Micro-quantitative determination of ciprostene in plasma by gas chromatography-mass spectrometry coupled with an antibody extraction.

A simple and highly sensitive method has been developed for the determination in plasma of ciprostene, 9 beta-methyl-6 alpha-carbaprostaglandin I2, using gas chromatography-mass spectrometry following solid-phase extraction on an immobilized antibody column. The anti-ciprostene antibody obtained from rabbit serum was coupled to an agarose support matrix, and the immobilized antibody thus prepared was used as an extraction phase for sample clean-up. The extracted drug was treated with pentafluorobenzyl bromide followed by bis(trimethylsilyl)trifluoroacetamide. The derivative was quantitatively analysed by negative-ion chemical ionization gas chromatography-mass spectrometry. The lower limit of quantitation was 50 pg/ml when 1 ml of human plasma was used. The plasma concentration of ciprostene in a dog treated with ciprostene at 2.5 micrograms/kg was determined successfully by this method.

Shaping and reshaping of salmonid genomes by amplification of tRNA-derived retroposons during evolution.

Three families of tRNA-derived repeated retroposons in the genomes of salmonid species have been isolated and characterized. These three families differ in sequence, but all are derived from a tRNA(Lys) or from a tRNA species structurally related to tRNA(Lys). The salmon Sma I family is present in the genomes of two species of the genus Oncorhynchus but not in other species, including five other species of the same genus. The charr Fok I family is present only in four species and subspecies of the genus Salvelinus. The third family, the salmonid Hpa I family, appears to be present in all salmonid species but is not present in species that are not members of the Salmonidae. Thus, the genome of proto-Salmonidae was originally shaped by amplification and dispersion of the salmonid Hpa I family and then reshaped by amplification of the Sma I and Fok I families in the more recently evolved species of salmon and charr, respectively. We speculate that amplification and dispersion of retroposons may have played a role in salmonid speciation.

[Kinetic estimation of infused Lipiodol within a hepatoma using a CT dynamic study].

Distribution of Lipiodol within hepatic tumor was evaluated using serial computed tomography. Imagistic Lipiodol transition into the tumor via hepatic artery was slower than that of water soluble contrast medium, spreading to central part first and turn to peripheral part of the tumor. This method was thought to be useful to clarify Lipiodol distribution in hepatic tumor and to decide the method or necessity of additional trans-arterial therapy.

A new method of hernioplasty for adult groin hernias centering on repair of the transversalis fascia.

A new method of hernioplasty for adult groin hernias centering on repair of the transversalis fascia was assessed by a questionnaire conducted every five years on adult patients who had received the above operation. The efficacy of this method was evaluated in terms of the postoperative quality of life and prevention of recurrence of groin hernias. Over the last 9 years, 834 adult patients with 921 groin hernias, including bilateral lesions, have undergone the above procedure and overall, favorable results were obtained, with only one case of recurrence (0.1 per cent) being reported. Contralateral herniation, which has been often reported following Bassini's or Hatakoshi's method, was found in only one of our patients although the number of patients who complained of pain in the operated region was comparatively high, being 51 cases or 11.4 per cent.

Synthesis of halogen-substituted 1,5-benzothiazepine derivatives and their vasodilating and hypotensive activities.

In an attempt to improve the effectiveness and duration of the action of diltiazem (1), a 1,5-benzothiazepine calcium channel blocker, its derivatives (2) with halogen substituents on the fused benzene ring were synthesized. These compounds were evaluated for their effects on vertebral and coronary blood flows and antihypertensive activity. The structure-activity relationships are discussed. The 8-chloro derivative ((+)-2b), the most potent compound in this series, was selected for clinical evaluation as a cerebral vasodilating and antihypertensive agent.

[Surgical resection of cardiac metastasis of thyroid cancer--a case report].

A case of metastasis of thyroid cancer into the cardiac cavity is described. A 73-year-old female suffered from sudden chest pain, dyspnea and dizziness and admitted to a local clinic on emergency. A diagnosis of bilateral multiple pulmonary infarction was made by lung perfusion scintigraphy. However, a mass migrating between the right atrium and the right ventricle across the tricuspid valve was demonstrated by echocardiography, and the patient was referred to us under a diagnosis of myxoma complicated with pulmonary infarction. Incision of the right atrium disclosed a gelatinous mass resembling frog eggs, attached to the right ventricle side of the anterior cusp of the tricuspid valve and swinging across the orifice of the valve. The gross lesion was removed completely. Since the histological diagnosis was clear cell carcinoma, no surgery was performed on the pulmonary artery. Postoperative re-examination by ultrasonography and CT revealed thyroid cancer as the primary lesion.

