[Conotruncal repair of tetralogy of Fallot under 2 years old].

From October 1985 to September 1990, a repair within the conotruncal portion of the right ventricle-Conotruncal repair-was used for the primary repair of consecutive 34 patients with tetralogy of Fallot under the age of 2 years. There were three summarized points of this maneuver: the first was non-use of the tricuspid septal leaflet for closure of the ventricular septal defect, the second was short patch infundibuloplasty with a large monocusp, and the last was total resection of the infundibular septum. Length of the patch infundibuloplasty was just 30% of the right ventricle length according to our formula: 30% of RV (cm) = 1.28 X BSA (m2) + 1.10. The VSD patch was placed between the pulmonary annulus and the ventricular septal crest, consisting of the membranous flap and the posterior extension of the trabecula septomarginalis. The right ventricular (RV) to systemic arterial pressure ratio was 0.49 +/- 0.15 (N = 33) and central venous pressure was 10.2 +/- 1.9 cmH2O (N = 33) six hours after ICU admission and 8.4 +/- 1.5 cmH2O (N = 7) 18 days after operation. RV end-diastolic volume showed no increase after operation: 105 +/- 31 before repair to 104 +/- 23% of Normal after repair in the same patients. Post-operative intubation period was 14 +/- 8 hours and hospital admission period was 17 +/- 6 days after operation. All patients were in sinus rhythm. The 28% of patients showed incomplete right bundle branch block (RBBB) and other 28% of patients showed complete RBBB.(ABSTRACT TRUNCATED AT 250 WORDS)

[A study of the androgenic activity of anti-androgen].

To investigate the androgenic activity of steroidal and nonsteroidal anti-androgens, the effect of several compounds on some parameters as an androgenic activity was measured in the ventral prostate (VP) and seminal vesicle gland (SV) of castrated male rats. Ornithine decarboxylase (ODC) activity, arginase activity, androgen receptor (AR) in cytosol and nucleus, histological examination and weight of accessory sex organs were used as parameters for androgenic activity. The ventral prostate weight was increased by the administration of not only steroidal anti-androgen, medroxyprogesterone (MPA), chlormadinone acetate (CMA) and cyproterone acetate (CPA), but also estradiol-17 beta(E2) and nonsteroidal anti-androgen, AA 560. However flutamide (Flu) failed to increase VP weight. In SV, the tendency to increase weight by the administration of these compounds was also observed. However, it was not obvious as in VP. It was proven by the measurement of the parameters that all tested compounds except Flu had an androgenic activity. It seems that the androgenic activity was caused by a different mechanism because the parameter response to the administrated compounds was not identical.

Canine heart-lung transplantation after 24-hour hypothermic preservation.

We compared the efficacy of Belzer UW solution (UWS) with modified Collins solution (MCS) in a 24 h canine heterotopic heart-lung transplantation model. Nine pairs of mongrel dogs were divided into 2 groups: MCS group (n = 5) and UWS group (n = 4). The donor heart was arrested with cardioplegia. The heart and lung were flushed with MCS or UWS, excised en bloc and immersed in 4 degrees C MCS or UWS for 24 h. Graft function was adequate in 1 of 5 grafts in the MCS group and in all 4 grafts in the UWS group (P less than 0.05). In the UWS group, the prepreservation and posttransplantation cardiac output was 71.2 +/- 21.6 and 96.4 +/- 44.4 ml/min per kg, the PaO2 was 80.8 +/- 12.9 and 74.1 +/- 2.7 mmHg, and the PaCO2 was 25.6 +/- 5.5 and 35.9 +/- 13.3 mmHg, respectively. No difference was significant. In the UWS group, the pulmonary vascular resistance increased significantly to 13.8 +/- 3.7 from 8.8 +/- 5.2 Wood units (P less than 0.05). Post-transplantation myocardial water content in the UWS group (78.6 +/- 2.0) was less than in the MCS group (81.1 +/- 1.4, P less than 0.05). The wet/dry ratio of the lung increased significantly to 8.3 +/- 1.6 from 4.4 +/- 0.6 in the UWS group (P less than 0.01) and to 10.3 +/- 0.8 from 4.1 +/- 0.1 in the MCS group (P less than 0.01), but was less in the UWS group (P less than 0.05). These results suggest that UWS is more effective than MCS in 24 h heart-lung preservation, but does not prevent pulmonary oedema adequately.

Prolonged preservation of cadaver heart with Belzer UW solution: 24-hour storage system for asphyxiated canine hearts.

