High-dose rate intracavitary therapy for cervical cancer with a microSelectron: a preliminary report.

Our early experience using microSelectron HDR to treat cervical cancer patients at Osaka University Hospital is presented. From June 1991 through December 1992, a total of 20 patients (stage Ib, 6; stage IIa, 2; stage IIb, 5; stage IIIb, 6 and stage IVa, 1) with previously untreated invasive uterine cervical cancer and intact uterus were treated with high-dose rate intracavitary therapy administered with a microSelectron. For the treatment, a standard rigid applicator made of stainless steel for a microSelectron was used. Twenty to 30 Gy of external irradiation was administered to the whole pelvic field and 30 to 20 Gy to the central shielded field (total 50 Gy/5-6 weeks) for patients in stage II-IVa. For stage Ib, 40 Gy was delivered to the central shielded field. The 192Ir source had an activity of 370 GBq as of the measuring time. Source loading corresponded to the Manchester System for cervical cancer. Thirty-two or 30 Gy was administered at point A in four fractions over four weeks of intracavitary irradiation. Early primary tumor responses for all patients were complete. There have been two local recurrences in stage IIb and IIIb patients. Three patients in stages Ib, IIb, and IIIb developed para-aortic lymph node metastases, and two of them died from generalized metastasis. No acute radiation injury has been observed. One patient in stage IIb developed subileus five months after irradiation. From our early experience, it is concluded that microSelectron HDR can be used for cervical cancer patients as safely and effectively as our previously used high-dose rate machine.

Mid-dose rate intracavitary therapy for cervical cancer with a Selectron: a preliminary report.

Our early experience with Selectron MDR in treating cervical cancer patients at Osaka University Hospital is presented. From May 1991 through December 1992, a total of 22 patients (stage Ia, 1; stage Ib, 3; stage IIa, 1; stage IIb, 2; stage IIIb, 13 and stage IVa, 2) with previously untreated uterine cervical cancer and intact uterus were treated with mid-dose rate intracavitary therapy administered with a Selectron. A rigid applicator made of stainless steel for the Selectron was used for the treatment. The 137Cs source had an activity of 1.48 GBq as of reference time. Source loading corresponded to the Manchester System. Early tumor responses for all patients were complete. No acute radiation injury has been observed. There have been two local recurrences in stage IIIb patients. One of them developed para-aortic lymph node metastasis and died from distant metastasis. Another patient in stage IIIb had para-aortic and left supraclavicular lymph node metastasis and died from distant metastasis. Four patients developed rectal bleeding (grade 1, 3; grade 3, 1). One of them had been treated for aplastic anemia with steroid. The cause of grade 3 rectal bleeding was considered to be technical failure in intracavitary application. The remaining two patients recovered without treatment. From our early experience, it is concluded that Selectron MDR can be used for cervical cancer patients as safely and effectively as our previously used high-dose rate machine.

Effect of hepatocyte growth factor on radiation response of HeLa, V79, CHO and primary cultured parenchymal hepatocyte in vitro.

Hepatocyte growth factor (HGF) is a multipotent cytokine enhancing regeneration of injured organs as liver, kidney and lung after injury. HGF enhances proliferation of various type of cells, inhibits proliferation of carcinoma cells, enhances motility of epithelial cells. We examined three cell lines (CHO, HeLa, V79) and primary cultured normal rat parenchymal hepatocytes to determine the effect of HGF on radiation response. HGF diminished survival of CHO and V79 cells determined by colony formation assay, whereas no significant change of survival was found in HeLa cells. No synergistic changes of survival were found when these three cell lines were irradiated with the addition of HGF. Thus, HGF did not enhance the radiation effect. We also analyzed the impact of irradiation with HGF on primary cultured normal rat parenchymal hepatocytes. At first, the release of glutamic-oxaloacetic amino-transaminase (GOT) in the supernatant was estimated. Irradiation (40 Gy) with or without HGF did not change GOT release in acute phase by 4 days after irradiation compared with the unirradiated control. Second, the DNA synthesis of rat parenchymal hepatocytes was analyzed using radioactive iodine-labeled deoxyuridine incorporation. HGF counteracted the suppression of DNA synthesis induced by irradiation. Thus, HGF may act as a mitogen even for irradiation-damaged normal cells.

CT diagnosis of pleural dissemination without pleural effusion in primary lung cancer.

We retrospectively reviewed the CT scans of 25 primary lung cancers with disseminated pleural nodules or minimal malignant pleural effusions that were not recognized preoperatively. Special attention was devoted to abutting interlobar fissures, thick major fissures, and disseminated nodules on the chest wall, the diaphragm, and in the interlobar fissures. Among 10 primary tumors abutting interlobar fissures, nine (90%) had at least one of these findings. Among 15 primary lung tumors which did not abut interlobar fissures, four (27%) had at least one of these findings. We conclude that CT is a useful modality for detecting the pleural dissemination of primary lung cancers when primary lung cancers abut interlobar fissures even if no pleural effusion is detectable on CT.

Symptomatic relief for patients with osseous metastasis treated with radiation and methylprednisolone: a prospective randomized study.