[An effect of priming task repetition on memory processes in an epileptic with amnesia].

The purpose of this paper is to investigate memory processes in an epileptic with amnesia. Selective impairment of memory in organic amnesia has been reported in some clinical observations. They also demonstrated that organic amnesia can retain some aspects of learning experience, despite their inability to recollect them. We investigated an effect of priming task repetition on memory processes in a female with organic amnesia aged 28 (probable encephalitic patient) and eight control college students aged 20-26. In this paper, her memory processes were analyzed, using the same picture priming task, carried twelve times during the period of seven months. We used semantic priming paradigm. Three types of prime-target pair were used: identical, semantically related and unrelated. After a prime picture, a target picture appeared on the CRT display. She was asked to name each target as rapidly and accurately as possible. We analysed reaction time (RT) in naming the picture targets, error rate, and verbal reports of the priming task in the pre- and post-examination interview. In the first examination, priming effect was observed in her RT as in the controls'. This suggests that she retained some amount of semantic memory, and the structure of her semantic memory closely resembled that in the controls. Naming RT reduced and error rate decreased with repetition of the examination. This suggests that she acquired picture naming skill during early few examinations. However it was difficult for her to recognize and recollect the picture stimuli in this period. Further, she did not show any episodic memory of the examinations. There was dissociation between improved RT performance and poor verbal reports during early few examinations. When the same priming task was repeated in early few examinations, the priming effect became weaker in her RT. In the controls, on the contrary, the priming effect remained during the whole task repetition. Semantic strategy was apparently used in the controls, but not in the amnesic patient. She apparently used strategy of non-semantic association between the prime and target. In the last few examinations, her naming RT delayed and the priming effect was observed again as in the first examination. She seemed to use semantic strategy. In addition to this priming effect, she re-collected a few pictures and showed some episodic memory of the examination.(ABSTRACT TRUNCATED AT 400 WORDS)

Sodium Stimulates Growth of Panicum coloratum through Enhanced Photosynthesis.

A sodium-requiring C(4) plant, Panicum coloratum Walt. cv Kabulabula, was grown with and without sodium. Rate of nitrogen uptake and photosynthesis were measured during the recovery from sodium deficiency. The beneficial effect of sodium on growth was apparent irrespective of nitrogen source, ammonium- or nitrate-nitrogen. The leaf photosynthetic rate ((14)CO(2) fixation) doubled by sodium within 1 hour of the application.

[Long term results of skeletal profile changes occurring from chin cap therapy of Japanese female skeletal Class III cases].

UNLABELLED: This cephalometric study was made to investigate the effects of chin cap therapy. The subjects were 21 Japanese females of skeletal class III who were treated without tooth extraction. The periods before treatment, during improvement of anterior teeth and after treatment were compared to see where and how effective the therapy had been. RESULTS: 1. Characteristics before the treatment period: These cases had backward position of the maxilla and forward position of the mandible with over growth. In the denture pattern there was a remarkable labioaxiversion of U-1. 2. Characteristics after the treatment period: There was improvement in the backward position of the maxilla. In the forward mandible, once these cases improved their position by improving the overjet, there was a regression followed by gradual forward growth as treatment continued. The denture pattern showed labioaxiversion of U-1 as before. 3. Before the treatment period, these cases showed large Go-Po and Ar-Go in the dimensional linear analysis. After treatment, these analysis showed normal sizes.

New biological properties of pyrroloquinoline quinone and its related compounds: inhibition of chemiluminescence, lipid peroxidation and rat paw edema.

Pyrroloquinoline quinone (PQQ) inhibited the chemiluminescence (CL) from mouse peritoneal cells initiated by zymosan, carrageenin and N-formyl-methionyl-leucyl-phenylalanine and CL generated by the xanthine-xanthine oxidase reaction and the lipid peroxidation in the rat brain homogenate. The inhibitory activity of PQQ was more potent than that of idebenone, alpha-tocopherol and ascorbic acid in all the three assay systems. In the xanthine-xanthine oxidase reaction, PQQ had no effect on the formation of uric acid at the concentration of CL inhibition. These results suggest that PQQ might have a radical scavenger-like activity. Structure-activity relationship of PQQ and its six related compounds showed that the 7- and 9-carboxyl groups of PQQ as well as the orthoquinone structure are responsible for the radical scavenger-like activity. In addition, the -NH group in the pyrrole ring of PQQ seemed to be essential for the antilipid peroxidative activity in the rat brain homogenate. When administered i.p., PQQ inhibited the development of 0.1% carrageenin-induced paw edema in rats. These results suggest that PQQ might have therapeutic effects on various diseases, of which development or exacerbation has been known to be associated with radical oxygens.