The efficacy of Belzer UW solution was compared to Collins' solution in the preservation of asphyxiated cadaver hearts in a canine model. Donor hearts were stored for 24 h: 2 h of in situ hypothermic (15 degrees C) coronary perfusion plus 22 h of simple immersion in ice-cold solution. Verapamil, propranolol and prostacyclin were used for myocytoprotection in both groups. After orthotopic transplantation, all animals were weaned off bypass without inotropic support. After 1 h, however, the cardiac output was significantly higher in the Belzer UW solution group (128 +/- 28 vs. 67 +/- 13 ml/kg/min, p less than 0.01).

[Prognostic factor for prostate cancer. Gunma Urological Oncology Study Group].

The study of prognostic factors in patients with prostate cancer (Pca) may be of value in understanding the natural history of the disease and may also assist in planning and analyzing the results of clinical trials. Moreover some information through this study would be beneficial to assessing the prognosis and decision of better therapy form of individual patients. The significance of items studied was evaluated from the two view points, survival rate and Pca death rate. Stage, pathological differentiation and acid phosphatase were significantly related to them, in the category grade depending manner. Past history and complication, and age were also significantly related to them but higher category grade as a survival factor showed lower Pca death rates. ESR, gait disturbance and hematuria were related to only survival rate. Any significant relationship was not observed in serum testosterone, voiding disturbance and cancer-pain. Prognostic factors should be clinically used through the well understanding of each characteristics. This paper also showed that statistical significance not always provide a wide difference between categories compared.

[The analysis of prognostic factors for prostate cancer by multivariate analysis].

Multivariable analysis (quantification method I and II) was used to investigate the relationship between survival rates and prognostic factors. The items studied were age, stage, pathological differentiation, gait disturbance, acid phosphatase and erythrocyte sedimentation rate (ESR). The data were available for 353 patients for all items. The significance of each item was evaluated in all patients-group, subgroup stratified by their cause of death, and subgroup stratified by their stage. The significance of age and gait disturbance which represent the general condition was proven in all patients, non-cancer death group and stage ABC group. In the cancer death group, stage D group was the most significant as prognostic factor. ESR was the most significant in the stage D group and the significance was gradually increased with the observation time.

[The indication and effects of palliative right ventricular outflow tract reconstruction for tetralogy of Fallot with pulmonary atresia].

Palliative right ventricular outflow reconstruction (RVOT) in 5 cases with tetralogy of Fallot with valvular pulmonary atresia and 5 cases with truncal pulmonary atresia were studied regarding indication and postoperative development of pulmonary artery. In 4 cases with valvular atresia, RVOT was performed following previous palliation such as Blalock-Taussig shunt, and resulted in improvement in clinical symptoms and in significant increase in postoperative PA-index from 105.4 mm2/m2 before operation to 159.4 mm2/m2. In one case, RVOT was performed as the first stage palliation and symmetrical development of pulmonary arteries were observed. All underwent successful total correction later. However, in 5 cases with the truncal atresia, 2 died after RVOT and postoperative development of pulmonary artery was insignificant. RVOT is indicated for cases with poor pulmonary arterial development despite of palliation, and in future, RVOT as an initial palliation will be indicated in infants with hypoplastic pulmonary artery.

[Pharmacokinetic and clinical studies on cefdinir fine granules in children].

A new oral cephem, cefdinir (CFDN) was evaluated for its efficacy, safety and pharmacokinetics in 13 children. The results are summarized as follows. Plasma concentration peaked at 3-4 hours after administration of 3 mg/kg of CFDN with Cmax ranging from 0.57-0.89 microgram/ml except 1 case in which the absorption of the drug was poor. Recovery rates in urine averaged at 13.4%, with a large individual variation. Ten children with 12 bacterial infections were treated with 9 mg/kg/day of CFDN fine granule. Clinical responses were good in 10 patients and fair in 1 patient, with an efficacy rate of 90.9%. No clinical adverse reactions nor abnormal laboratory findings were encountered.

[Morphological and functional analysis of dento-orofacial complex in monozygotic twins with Duchenne type muscular dystrophy].

