Clinical pharmacology and pharmacokinetics of clonidine.

A 300-mug oral dose of clonidine was administered to 5 normal volunteers and measurements of plasma concentration and effects upon blood pressure, heart rate, circulatory reflexes, sedation, and dry mouth were made for the following 8 hr. The plasma concentration rose to a peak of 1.02 +/- 0.52 ng/ml (SD) at 90 min and fell with a mean half-life of 12.7 hr. Blood pressure of the group fell from 111.0/77.0 to 87.2/60.4 after 3 hr and was 95.2/62.2 mm Hg at 8 hr. Heart rate in recumbency was slowed. Marked sedation and a fall in salivary flow followed the same time-course as the plasma concentration. The cold pressor response was reduced but the Valsalva overshoot was little affected.

Lymphocytotoxic antibody detection and crossmatching for renal transplantation.

Sera of 134 recipients awaiting first renal transplants have been tested for cytotoxic antibody by three methods in order to assess the different techniques, and various factors in the techniques have been investigated. The use of only one technique would have failed to detect some of the antibodies found in the sera of 35 patients and it was necessary to use both a 37 C test and the NIH 20 C technique in order to demonstrate them. Results on 402 serial samples from 108 patients indicate the importance, because of fluctuation in antibody levels. of using more than one sample, i.e., previous sample in addition to a fresh specimen from any patient when crossmatching prior to renal transplantation. Lymphocyte suspensions from 44 of 118 cadaver donors were found to be unsatisfactory for accurate cytotoxic crossmatching due to a high background of dead cells. DNase renders such suspensions suitable for use and should be used in such cases. It is felt that if more than one technique were generally used, improved detection of cytotoxic activity would result and would help to resolve present conflicting opinions about the effects of cytotoxic antibody on graft survival. Results are given of 19 patients who had formed cytotoxic antibody prior to renal transplantation in the Unit. Eighteen of these have surviving grafts.

Assessment of antipyrine kinetics by measurement in saliva.

The half-life of antipyrine has been estimated from saliva samples in ten subjects by a gas chromatographic method. Half-life, apparent volume of distribution and total body clearance estimated from saliva and plasma concentrations of antipyrine are not significantly different. The concentration of antipyrine in saliva is independent of flow rate within the range expected in healthy subjects in response to mechanical and sapid stimuli. Antipyrine estimation in saliva could facilitate many areas of phaamacokinetic research limited by the difficulty of obtaining serial plasma samples.

A study of HL-A types in Rh haemolytic disease of the newborn.

The HL-A types of 288 women, immunised to the Rh(D) antigen as a result of pregnancy, have been compared to the HL-A types of 139 women who although they have had Rh(D)-positive children have not produced anti-Rh(D). There is an indication that an increased incidence of the HL-A3 antigen is associated with the immunised mothers. A study of the HL-A types of husbands and children of both immunised and non-immunised women did not suggest that their HL-A types played a significant role in anti-Rh(D) production. The frequencies of the HL-A antigens in each category studied have been tabulated for record purposes, as well as the frequencies in a random panel of persons living in the Newcastle environs. Full ABO, Rh and HL-A details of 50 immunised mothers and their families and of 58 non-immunised mothers and their families are available.

St.Agnes--the first year as the team sees it.

The St. Agnes Community Health Centre was established in October 1974, in the rapidly growing area of Tea Tree Gully, South Australia. It was built to provide additional facilities to an already existing comprehensive medical centre. The embryo centre is described during the first year of operation through the eyes of the health team.

A clinical, pneumoencephalographic and radioisotopic study of normal-pressure communicating hydrocephalus.

Human serum albumin labelled with iodine-133 or technetium-99(m) was injected by the lumbar or cisternal route into patients suspected of having communicating hydrocephalus, and scintigrams were performed up to 24 hours after injection.The CSF isotope studies were shown to be a valuable adjunct to clinical examination and pneumoencephalography in the diagnosis of hydrocephalus. This was especially true in suspected cases of "normal"-pressure hydrocephalus where there may be considerable uncertainty as to which patients with normal pressure and enlarged ventricles will benefit from a shunting procedure. The CSF isotope study provides useful information to the clinician in differentiating patients with symptomatic hydrocephalus from the larger group with dementia, cerebral atrophy and hydrocephalus ex vacuo.

Rhesus isoimmunization after abortion.

Out of 177 Rh(D)-negative patients studied, 96 were successfully followed-up after spontaneous or therapeutic abortion. Rh antibodies were detected by the indirect Coombs test in two patients and by an enzyme technique only in a further seven, an overall incidence of 9.4%.The prophylactic use of anti-D immunoglobulin is now recommended for Rh-negative non-immunized patients undergoing abortion, but the dose could be less than 75 mug. The Kleihauer test was of no value in predicting the risk of isoimmunization.