[Successful repair of a cardiac lipoma of the left ventricular free wall--a case report].

We recently encountered a patient with a very rare type of cardiac lipoma, which occurred at the left ventricular free wall and grew into the left ventricular cavity. No surgical treatment of this type of cardiac lipoma has been reported in literature. The patient was a 63-year-old woman who visited our hospital with a chief complaint of palpitation. At the outpatient clinic of our hospital, a tumor of the left ventricle was disclosed by echocardiography. After hospitalization, the patient was diagnosed as cardiac lipoma by CT scans and MR images. She was operated as follows. The apex of the left ventricle was incised and the cardiac cavity was exposed. The lipoma had its root approximately in the center of the posterior free wall of the left ventricle and had grown into the left ventricular cavity with a size of 2.5 X 2.5 X 1.8 cm and a weight of 2.8 g. After removal, the postoperative course was uneventful, but the chief complaint (palpitation) still persisted. Postoperative Holter electrocardiography disclosed that the patient had suffered from sick sinus syndrome and that palpitation had not originated from the cardiac lipoma.

[Study on CAZ levels in serum, urine and myocardial tissue during and after open-heart surgery with cardiopulmonary bypass].

Ceftazidime (CAZ) was administered to 10 patients who underwent open heart surgery using an artificial heart and lung (AHL), and the concentrations of the drug in the blood, urine, filtrate and cardiac muscle were measured. Following anesthesia induction, patients were injected intravenously with 2 g CAZ, and then 2 g CAZ was injected initially into the filling solution of the AHL followed by 1 g every 1 hr. After the surgery, patients received 1 g CAZ intravenously followed by the same dose every 12 hr till Day 5 of the surgery. Results obtained were as follows: 1) The concentration of CAZ in the cardiac muscle averaged 41.2 +/- 12.3 micrograms/g, and the ratio to the concentration in the plasma averaged 0.61 +/- 0.12, being considered to be reasonable. 2) The change in the mean plasma concentration of CAZ during extracorporeal circulation was maintained within the range of 109.7 to 337.0 micrograms/ml, and the concentration increased slightly with the repeated injection. In 2 cases, the plasma concentration was transiently more than 400 micrograms/ml, however, no side effect was observed. 3) The mean plasma concentration of CAZ after surgery attained 214.4 +/- 38.9 micrograms/ml, and half-time averaged 2.8 hr. 4) The excretion rates of CAZ in urine and filtrate by the termination of the extracorporeal circulation were 30.6 and 5.2%, respectively, thus the excretion of the drug in filtrate was lower than that in urine. The total excretion rate in both urine and filtrate for 6 hr after surgery was 55.3%. 5) Neither postoperative infection nor side effect were observed in all cases. CAZ is considered to be a useful antibiotics in the cardiac surgery management.

Neurofibromatosis with spinal paralysis due to arteriovenous fistula.

A case of neurofibromatosis with spinal paralysis due to arteriovenous fistula is reported. Preoperatively, the appropriate diagnosis was missed because angiography had not been performed. Postoperative angiography disclosed that an arteriovenous fistula formed a tumor-like mass, and that a part of the mass had invaded the spinal canal, compressing the spinal cord. It is concluded that the possibility of an arteriovenous fistula should be kept in mind in neurofibromatosis patients with spinal cord symptoms.

Thienylpyrazoloquinolines with high affinity to benzodiazepine receptors: continuous shift from inverse agonist to agonist properties depending on the size of the alkyl substituent.

2-(5-Alkylthien-3-yl)-(1),2-(4-alkylthien-2-yl)-(2), and 2-(5-alkylthien-2-yl)-2,5-dihydro-3H-pyrazolo[4,3-c]quinolines (3) were prepared in four steps starting from ethyl 4-chloroquinoline-3-carboxylate (4) and hydrazinothiophene-carboxylates 5, 8, and 9. All the assayed compounds possessed high affinities for benzodiazepine receptors (Ki = 0.3-2.6 nM). The activities of agonists and inverse agonists were assessed on the basis of inhibition or facilitation of pentylenetetrazole-induced convulsions, respectively. Introduction of alkyl groups of different sizes into the unsubstituted inverse agonistic compounds results in a corresponding shift in the activity from an inverse agonist to an antagonist to an agonist. The susceptibility of such a shift increases in the order of 1 less than 2 less than 3. This tendency may be explained by slight differences in the geometry of the alkyl substituents among the three series.

