechanics and cause flow separation, which might in turn promote atherogenesis. This study discusses theoretically several quantitative measures of arterial tortuosity and curvature in two dimensions and tests them with computations from digitized femoral arteriograms. When reproducibility, sensitivity to scaling and computational procedure, and agreement between the measures were all taken into account, the total curvature and distance factor were considered the most suitable measures. Significant correlations were found between tortuosity and atherosclerosis measures, but the interpretation of this finding is not straightforward.

Sixty women with genital warts were randomly allocated to treatment with either weekly application of 20% podophyllin solution or self-treatment with 0.5% podophyllotoxin cream twice daily for three days in weekly intervals. After a maximum of 4 treatment cycles a final assessment was carried out after 3 months. Primary clearance after termination of treatment was 82% for podophyllotoxin and 59% for podophyllin solution. After excluding relapses at the 3-month follow-up, final clearance for podophyllotoxin (71%) was significantly better (P < 0.05) than that for podophyllin solution (48%). The total frequency of warts eradicated was 94% with podophyllotoxin and 74% with podophyllin solution (P < 0.001). Local adverse effects were generally mild or moderate. Podophyllotoxin cream provides a mode of easy application for women with external genital warts and had in this study a significantly better effect than podophyllin solution.