The form of dento-orofacial complex and masticatory muscle function of monozygotic twins with Duchenne type muscular dystrophy were investigated. They had no environmental difference. Morphological analysis were performed on the dental casts and cephalograms. EMG recordings were derived from the bipolar surface electrodes on the masseter muscle and the anterior belly of digastric muscle on the left side. Each consisted of the data for three years. Results obtained are as follows: 1) Based on the average data, these patients showed an elongated dental arch in the maxilla and mandible, which might be caused by enlarged tongues. There were little difference in the tooth and dental arch sizes between them. 2) Cephalometric findings indicated that the elder brother showed a clockwise rotation of the mandible with larger gonial angle than the younger brother. Both of them showed a larger gonial angle based on the mean values. 3) Analysis of EMG recordings revealed an elongated silent period induced by teeth tapping and chin tapping, and a variable masticatory rhythm compared with that of normal sample. Moreover an annually increased imbalance between masseter muscle and digastric muscle was evident, which were parallel to the change of the blood creatine kinase value. Differences in the form and function of orofacial complex between them might be caused by their polygene heredity and the large size of DMD gene (XP 21).

Experimental aortocoronary artery bypass grafting using a CO2 laser on the dog: acute experiment.

This study was designed to evaluate the feasibility of using laser tissue welding in aortocoronary bypass operation. Simulated aortocoronary bypass operations were performed on 10 dog hearts supported by extracorporeal circulation. Distal anastomoses between internal mammary artery and coronary artery were achieved using the laser technique, which includes four stay sutures of 7-0 polypropylene and tissue welding between the stays by the laser at a power level of 65 mW. Short-term luminal patency was 100% without stenosis by angiography. Microscopy and histology showed that CO2 laser caused medial change resulting in fusion with preservation of normal intimal morphology. The aortocoronary bypass operation using a CO2 laser is technically feasible, and this technique may open up a new method for patients with small peripheral coronary artery obstruction and for pediatric patients with certain conditions (eg, Kawasaki disease).

Characteristic response to taste stimuli of the intensities of higher harmonics in an electrochemical oscillatory system.

In general, the electrochemical characteristics of solid/liquid or liquid/liquid interfaces are highly nonlinear, i.e., the capacitance changes markedly according to the applied voltage. In this paper, we propose a novel method for evaluating these nonlinear characteristics quantitatively. That is, a sinusoidal voltage source is applied to a test solution and the waveform of the output current is analyzed by Fourier transformation. It is shown theoretically that higher harmonic components in the Fourier transformation afford us useful information on nonlinear behavior. It is stressed that our technique is entirely different from the classical impedance method, i.e., nonlinear components of the impedance can be evaluated in our method, having been ignored previously in the classical impedance measurement. As an application of this method, we have studied the effect of taste compounds on the intensities of the higher harmonics, using an electrochemical cell containing an aqueous solution of sodium oleate. It has been found that the intensities of the higher harmonics exhibit characteristic changes upon the addition of taste compounds, the change being dependent upon the taste category. The characteristic response to taste compounds in the electrochemical nonlinearity is discussed in relation to the experimental trend of the dynamic isotherm for oleic acid at an air/water interface.

Human calicivirus-associated diarrhea in children attending day care centers.

We investigated human calicivirus (HCV)-associated diarrhea in children attending day care centers by using stool specimens collected in 1981-1983. We used a screening enzyme-linked immunosorbent assay (ELISA) derived from reagents prepared against the Sapporo strain of HCV and confirmed positive results with a blocking ELISA and immunosorbent electron microscopy. HCV was detected in 11 (2.9%) of 375 diarrheal stools and in none of 86 stools from asymptomatic contacts. This incidence rate was half that noted for rotaviruses and higher than that noted for Campylobacter, Salmonella, and Shigella in the original study. HCV was found in stool specimens from children in nine day care centers; HCV-associated diarrhea was sporadic, occurred with greater frequency in young children, and had a summer-fall predominance. Our results indicate that HCV is an important cause of diarrhea in day care centers and that frozen stool samples can yield epidemiological data on HCV infection.

An outbreak of gastroenteritis associated with acute rotaviral infection in schoolchildren.

In April 1988 a large outbreak of group C rotavirus infection associated with acute gastroenteritis occurred among schoolchildren and their teachers simultaneously at seven elementary schools in Fukui city, Japan. Of 3,102, 675 (21.8%) became ill. Clinical symptoms were mild, predominantly abdominal pain and vomiting, with diarrhea reported in only 27.6%. The outbreak subsided within 2 d. No pathogenic bacteria were found in fecal specimens; the virus particles detected by electron microscopy were morphologically indistinguishable from conventional infantile rotavirus. Immune electron microscopy showed that these virions formed large aggregates with convalescent serum and with the reference serum specific to group C rotavirus. Polyacrylamide gel electrophoresis showed similar RNA patterns for virus from this outbreak and typical group C rotavirus.