Endothelium-dependent calcium-induced relaxation in the presence of Ca2+-antagonists in canine depolarized coronary arteries.

1. We examined the mechanisms underlying Ca2+-induced relaxation in the presence of clentiazem, a new Ca2+-antagonist, in depolarized coronary arteries of the dog. 2. Ca2+ (3 x 10(-5)-3 x 10(-3) M) caused an unexpected relaxation in the presence of a high concentration of clentiazem (10(-6) M) in coronary, but not in mesenteric or renal arteries. 3. The Ca2+-induced relaxation was also observed in the presence of established Ca2+-antagonists such as diltiazem (3 x 10(-6) M), nifedipine (3 x 10(-8) M) and verapamil (3 x 10(-6) M). 4. The Ca2+-induced relaxation was inhibited by removal of the endothelium, treatment with oxyhaemoglobin (1.5 x 10(-6) M) or methylene blue (10(-5) M), but not by treatment with indomethacin (5 x 10(-6) M). 5. The Ca2+-induced relaxation was observed in an endothelium-denuded coronary artery segment when closely apposed to an endothelium-containing segment of coronary or mesenteric artery. 6. These results suggest that Ca2+-induced relaxation in the presence of high concentrations of Ca2+-antagonists is mediated through endothelium-derived relaxing factor (EDRF). In addition, Ca2+-antagonists do not affect the Ca2+-influx necessary for the release and/or synthesis of EDRF.

Transformation suppressor genes.

Flat revertants with reduced malignancy in vivo can be isolated from Kirsten sarcoma virus-transformed NIH3T3 cells following transfection with a normal human fibroblast cDNA expression library. We have recovered from one such revertant a 1.8 kb cDNA clone, "Krev-1", that possesses revertant-inducing activity in transfection assay. The Krev-1 cDNA has the capacity to encode a protein with a calculated molecular weight of 21,000 having strong structural similarity to ras proteins (ca. 50% homology), especially in their GTP/GDP-binding, effector-binding and membrane attachment domains. Point-mutations of Krev-1 protein at the 12th and the 59th amino acid positions from the aminoterminus potentiated the transformation suppressor activity by 2-5 times, and point-mutations within the putative effector-binding domain and at the C-terminal acylation site reduced the activity.

[A case of massive hepatoma which responded to SMANCS/Lipiodol regimen with intra-arterial infusion].

Transcatheter arterial chemotherapy (SMANCS/lipiodol) was applied to massive hepatoma, which had a high AFP 213,000 ng/ml, A-P shunt, tumor thrombosis and metastatic lung cancer. After 3 months, the AFP value reduced to 18 ng/ml, massive hepatoma and the A-P shunt disappeared, but AFP-negative nodular hepatoma recurred around initial hepatoma. Each time, we injected SMANCS/lipiodol to the recurring hepatoma. The therapy in the initial stage was not so effective. The portal vein was not observed in the initial stage, but appeared after the second dosage. Metastatic lung cancer was declining in the initial dosage and 23 months later disappeared after the third dosage. The massive hepatoma occupied entirely the rt. lobe of the liver. The patient lived for 4 years, had total admission periods of 190 days and could return to life in society. In this case, we considered that transcatheter arterial chemotherapy (SMANCS/lipiodol) had remarkable effects.

[A successfully operated case of isolated tricuspid insufficiency due to traumatic laceration of anterior leaflet of tricuspid valve].

A case of isolated traumatic tricuspid insufficiency is presented. The patient was a 26-year-old designer with chief complaints of palpitation and dyspnea on exertion, who had the traffic accident three years and four months ago. The cardiac catheterization and two-dimensional echocardiography showed isolated insufficiency, and revealed that the basic damage was laceration of anterior leaflet of tricuspid valve itself. Tricuspid valve was replaced with SJM M-31 valve. His postoperative course was satisfactory. Until present to our knowledge, 13 cases of traumatic tricuspid insufficiency have been reported in Japan including our case. In three instances the papillary muscles ruptured. In nine patients the chordae tendineae were torn and in this case, there was primary laceration of anterior leaflet.