A multi-institutional prospective study to evaluate the relief of symptoms for patients with osseous metastasis treated with irradiation and methylpredonisolone was performed. A total of 38 patients with osseous metastasis, treated between January 1991 and December 1992, were included in this study. Patients were randomly allocated to one of two treatments: radiation alone (RT alone) (n = 18) or radiation combined with methylpredonisolone (RT + MP) (n = 20). Symptoms were scored by patients themselves using original Quality of Life (QOL) scores. Stratification by urinary hydroxyproline/creatinine ratios (Hypro/Cr) showed no significant differences in QOL scores between RT and RT + MP in the group with good prognosis, which showed a lower Hypro/Cr (< 3.6 mg/g) during treatment. Following treatment, the score for both groups improved. On the other hand, in the group with poor prognosis, with a higher Hypro/Cr (> or = 3.6 mg/g), a significantly greater improvement in QOL scores was observed for the RT + MP group than for the RT alone group (p < 0.05). There was also a marked difference in the RTOG pain scores, evaluated by physicians before treatment and at the end of treatment, between the two treatment groups and the prognostic groups divided according to Hypro/Cr. No correlation was seen at initial presentation between urinary Hypro/Cr and serum tartrate-resistant acid phosphatase (TrACP). In conclusion, the treatment regimen of RT + MP is recommended for patients with osseous metastasis with poor prognosis who show relatively high Hypro/Cr values. Radiotherapy was equally effective in terms of RTOG pain score for both the poor and good prognostic groups. Serum TrACP was not useful for determining the prognosis of our patient groups.

Corticobasal degeneration: MR with histopathologic comparison.

The purpose of this study was to illustrate the MR findings of corticobasal degeneration and to compare those findings with pathologic specimens. MR findings of atrophy in the perirolandic gyri, atrophy of the basal ganglia, and T2 prolongation in the posterolateral putamen are useful evidence supporting the clinical diagnosis of corticobasal degeneration.

Intra-arterial digital subtraction angiography with extra-large fields using a computed radiography system in evaluating peripheral vascular disease.

The authors evaluated the usefulness of intra-arterial digital subtraction angiography (DSA) with extra-large field sizes using a computed radiography (CR) system in evaluating peripheral vascular disease (PVD). Intra-arterial DSA using CR was performed in 55 patients with suspected PVD. A 4 F catheter was advanced into the abdominal aorta via the transbrachial approach and 90 ml of contrast medium was injected during six exposures using a long leg changer mounting a total of 18 imaging plates on six surfaces. Visualization of the superficial femoral and popliteal arteries was judged as diagnostic in all cases. The abdominal aorta, iliac arteries, and subtrifurcation were also visualized in most cases, but visualization was suboptimal in some cases. As a complication, median nerve palsy occurred in one case. This technique is thought to be a useful method for evaluating PVD because of the advantages of a large field of view and wide exposure latitude, in spite of its relatively long processing time.

Renal cortical retention of contrast medium after angiography as assessed by delayed CT: a multivariate analysis.

Renal cortical retention (RCR) of contrast medium discovered by delayed X-ray examination is sometimes reported in patients with problems in the urinary system. However, we frequently found RCR even in patients with normal renal function. Therefore, we examined the incidence and factors involved in RCR by delayed computed tomography (CT) 12-24 hours after angiography in 168 patients. RCR was found in 80 of 168 cases (48%). Ioxaglate (60%) and iohexol (60%) showed higher incidences of RCR than diatrizoate (37%) and iopamidol (37%). Multivariate logistic regression analysis was performed to determine the predisposing factors of RCR. Dose of administered contrast medium by body weight (p = 0.004), age (p = 0.009), sex (p = 0.013), type of contrast medium (p = 0.003), serum albumin (p = 0.011), and serum creatinine (p = 0.002) were identified as significant and independent predisposing factors of RCR. We suggest that RCR is not a rare phenomenon if delayed CT is carried out.

Neural network analysis of breast cancer from MRI findings.

PURPOSE: To evaluate how much the experience of radiologists affects the performance of an artificial neural network (ANN) trained by two highly experienced radiologists. MATERIALS AND METHODS: Before biopsy two experienced radiologists reviewed the MR images of 100 adult patients with suspicious breast lesions and evaluated their findings based on six features. This database was then used to train a three-layered feed-forward neural network. The network's generalizing ability was then tested to predict the outcome of biopsy in 56 new patients' records which were extracted by 10 participating radiologists. The MRI findings of each reader were presented to the ANN to evaluate the effect of various levels of experience on the output of the ANN. The performance of the ANN was then compared with that of attendant physicians in terms of sensitivity, specificity, and accuracy as well as ROC analysis. RESULTS: The best ANN outcome offered a correct diagnosis in 40 of 41 of the patients with malignant breast cancer and 10 of 15 with benign entity presented in the testing set. The output of the trained ANN outperformed the attendant radiologists with low levels of experience and showed comparable performance with radiologists with higher levels of experience. CONCLUSIONS: The ANN is able to work as a backup system to assist radiologists in the diagnosis of breast cancer.

Adaptive spatial filtering technique for storage phosphor radiography in portable chest radiographs: parameter optimization.

PURPOSE: To investigate the optimal parameters of adaptive spatial filtering (ASF) in storage phosphor radiography for processing portable chest radiographs. MATERIALS AND METHODS: Two types of weighting factor curves starting at 383 digital pixel values (PV) (type B) and at 511 PV (type C) were selected for the optimization of ASF. The PV of 27 areas of apparent pulmonary lesions and seven retrocardiac areas were measured for original unprocessed and ASF-processed images. Three radiologists compared 30 ASF-processed portable chest radiographs with apparent pulmonary or pleural lesions with unprocessed images. RESULTS: PV measurements revealed no significant change in pulmonary densities in type B, and an increase in PV of lower pulmonary densities in type C. Densities of retrocardiac areas were more enhanced in type C than in type B. Observer testing showed that pulmonary densities were evaluated as unchanged in 90% of type B images and 76% of type C images. Changes in mediastinal densities were evaluated as adequate in 80% of type B and 90% of type C images. CONCLUSION: The starting point of the weighting factor curve of ASF in portable chest radiographs should be set the same as in chest radiographs in the upright posteroanterior position with high kVp.
