[Ileovesical fistula due to Crohn's disease: a case report].

A 24-year-old male first experienced pollakisuria, dysuria, pneumaturia and diarrhea in May 1988. Intravenous pyelography showed a normal upper urinary tract but bladder wall irregularity at the dome was observed. Cystoscopic examination revealed bullous edema, erythema and presence of a mucous-like substance. Barium enema X-ray examination revealed inflammatory changes at the terminal ileum but no fistulous connection was noted. Mild ulceration was observed on colon fiberscopic examination. At operation, a severely inflamed lower ileum firmly adherent to the dome of bladder as well as to the sigmoid colon was observed. Fistulous communication between bladder and ileum was also noted. Resection of diseased ileum, sigmoid colon and partial cystectomy were carried out. The patient remains well, without enteric or bladder symptoms.

[Transverse ectopia of the testis. A case report].

A case of transverse ectopia of the right testis with left inguinal hernia is reported. A one-year-old-boy was admitted to our hospital with swelling of the left scrotal contents and right undescended testis. At operation, two testes were situated on the left side, one in the left scrotum and another in the left inguinal canal. Right transseptal orchiopexy was performed. Including our case, we discovered 73 cases of transverse ectopia of the testes in the Japanese literature. A brief discussion was made about the disease.

Blood pressure response to a calcium entry blocker in normotensive subjects with or without a family history of hypertension.

The possibility that a familial background of hypertension might influence the blood pressure response to a calcium entry blocker was evaluated in 15 normotensive relatives of patients with essential hypertension and 18 normotensive subjects with no family history of hypertension. Under control conditions, blood pressure, heart rate, plasma noradrenaline, adrenaline, and renin activity did not differ between the two groups. Nifedipine, at a dose of 10 mg administered sublingually, lowered the blood pressure and increased the heart rate, plasma noradrenaline, and renin activity. The normotensive relatives of patients with essential hypertension did not differ in their responses from the normotensive subjects with no family history of hypertension, with the exception of plasma noradrenaline thirty minutes after nifedipine. These results provide evidence to suggest that there is no functional abnormality with increased dependency of vascular smooth muscle tone on calcium influx in the prehypertensive state.

A preliminary pharmacokinetic study of cyclosporine A in 10 Japanese renal transplant patients.

A pharmacokinetic study of cyclosporine A (CyA) was performed in ten Japanese renal transplant patients. The peak blood CyA concentration appeared at about 2.5 h after oral administration of CyA 180-360 mg, and the mean peak CyA concentration was 582 +/- 238 (SD) ng/ml. The apparent clearance was 39.9 +/- 16.9 (SD) ml/min/kg which is almost the same as reported in European transplant patients. The apparent distribution volume was 5.34 +/- 3.05 (SD) l/kg. The wide variations observed both in the apparent clearance and in the apparent distribution volume are ascribed to the wide variation of the area under the curve (AUC). No correlation was found between the oral dose per body weight and peak blood CyA level or AUC in the 10 subjects. However, a linearity was ascertained between blood CyA concentrations and oral CyA doses (250 mg and 500 mg), though in one patient. Especially, in one patient, blood CyA level was under the lower detection limit of our assay method, 50 ng/ml, over 20 h after oral administration of CyA. This study strongly supports the need of the blood monitoring in the CyA therapy after renal transplantation.

[A case of angiomyolipoma originating from polycystic kidney with horseshoe kidney].

A case of renal angiomyolipoma originating from polycystic kidney with horseshoe kidney is reported. A 32-year-old woman visited our hospital with the complaint of proteinuria. with computerized tomographic scan and further examinations the patient was diagnosed as having renal angiomyolipoma with tuberous sclerosis. The tumor originated from a polycystic horseshoe kidney. Three weeks later, she complained of right flank pain and was diagnosed with spontaneous rupture of the angiomyolipoma. Right heminephrectomy was performed and histological examination confirmed the preoperative diagnosis. Some discussion is made on the characteristics and treatment of renal angiomyolipoma, and the statistics on renal diseases with tuberous sclerosis in Japan are presented.

