[The treatment of acute myocardial infarct with perlinganit].

As many as 43 patients with myocardial infarction were under observation. There were 30 men and 13 women aged 36 to 82 years. Of these, 26 patients had large-focal and 17 transmural myocardial infarction. Perlinganit was injected in the form of 0.1% solution by means of a drug-dosing apparatus (DDA-1). The drug infusion lasted 51.1 +/- 4.8 h on the average, the mean infusion rate was 74.5 +/- 3.4 micrograms/min. After 24 hours it was required that in 32 patients the mean infusion rate be increased 1.5-fold. The hemodynamics was assessed with the aid of tetrapolar chest rheography. Well-defined clinical and hemodynamic effects were revealed. In some patients, the clinical signs of left ventricular insufficiency could be eliminated. Contractile function of the left ventricle returns to normal within the first hours of disease. During perlinganit treatment, the antianginal and antiarrhythmic effects were recorded. The data obtained point to the fact that the use of perlinganit for the treatment of acute myocardial infarction is advisable.

[Erythrocyte morphology in anemia after artificial circulation (based on scanning electron microscopic data)].

Erythrocyte stereo-ultrastructure of 58 patients operated under the conditions of extracorporeal circulation was studied by scanning electron microscopy. It is established that after the operation with extracorporeal circulation the hemoglobin level is always decreasing, 3 mg% on the average; hemoglobin decrease up to 10 mg% and less was considered as anemia, this occurring according to the approximate calculations, in 10-11% of cases. The causes of anemia in an early postoperative period are as follows: mechanical trauma of erythrocytes in the system of extracorporeal circulation; disturbance of the osmotic balance; alteration of the erythrocyte membrane plasticity due to the change of its enzyme-lipid component. Anemia in the late postoperative period is due to the purulent inflammatory processes. Morphological substrate of anemia is represented by the iron-deficient, target-like, two-pit erythrocytes, spherocytes, lacerated erythrocytes and other forms.

[Principles of early postoperative management of patients after open-heart surgery].

Proceeding from their personal 5-years experience comprising over 1500 operations with extracorporeal circulation the authors formulate their tactics of the early postoperative therapy. The attention is mainly paid to the maintenance of the optimum pulmonary gas exchange, adequate haemodynamics and stable homeostasis. An important role belongs to prolonged artificial pulmonary ventilation that was used for therapeutic purposed in 324 cases of cardiac and respiratory insufficiency and for preventive purposes--in 908 patients. The therapeutic tactics for various forms of respiratory and cardiac insufficiency and multiform cardiac rhythm disorders is presented in detail. On the example of 149 patients shows that a moderately incomplete blood replacement in the postoperative period does not result in clearsut anaemia. The authors recommend donor blood transfusions when the hemoglobin level is below 10 Gm%.