[Mass screening for cervix cancer under the law of health for the elderly in Fukuoka Prefecture].

Mass screening for cancer of the uterine cervix was performed mainly in a screening car or by a gynecological practitioner in Fukuoka Prefecture. In 1983, 114,000 women were screened for cervix cancer, and 102 cervix cancers were detected. Cytology activity index was 110. To reduce deaths from cervix cancer, 400,000 women should be screened by cytology. Detection of cancer in the population with abnormal cytology was not satisfactory in some departments. The gynecologists in these departments should be trained for colposcopic practice. The training of cytotechnologists is also necessary for the elimination of false negative cytology. These trainings are now being done under the law of health for the elderly.

Impaired monocyte function in glycogen storage disease type Ib.

Phagocyte (neutrophil and monocyte) function was evaluated in a boy with glycogen storage disease type Ib. Neutrophils were found to be defective in motility and respiratory burst and monocytes showed a defect in respiratory burst but not in motility. These results suggested that the glucose-6-phosphate transport system plays a role in the function of neutrophils and monocytes and that these two phagocytes are different from each other in their energy metabolism for motility.

Urinary dopamine excretion in normotensive subjects with or without family history of hypertension.

To define the role of the renal dopaminergic system in the pathogenesis of essential hypertension, urinary free dopamine excretion was examined in 23 normotensive subjects who had one or more first-degree relatives with essential hypertension, and also in 36 matched control subjects without any such family history. The group urinary dopamine excretion and urinary sodium excretion were not different. However, a significant urine dopamine-sodium relationship was apparent in the controls but not in the relatives due to relatively high dopamine output in those with lower sodium excretion. The two groups were similar as regards blood pressure (BP), plasma renin activity (PRA), prolactin and catecholamines. These findings demonstrate an alteration in the urine dopamine-sodium relationship in some normotensive subjects with genetic risk of hypertension.

Effect of a calcium entry blocker on blood pressure, plasma renin activity, aldosterone and catecholamines in normotensive subjects.

The effects of the calcium entry blocker nifedipine on blood pressure, heart rate, plasma renin activity, aldosterone, noradrenaline and adrenaline were studied in 23 normotensive subjects in the supine and upright positions. Nifedipine, 10 mg administered sublingually, lowered mean blood pressure and increased heart rate, plasma noradrenaline and renin activity without increasing plasma aldosterone in the supine position. The increase in plasma aldosterone in response to upright posture was inhibited by nifedipine, whereas the rise in plasma noradrenaline was augmented. These results suggest that intracellular calcium is important as a regulator of aldosterone secretion as well as of vascular tone in normotensive subjects.

Cerebro-hepato-renal syndrome of Zellweger. Clinical and autopsy findings and a review of previous cases in Japan.

An autopsy case of cerebro-hepato-renal syndrome of Zellweger, which occurred in a 14-year-old Japanese girl, is reported. The autopsy revealed widely distributed cystic changes in addition to renal blastema of both kidneys, and the liver was cirrhotic. The case was complicated by anomalies such as high forehead, strabismus, and partial defect of chorioidea. So far there have been only 10 reported cases of cerebro-hepato-renal syndrome of Zellweger in Japan.

Elimination characteristics of bredinin from patients serum in hemodialysis.

The hemodialysability of bredinin, a new immunosuppressive drug, was investigated in five patients on longterm hemodialysis. Blood samples were collected periodically during and inter-dialysis and measured by high-performance liquid chromatography (HPLC) for bredinin. The half-life (t 1/2) of the elimination phase was 4.8 h in our patients during hemodialysis, a 97% reduction of the 155 h t 1/2 reported in uremic patients. The fraction of drug in the body which is removed by dialysis (f) calculated averaged 0.43. A mean of 43% of the drug in the body was removed during 4-hour hemodialysis. Because of the reduction in elimination t 1/2 and the significant drug removal during dialysis we conclude that bredinin is dialyzable.
